Objective: To observe the conversion disorder in a neurological emergency department. Methods: It is common that the initial approach to this patients include the use of various diagnostic exams. In this series we reviewed 94 patients that arrived a neurological emergency room in a 3 year period. Results: 72 patients were females (76%), and the initial presumptive diagnosis were: neurovascular syndrome in 36 patients (38.3%), convulsive disorder in 20 patients (21.28%), and conversive disorder in 8 patients (8.51%). 82 patients had motor symptoms and 61 sensitive symptoms. 88 patients (93%) required neuroimaging studies, 77 (81%) patients underwent through basic biochemical panels. Other tests performed were:electroencephalogram in 12 patients (12.77%), electromyography in 11 patients (11.7%), lumbar punction in 8 patients (8.04%) and regarding the medical consult in the care of these patients 11 were evaluated by 1 specialists, 35 (37.2%) by 2 different specialties, 42 (44.63%) patients required evaluation by 3, and 6 patients (6.38%) required evaluation by 4 different specialties. Conclusions: Based on this data, we conclude that conversion disorders require a lot of resources in the emergency room and that the similarities with neurological diseases demands a complete workup including expensive diagnostic tools. However, this patients can be discharged safely without requiring hospitalization.

Spinal cord injury(SCI)is a severe lesion comporting various motor,sensory and sphincter dysfunctions,abnormal muscle tone and pathological reflex,resulting in a severe and permanent lifetime *** primary injury is the immediate effect of trauma and includes compression,contusion,and shear injury to the spinal cord.A secondary and progressive injury usually follows,beginning within minutes and evolving over several hours after the first *** ischemia is one of the most important mechanisms involved in secondary injury,a treatment to increase the oxygen tension of the injured site,such as hyperbaric oxygen therapy,should theoretically help *** a meta-analysis concluded that hyperbaric oxygen therapy might be helpful for clinical treatment as a safe,promising and effective choice to limit secondary injury when appropriately started,useful and well-defined protocols/guidelines still need to be created,and its application is influenced by local/national *** topic is not a secondary issue because a well-designed randomized controlled trial requires a proper sample size to demonstrate the clinical efficacy of a treatment,and the absence of a common practice guideline represents a limit for results *** narrative review aims to reassemble the evidence on hyperbaric oxygen therapy to treat SCI,focusing on adopted protocols in the studies and underlining the critical ***,we tried to elaborate on a protocol with a flowchart for an evidence-based hyperbaric oxygen therapy *** conclusion,a rationale and shared protocol to standardize as much as possible is needed for the population to be studied,the treatment to be adopted,and the outcomes to be *** studies,above all,well-designed randomized controlled trials,are needed to clarify the role of hyperbaric oxygen therapy as a strategic tool to prevent/reduce secondary injury in SCI and evaluate its effectiveness based on an evidence-based

Spigelian Hernia (SH) is an uncommon anterior abdominal wall defect, it represents 0.1% - 2% of all abdominal wall hernias. SHs have been traditionally repaired by open technique, but laparoscopic approaches are becoming more common and widely described in the literature. Here we present a case report of a 69-year-old woman who presented with abdominal pain, nausea, abdominal distention and absence of bowel movements for 2 days. A computed tomography performed in an external facility revealed a right-sided and incarcerated SH containing bowel and mesentery. The patient was treated surgically and the abdominal wall defect was repaired by a laparoscopic transabdominal preperitoneal (TAPP) approach. The patient was discharged 24 hours after surgery in excellent conditions. We hold that the TAPP approach is anatomically the soundest repair, with all the added benefits of minimally invasive surgery.

The three main components of the media layer of the aorta are elastic fibers,collagen fibers and smooth muscle cells(SMC).[1]This layer’s elastic properties are major determinants of its biomechanics in health and disease states.[2,3]Age-related changes in such elastic properties are associated with altered hemodynamic parameters,such as systolic hypertension,[4,5]and end-organ damage,[6,7]and are thought to result from changes of its main components.

Moral hazard is an important factor threatening stability of dynamic alliance. Firstly, the paper describes the problem of moral hazard in dynamic alliance and introduces the Tirole Model of moral hazard prevention. Then, by introducing a third-party system into dynamic alliance, a model of incomplete information dynamic game theory is formulated in the principal-agent framework. The model shows the dynamic consistency between a member＇s income and the dynamic alliance＇s profit, and that moral hazard can be prevented by an allocation policy With both characteristics of incentive measures and supervision, which is designed by a third-party system. Finally, the composition of the third-party system is discussed.

We are describing a transverse incision that we have found very useful in performing surgical procedures where the fractured site needs freshening, excision of the necrotic bones or shortening. Operative procedures using this technique have been extensively used by us since 1980 and we have registered 1187 cases with average follow up of 10 years. The incision is transverse and can be performed in any part of leg and extends from 1 cm lateral to anterior border of the tibia and runs medially till 1 cm medial to medial border of tibia. The incision allows adequate visualization of the interested area, less of periosteal stripping, ease of closure, no post operative wound dehiscence and cosmetically very much acceptable.

The psychiatric profile of the U.S. patient population across age groups

Introduction: As the U.S. population undergoes continuous shifts the population’s health profile changes dynamically resulting in more or less expression of certain psychiatric disorders and utilization of health-care resources. In this paper, we analyze national data on the psychiatric morbidity of American patients and their summated cost in different age groups. Methods: The latest data (2009) on the number of hospital discharges and national bill (hospital charges) linked with psychiatric disorders were extracted from the Nationwide Inpatient Sample (NIS). Results: National data shows that mood disorders are the largest diagnostic category in terms of percentage of psychiatri-crelated discharges in the 1 - 17 years age group. The proportion decreases gradually as age progresses while delirium, dementia, amnestic and other cognitive disorders increase exponentially after 65 years of age. Schizophrenia and other psychotic disorders as well as alcohol and substance-related disorders peak in the working age groups (18 - 64 years). From an economic point of view, mood disorders in the 18 - 44 age group has the highest national bill ($5.477 billion) followed by schizophrenic and other psychotic disorders in the same age group ($4.337 billion) and mood disorders in the 45 - 64 age group ($4.310 billion). On the third place come schizophrenic and other psychotic disorders in the 45 - 64 age group ($3.931 billion). Conclusion: This paper illustrates the high cost of psychiatric care in the U.S., especially the large fraction of healthcare money spent on working-age patients suffering from mood disorders. This underlines psychiatric cost-efficiency as a vital topic in the current healthcare debate.
