Burden of gallstone disease in the United States population:Prepandemic rates and trends

BACKGROUND Gallstone disease is one of the most common digestive disorders in the United States and leads to significant morbidity,mortality,and health care utilization.AIM To expand on earlier findings and investigate prepandemic rates and trends in the gallstone disease burden in the United States using national survey and claims databases.METHODS The National Ambulatory Medical Care Survey,National Inpatient Sample,Nationwide Emergency Department Sample,Nationwide Ambulatory Surgery Sample,Vital Statistics of the United States,Optum Clinformatics®Data Mart,and Centers for Medicare and Medicaid Services Medicare 5%Sample and Medicaid files were used to estimate claims-based prevalence,medical care inc.uding cholecystectomy,and mortality with a primary or other gallstone diagnosis.Rates were age-adjusted(for national databases)and shown per 100000 population.RESULTS Gallstone disease prevalence(claims-based,2019)was 0.70%among commercial insurance enrollees,1.03%among Medicaid beneficiaries,and 2.09%among Medicare beneficiaries and rose over the previous decade.Recently,in the United States population,gallstone disease contributed to approximately 2.2 million ambulatory care visits,1.2 million emergency department visits,625000 hospital discharges,and 2000 deaths annually.Women had higher medical care rates with a gallstone disease diagnosis,but mortality rates were higher among men.Hispanics had higher ambulatory care visit and hospital discharge rates compared with Whites,but not mortality rates.Blacks had lower ambulatory care visit and mortality rates,but similar hospital discharge rates compared with whites.During the study period,ambulatory care and emergency department visit rates with a gallstone disease diagnosis rose,while hospital discharge and mortality rates declined.Among commercial insurance enrollees,rates were higher compared with national data for ambulatory care visits and hospitalizations,but lower for emergency department visits.Cholecystectomies performed in the United States inc.uded 60500

Human steering angle estimation in video based on key point detection and Kalman filter

Human pose recognition and estimation in video is pervasive.However,the process noise and local occlusion bring great challenge to pose recognition.In this paper,we introduce the Kalman filter into pose recognition to reduce noise and solve local occlusion problem.The core of pose recognition in video is the fast detection of key points and the calculation of human steering angles.Thus,we first build a human key point detection model.Frame skipping is performed based on the Hamming distance of the hash value of every two adjacent frames in video.Noise reduction is performed on key point coordinates with the Kalman filter.To calculate the human steering angle,current state information of key points is predicted using the optimal estimation of key points at the previous time.Then human steering angle can be calculated based on current and previous state information.The improved SENet,NLNet and GCNet modules are integrated into key point detection model for improving accuracy.Tests are also given to illustrate the effectiveness of the proposed algorithm.

This paper reports on the status of technology development under a national project launched in 2019 to address the problem of decreased system inertia associated with the large-scale integration of renewable energy.The project comprises two parts:the development of a system inertia observation technology using a continuous monitoring system to observe inertia and development of an inverter equipped with a function to provide virtual inertia as a countermeasure device.Utilizing both these efforts,the project aims to facilitate the introduction of renewable energy in the future with minimum restrictions.It was confirmed that the trend of inertia observed with the developed method was generally the same as that of the total inertia of synchronous machines observed by an electric utility.The effectiveness of the countermeasure device in reducing the frequency swing during a disturbance was confirmed through evaluation tests.

Potential sex differences in activation of pain-related brain regions in nonhuman primates with a unilateral spinal nerve ligation

The lack of truly robust analgesics for chronic pain is owed,in part,to the lack of an animal model that reflects the clinical pain state and of a mechanismbased,objective neurological indicator of pain.The present study examined stimulus-evo ked brain activation with functional magnetic resonance imaging in male and female cynomolgus macaques following unilateral L7 spinal nerve ligation and the effects of clinical analgesics pregabalin,duloxetine,and morphine on brain activation in these macaques.A modified straight leg raise test was used to assess pain severity in awake animals and to evo ke regional brain activation in anesthetized animals.The potential effects of clinical analgesics on both awake pain behavior and regional brain activation were examined.Following spinal nerve ligation,both male and female macaques showed significantly decreased ipsilateral straight leg raise thresholds,suggesting the presence of radicula rlike pain.Morphine treatment inc.eased straight leg raise thresholds in both males and females whereas duloxetine and pregabalin did not.In male macaques,the ipsilateral straight leg raise activated contralateral insular and somatosensory cortex(Ins/SII),and thalamus.In female macaques,the ipsilateral leg raise activated cingulate cortex and contralateral insular and somatosensory cortex.Straight leg raises of the contralateral,unligated leg did not evoke brain activation.Morphine reduced activation in all brain regions in both male and female macaques.In males,neither pregabalin nor duloxetine decreased brain activation compared with vehicle treatment.In females,however,pregabalin and duloxetine decreased the activation of cingulate cortex compared with vehicle treatment.The current findings suggest a diffe rential activation of brain areas depending on sex following a peripheral nerve injury.Diffe rential brain activation observed in this study could underlie qualitative sexual dimorphism in clinical chronic pain perception and responses to analgesics.Future pain management approaches fo

Accurate and generalizable quantitative scoring of liver steatosis from ultrasound images via scalable deep learning

