Background: Measures to contain the COVID-19 transmission reached teaching routines of universities worldwide with possible mental health consequences for anxiety. This study assessed prevalence and risk factors for stress, depression, and anxiety (SDA) in medical students during quarantine by COVID-19. Methods: A cross-sectional observational study of medical students by means of the DASS-21 questionnaire. Risk factors for SDA were assessed based on epidemiologic questions related to COVID-19. Receiver Operating Characteristics (ROC) curves were calculated for each predictor, as well as sensitivity and specificity. Results: This survey reached 1009 responses. A prevalence of 77.5% for some SDA disorder was found, 63% being severe. Previous diagnosis of psychiatric disorder was a factor of risk for anxiety (OR 2.78 CI95% 1.44 - 14.25, p = 0.044), as well as for depression (OR 3.37 CI95% 1.98 - 6.02, p Conclusion: Psychiatric conditions as well as chronic illnesses were risk factors for high prevalence of anxiety, depression and stress during the COVID-19 pandemic among medical students.

Nurse Mentor Training Program to Improve Quality of Maternal and Newborn Care at Primary Health Centres: Process Evaluation

Quality of maternal and newborn care could be improved if health care providers’ knowledge and competencies as well as system level constraints are addressed. However, due to several barriers staff nurses who form the frontline of health care workforce have limited access to enhancing their clinical knowledge and competencies. To address this gap, a new cadre of nurse mentors (NMs) for the public health system were trained by specialists from a teaching hospital in a special 5-week training course. This included 54 hours of theory and 110 hours of practical in clinical obstetric and newborn care, apart from mentoring, quality improvement and health systems issues. The nurse mentors were assigned to support staff nurses in the primary health care centres (PHCs) in eight northern Karnataka districts. Each NM covered 6-8 PHCs monthly for 2 - 3 days and thus a total of 385 PHCs were reached. They received support in the field through supportive supervision visits done by the specialists who had trained them, as well as by refresher training and clinical postings to the district hospitals. This paper presents impact of the training program on change in immediate and long term knowledge and competency scores of nurse mentors. Their baseline knowledge scores changed from 44.3 ± 12.7 to 72.1 ± 13.8 immediately after the training in obstetric and from 18.2 ± 19.1 to 66.4 ± 14.9 in newborn (p p p > 0.05). Skills score soon after training increased from 62.2 ± 13.2 to 69.6 ± 12.5 in obstetric after a 1 year period and from 52.6 ± 9.3;63.5 ± 14.4 in newborn (p < 0.001) content areas respectively. These findings have implications for those interested in improving quality of maternal and child care through nurse-dependent health delivery systems.

Semi-Supervised Medical Image Segmentation Based on Generative Adversarial Network

At present,segmentation for medical image is mainly based on fully supervised model training,which consumes a lot of time and labor for dataset *** address this issue,we propose a semi-supervised medical image segmentation model based on a generative adversarial network framework for automated segmentation of *** network is mainly composed of two parts:a segmentation network for medical image segmentation and a discriminant network for evaluating segmentation *** the initial stage of network training,a fully supervised training method is adopted to make the segmentation network and the discrimination network have certain segmentation and discrimination *** a semi-supervised method is adopted to train the model,in which the discriminant network will generate pseudo-labels on the results of the segmentation for semi-supervised training of the segmentation *** proposed method can use a small part of annotated dataset to realize the segmentation of medical images and effectively solve the problem of insufficient medical image annotation data.

Background: Type 2 diabetes mellitus (T2DM) is a chronic disease that has become a major health care concern, especially in developing countries like Pakistan. Lifestyle modification and appropriate pharmacotherapy are shown to improve blood glucose levels, lipid abnormalities and blood pressure. It is not known how many patients adhere to advice and drugs prescribed. This study aimed to determine adherence to lifestyle and therapeutic advice. Methods: A cross sectional hospital based study was conducted among patients attending the diabetic clinic at the Aga Khan University hospital, using a structured questionnaire. Adult patients with T2DM and with at least one year duration of diabetes were included in the study. Results: Participants were aged between 32 and 92 years old with a mean age of 55.7 years old (SD ± 10.7). Mean duration of diabetes was 10.7 years old (SD ± 7.7). Majority (94%) of the patients were literate. Around half (47.3%) of the patients have had achieved glycemic target (HbA1c teaching hospital. This will need to be studied further in the community and efforts are required to motivate patients.

目的采用欧洲脊柱体模(European spine phantom,ESP)和志愿者用常用不同品牌的双能X线吸收测量仪(dual energy X-ray absorptiometry,DXA)测量的脊柱和髋部骨密度(bone mineral density,BMD)值进行体模校准和体内交叉校准,为建立统一的中国人群骨密度参考数据库,实现全国不同品牌DXA T值的标准化提供基础数据。方法采用FUJIFILM、GE-Lunar、Hologic、Medilink和Osteosys 5个品牌的DXA。体模校准实验使用同一个ESP(编号SN 145)在每个扫描仪上扫描10次,每次重新定位。体内交叉校准研究在同一天使用GE-Lunar iDXA系统和其他4家品牌之一的DXA对100名20~80岁健康志愿者进行腰椎(L1-4)和髋部扫描。采用SPSS 22.0完成线性回归分析,将FUJIFILM、Hologic、Medilink和Osteosys的脊柱、股骨颈和全髋BMD与GE-Lunar相对应部位的BMD进行交叉校准。并根据一个采用GE-Lunar DXA的75321名20岁及以上中国成年人全国性骨密度调查数据库计算正常参考值,采用本次体内交叉校准换算方程推导出其他4种品牌DXA的等效青年人参考数据库。结果GE-Lunar、FUJIFILM、Hologic、Medilink和Osteosys DXA的ESP L2-4 BMD(单位:g/cm^(2))均值分别为1.170±0.005、0.930±0.005、0.980±0.005、1.000±0.009和1.100±0.003,各品牌间BMD差异有统计学意义(P<0.01)。10次重复扫描的变异系数(CV%)分别为0.31%、0.53%、0.37%、0.91%和0.25%。FUJIFILM、Hologic、Medilink和Osteosys DXA脊柱和髋部的BMD测量值与GE-Lunar测量值高度相关,Pearson相关系数r为0.911~0.982,并计算出每种DXA的BMD值与GE相应部位BMD值的换算公式。列出5家DXA等效中国青年人骨密度参考范围,实现这些DXA设备扫描同一患者时计算的T值一致。结论本研究提供了国内5种品牌DXA的ESP和体内交叉校准数据以及换算公式,获得了统一的中国人DXA骨密度正常参考值,为实现全国DXA脊柱和髋部骨密度T值的标准化提供基础数据。
