Background:Aavarai Bhavanai Chooranam(ABC)is a Siddha polyherbal formulation containing ten medicinal plants that have been used in the Siddha system of medicine,practiced in the Southern part of Tamil Nadu,India for the management of ***:The study is carried out to scientifically validate the traditional formulation and to study the mechanism of ***:In the present study,the antiurolithiatic activity of ABC was evaluated in Wister albino rats by using ethylene glycol through drinking *** I acted as the standard control group and was given unlimited access to conventional rat food and *** II-V was given ethylene glycol(0.75%)in water to induce renal calculi for 28 *** III got the typical anti-urolithiatic medication,Cystone(750 mg/kg),from the 15th day to the 28th day,whereas Group II functioned as the lithiatic control group and received the *** Bhavanai Chooranam(200 mg/kg)was administered to Group IV from the 15th to the 28th day as a curative *** the first day to the 28th day,Group V received Aavarai Bhavanai Chooranam(200 mg/kg),which was administered as a preventative *** groups'urine samples were taken,and using accepted techniques,their calcium,magnesium,oxalate,inorganic phosphate,protein,and creatinine levels were *** treatment promotes diuresis which leads to flushing of the renal stones and maintains the alkaline condition in the urinary system which probably mediates the antilithiatic ***:ABC provides structural and functional protection to the kidneys by enhancing its physiological function against stone formation and validates its clinical use.

Serum Vitamin D Levels and Severity of Liver Dysfunction in Cirrhotic Patients

Background and objective: Cirrhosis affects hundreds of millions of patients all around the world. Vitamin D deficiency is frequently observed in chronic hepatic disease. Vitamin D level may be an important survival marker in advanced liver cirrhosis. Material and Methods: The study is a sectional one of the descriptive-analytical type, where 90 of the patients with hepatic cirrhoses were selected with census sampling method. In all the cirrhotic patients, for diagnosis in clinical findings, the serological marker, autoantibodies were stable;biochemical endoscopy and imaging were the histological evidence and examinations and were then analyzed by SPSS version 22 software. Results: Based on the study, from the aspect of Child-Pugh classification, the highest vitamin D levels had the highest value in Child-Pugh class A with a frequency of 13 (43%) in the form of insufficiency, and had the highest value in Child-Pugh class B and C in the form of slight deficiency. Conclusion: It was specified based on the results obtained from the research that different vitamin D levels and liver failure severity have a significant relationship with each other (p < 0.05) in patients suffering from hepatic cirrhosis, such that the serum vitamin D level decreases as liver failure severity increases.

Pheochromocytomas are catecholamine producing tumors and although uncommon present a great challenge to the anesthesiologist since it has nonspecific clinical symptoms and risk of critical events, including death when not previously diagnosed. Clinical manifestation is variable, unspecific and depends on the catecholamine production profile. The classic triad of headache, palpitation and diaphoresis is present in up to 70% of the cases and only 50% have sustained hypertension. The best approach for pheochromocytoma treatment is surgical excision of the affected adrenal gland. The introduction of alpha adrenergic blockade medication, such as phentolamine and phenoxybenzamine had the highest impact in perioperative mortality reduction due to inhibition of the deleterious effect of vasoconstriction. The majority of anesthetic techniques and drugs are considered safe. Post-operative care in intensive care unit is advisable since patients may present instability of blood pressure and hypoglycemia. Genetic testing should be done in first-degree relatives of confirmed cases or when a genetic syndrome is suspected.

Foreign body aspiration is common in young children or in adults with advanced age and altered mentation. Usually symptoms present acutely, leading to early intervention. We present a rare case of a healthy young female who aspirated her nasal ring but remained asymptomatic for almost eight years. The patient ultimately required a right upper lobe lobectomy for bronchiectasis.

The Use of the V.A.C. RX-4 for Multiple Soft Tissue Wound Application in the Single Patient: A Case Report

Contact burn injuries account for a considerable proportion of admissions that frequently require debridement. Such debridements of these multiple open wounds might benefit from the application of negative pressure wound therapy (NPWT) for the removal of proinflammatory mediators and to promote granulation tissue with macrostrain and microstrain. Having four or more Vacuum Assist Closure (V.A.C.) Ulta devices connected to the same patient for adequate wound coverage is cumbersome in the management of the patient’s wound care and tethers the patient to the bed. The V.A.C. RX-4 is a multichannel device that can deliver NPWT with a smaller footprint. In addition, the V.A.C. RX-4 has a weight of 16 lbs. versus one V.A.C Ulta which is 7.4 lbs. Therefore, collectively, four V.A.C. Ultas would equal 29.6 lbs. or almost double the weight of a single V.A.C. RX-4. Use of the V.A.C. RX-4 by healthcare providers can mean greater mobility for the patient and easier transport between patient destinations within the hospital. This case report demonstrates the utility of the V.A.C. RX-4 for open and freshly debrided, large soft tissue wounds in a burn patient.

EH （Epithelioid hemangioendothelioma） is a rare vascular malignant neoplasm. Only 434 cases had been reported until 2006. Symptoms may be nonspecific, but it atso can involve hepatic veins resulting in Budd-Chiari syndrome. The main differential diagnosis is cholangiocarcinoma, sometimes only distinguishable through immunohistochemistry study. We report a case of a young woman diagnosed with hepatic EH and listed for liver transplantation.

Vitamin D is important in multiple aspects of health and its effects are mediated through the Vitamin D Receptor (VDR). We wanted to test the hypothesis that specific haplotypes of the VDR gene are associated with markers of disease severity, inflammation and bone health in Sickle Cell Disease (SCD). Genotyping was performed on DNA specimens from 1141 study participants in the NIH-funded Silent Infarct Transfusion (SIT) trial. We used the clinical and laboratory data to create separate endothelial dysfunction, vaso-occlusive severity scores and phenotype variables. Seventy-nine Single Nucleotide Polymorphisms (SNP) in the VDR gene and three associated genes—CYP27B1, VD binding protein, retinoid X receptor, were evaluated. The discovery cohort individuals had VDR haplotype information from a prior Genome-Wide Association Study (GWAS). The validation cohort was analyzed for SNPs that were significant in the discovery cohort. The pheno-type data were obtained from the demographic and clinical information of the participants, and were used to create the severity scores, vaso-occlusive score, endothelial dysfunction severity, and overall severity score. Potential gene-gene interactions were analyzed for prediction of disease severity within each severity score. Two SNPs were associated with the overall severity score, 3 SNPs with the endothelial dysfunction severity score and 4 SNPs with the vaso-occlusive severity score. After permutation testing to correct for multiple comparisons, only one of the associations remained significant. SNP rs7965281 was found to be associated with the endothelial dysfunction severity score and remained significant after correcting for multiple comparisons using permutation testing. In the validation cohort, that SNP was again tested for association with each of the severity scores. There was no association with the endothelial or the overall severity score but a trend towards association with the vaso-occlusive severity score (p = 0.02). N
