The lower Mississippi River(LMR) has been heavily modified for multiple human purposes such as navigation, flood control, and bank stabilization. However, the LMR simultaneously supports a diverse fish fauna that includes recreational and commercial fisheries. Due to river training and diversion structures constructed during the past 80 years, the historic characteristics of the LMR have been drastically altered and have likely influenced fishes and fisheries in the system. One common restoration measure used throughout the LMR has been to "notch" wing-dike structures that close secondary(side) river channels. Dike notching allows year-round flows through secondary channels, which enhances habitat diversity and promotes biological productivity at the ecosystem scale. Although notching is presumed good for LMR fishes and other biota, few studies have examined its effects on fish assemblages. In this study, fish assemblages were sampled at seven LMR secondary channels spanning from river kilometer(rkm) 628(Louisiana-Mississippi, U.S.A.) upstream to rkm 1504(Missouri-Kentucky, U.S.A.). Four secondary channels were termed "permanent"(i.e.,with notched dikes) while three secondary channels were termed "temporary"(i.e., without notched dikes).Fishes were sampled by boat-mounted electrofishing conducted during falling and low stages from1995—1997. Fish assemblages differed between permanent and temporary secondary channels, and varied somewhat between falling and low stages. Gizzard shad(Dorosoma cepedianum), threadfin shad(D. petenense), and white bass(Morone chrysops) demonstrated consistent preferences for low-current conditions associated with temporary secondary channels. Conversely, blue catfish(Ictalurus furcatus), flathead catfish(Pylodictis olivaris), and freshwater drum(Aplodinotus grunniens) were more associated with permanent secondary channels. 对1例口腔EB病毒(Epstein-Barr virus,EBV)阳性皮肤黏膜溃疡病例组织活检样本形态、免疫学特征、鉴别诊断和临床特点进行分析。患者临床表现为单个口腔黏膜溃疡,组织学见大量多态性细胞浸润,其中主要是小淋巴细胞和一些混合的其他炎症细胞,以及散在不典型大细胞,细胞核大、不规则,胞浆丰富,偶见分叶核、双核细胞和涂抹状细胞。免疫组织化学染色显示这些大的不典型细胞CD30阳性,配对盒基因5(paired box domain gene 5,PAX5),CD79a阳性,EBV阳性,CD20个别细胞阳性。背景小淋巴细胞主要是CD3阳性的T细胞。EBV阳性皮肤黏膜溃疡在形态学和免疫学上和经典的霍奇金淋巴瘤、大B细胞淋巴瘤等类似或重叠,诊断比较困难,正确诊断除需要了解其形态学和免疫学特点,还需充分结合病史,如年龄、是否使用免疫抑制剂、发病部位和临床表现等关键信息。

Objective:To compare gestational age at discharge between infants born at 30-34+6 weeks gestational age who were admitted to neonatal intensive care units (NICUs) in California,Massachusetts,and the United Kingdom. Design:Prospective observational cohort study. Setting:Fifty four United Kingdom,five California,and five Massachusetts NICUs. Subjects:A total of 4359 infants who survived to discharge home after admission to an NICU. Main outcome measures:Gestational age at discharge home. Results:The mean (SD) postmenstrual age at discharge of the infants in California,Massachusetts,and the United Kingdom were 35.9 (1.3),36.3 (1.3),and 36.3(1.9)weeks respectively (p = 0.001). Compared with the United Kingdom,adjusted discharge of infants occurred 3.9 (95%confidence interval (CI) 1.4 to 6.5) days earlier in California,and 0.9 (95%CI -1.2 to 3.0) days earlier in Massachusetts. Conclusions:Infants of 30-34+6 weeks gestation at birth admitted and cared for in hospitals in California have a shorter length of stay than those in the United Kingdom. Certain characteristics of the integrated healthcare approach pursued by the health maintenance organisation of the NICUs in California may foster earlier discharge. The California system may provide opportunities for identifying practices for reducing the length of stay of moderately premature infants.

Background: Controversy has surrounded the question about whether high-dose rofecoxib increases or naproxen decreases the risk of serious coronary heart disease. We sought to establish if risk was enhanced with rofecoxib at either high or standard doses compared with remote non-steroidal anti-inflammatory drug(NSAID) use or celecoxib use, because celecoxib was the most common alternative to rofecoxib. Methods: We used data from kaiser permanente in California to assemble a cohort of all patients age 18-84 years treated with a NSAID between Jan 1, 1999, and Dec 31, 2001, within which we did a nested case-control study. Cases of serious coronary heart disease(acute myocardial infarction and sudden cardiac death) were risk-set matched with four controls for age, sex, and health plan region. Current exposure to cyclo-oxygenase 2 selective and non-selective NSAIDs was compared with remote exposure to any NSAID, and rofecoxib was compared with celecoxib. Findings: During 2 302 029 person-years of follow-up, 8143 cases of serious coronary heart disease occurred, of which 2210(27.1%) were fatal. Multivariate adjusted odds ratios versus celecoxib were: for rofecoxib(all doses), 1.59(95%CI 1.10-2.32, p=0.015); for rofecoxib 25 mg/day or less, 1.47(0.99-2.17, p=0.054); and for rofecoxib greater than 25 mg/day, 3.58(1.27-10.11, p=0.016). For naproxen versus remote NSAID use the adjusted odds ratio was 1.14(1.00-1.30, p=0.05). Interpretation: Rofecoxib use increases the risk of serious coronary heart disease compared with celecoxib use. Naproxen use does not protect against serious coronary heart disease.

Background: Several previous studies have found that females and older individuals are at greater risk of having incomplete flexible sigmoidoscopy. However, no prior study has reported the subsequent risk of colorectal cancer (CRC) following incomplete sigmoidoscopy. Methods: Using data from 55 791 individuals screened as part of the Colon Cancer Prevention (CoCaP) programme of kaiser permanente of Northern California, we evaluated the likelihood of having an inadequate (< 40 cm) examination by age and sex, and estimated the risk of distal CRC according to depth of sigmoidoscope insertion at the baseline screening examination. Multivariate estimation of risks was performed using Poisson regression. Results: Older individuals were at a much greater risk of having an inadequate examination (relative risk (RR) for age 80+ years compared with 50-59 years 2.6 (95% confidence interval (Cl) 2.3-3.0)), as were females (RR 2.3 (95% Cl 2.2-2.5)); these associations were attenuated but remained strong if Poisson models were further adjusted for examination limitations (pain, stool, and angulation). There was an approximate threefold increase in the risk of distal CRC if the baseline sigmoidoscopy did not reach a depth of at least 40 cm; a smaller increase in risk was observed for examinations that reached 40-59 cm. Conclusions: Older individuals and women are at an increased risk of having inadequate sigmoidoscopy. Because inadequate sigmoidoscopy results in an increased risk of subsequent CRC, physicians should consider steps to maximise the depth of insertion of the sigmoidoscope or, failing this, should consider an alternative screening test.