BACKGROUND Hepatic steatosis is a major cause of chronic liver disease.Two-dimensional(2D)ultrasound is the most widely used non-invasive tool for screening and monitoring,but associated diagnoses are highly subjective.AIM To develop a scalable deep learning(DL)algorithm for quantitative scoring of liver steatosis from 2D ultrasound images.METHODS Using multi-view ultrasound data from 3310 patients,19513 studies,and 228075 images from a retrospective cohort of patients received elastography,we trained a DL algorithm to diagnose steatosis stages(healthy,mild,moderate,or severe)from clinical ultrasound diagnoses.Performance was validated on two multiscanner unblinded and blinded(initially to DL developer)histology-proven cohorts(147 and 112 patients)with histopathology fatty cell percentage diagnoses and a subset with FibroScan diagnoses.We also quantified reliability across scanners and viewpoints.Results were evaluated using Bland-Altman and receiver operating characteristic(ROC)analysis.RESULTS The DL algorithm demonstrated repeatable measurements with a moderate number of images(three for each viewpoint)and high agreement across three premium ultrasound scanners.High diagnostic performance was observed across all viewpoints:Areas under the curve of the ROC to classify mild,moderate,and severe steatosis grades were 0.85,0.91,and 0.93,respectively.The DL algorithm outperformed or performed at least comparably to FibroScan control attenuation parameter(CAP)with statistically significant improvements for all levels on the unblinded histology-proven cohort and for“=severe”steatosis on the blinded histology-proven cohort.CONCLUSION The DL algorithm provides a reliable quantitative steatosis assessment across view and scanners on two multi-scanner cohorts.Diagnostic performance was high with comparable or better performance than the CAP.

Response of human epidermal growth factor receptor 2-positive colorectal cancer to lapatinib monotherapy: A case report

BACKGROUND Human epidermal growth factor receptor 2(HER2)amplification is a molecular driver for a subset of colorectal cancers(CRCs)and one of the major causes of anti-epidermal growth factor receptor(EGFR)treatment failure.Compared to dual anti-HER2 treatments,which have been shown to be effective in HER2-positive metastatic CRC patients,single-agent anti-HER2 therapy is rarely used to treat CRC.CASE SUMMARY Herein,we report a case of RAS/BRAF-wild-type metastatic CRC that was identified as HER2-positive through circulating tumor DNA(ctDNA)testing by next-generation sequencing following the failure of two lines of therapy.Subsequently,the patient was given lapatinib monotherapy that led to a partial response with a progression-free survival of 7.9 mo.Moreover,serial ctDNA detection was used to monitor the efficacy of lapatinib.The aberration of HER2 copy number disappeared when radiographic assessment revealed a partial response.However,a high level of HER2 amplification was detected again at the time of disease progression.Finally,a phosphatidylinositol-4,5-bisphosphate 3-kinase catalytic subunit alpha mutation was identified at the time of tumor progression,which may explain the acquired resistance to lapatinib.CONCLUSION This is the first case report of HER2-positive RAS/BRAF wild-type metastatic CRC patient responding to lapatinib monotherapy.It highlights that ctDNA testing is an effective and feasible approach to evaluate the efficacy of anti-HER2 therapy.

A photolysis coefficient for characterizing the response of aqueous constituents to photolysis

UV photolysis and UV based advanced oxidation processes （AOPs） are gaining more and more attention for drinking water treatment. Quantum yield （Ф） and molar absorption coefficient （ε） are the two critical parameters measuring the effectiveness of photolysis of a compound. The product of the two was proposed as a fundamental measure of a constituent＇s amenability to transformation by photolysis. It was shown that this product, named the photolysis coefficient, kp, can be determined using standard bench tests and captures the properties that govern a constituent＇s transformation when exposed to light. The development showed the photolysis coefficient to be equally useful for microbiological, inorganic and organic constituents. Values of kp calculated by the authors based on quantum yield and molar absorption coefficient data from the literature were summarized. Photolysis coefficients among microorganisms ranged from 8500 to more than 600000 and are far higher than for inorganic and organic compounds, which varied over a range of approximately 10 to 1000 and are much less sensitive to UV photolysis than the microorganisms.

Estimating water deficit and its uncertainties in water-scarce area using integrated modeling approach

Accurate assessment of water deficit and related uncertainties in water-scarce areas is strategically important in various fields of water resources management. This study developed a hybrid approach integrating conceptual water balance model and econometric regression to estimate water shortage and its related uncertainties in water-scarce areas. This hybrid approach was used to assess the agricultural water deficit of Beijing, an extremely water-scarce area in China. A predictive model of agricultural water demand was developed using the stepwise multiple regression method, and was validated by comparing the predicted values with observed data. Scenario analysis was employed to investigate the uncertainties of agricultural water shortage and agricultural water demand. This modeling approach can assist water administration in creating sustainable water allocation strategies in water-scarce areas.

Microstructure and properties of nano-TiN modified Ti(C,N)-based cermets fabricated by powder injection molding and die pressing

Powder injection molding （PIM） and die pressing were employed to fabricate nano-TiN modified Ti（C,N）- based cermets. The shrinkage behavior, microstructure, porosity, and mechanical properties of the samples with and without nano-TiN addition fabricated by PIM and die pressing were analyzed. It is demonstrated that for either PIM or die pressing, the porosities are obviously reduced, the mechanical properties are significantly improved after adding nano-TiN, and the hard particles are refined; the rim phase thickness obviously becomes thinner, and the number of dimples in fracture also inc.eases. Compared the samples fabricated by die pressing, it is difficult for PIM to obtain dense Ti（C,N）-based cermets. Due to the too much existence of pores and isolated carbon, the mechanical properties of the sintered samples by PIM are inferior to those of the sintered ones by die pressing.
