Gram-scale synthesis of nanotherapeutic agents for CT/Tl-weighted MRI bimodal imaging guided photothermal therapy

Theranostic nanomedicine, which uses both imaging and therapeutic components for simultaneous disease diagnosis and treatment, is expected to improve patient treatment safety and outcomes by offering a more personalized approach to medicine. However, the poor reproducibilities of nanomedicines synthesized for optimized bioavailability and their potential toxicity are impeding clinical development. Moreover, milligram-scale synthetic methods are often inconsistent when transferred to mass production. To address these challenges, a facile, room temperature, aqueous phase synthesis of nanotheranostic agents using clinically validated mesoporous silica and naturally derived polydopamine has been developed. Since the synthetic procedure is simple and robust, and requires only simple mixing under ambient conditions, excellent batch-to-batch consistency has been achieved. As a result, this process can be easily scaled-up to produce gram-scale batches with physicochemical parameters similar to those of materials synthesized in smaller batches. The resulting nanotheranostic agents exhibit efficient X-ray tomography and T1-weighted magnetic resonance image contrast enhancing abilities due to their chemically ligated, benign Bi3+ and Fe3+ ions. Furthermore, the inclusion of a polydopamine shell makes the nanoparticle surface easy to functionalize and renders these materials highly efficient as photothermal agents. These nanotheranostic agents are suitable for mass production and for potential applications in multimodal imaging-guided therapy in clinical settings.

Q-space trajectory imaging(QTI)allows non-invasive estimation of microstructural features of heterogeneous porous media via diffusion magnetic resonance imaging performed with generalised gradient waveforms.A recently proposed constrained estimation framework,called QTI+,improved QTI's resilience to noise and data sparsity,thus increasing the reliability of the method by enforcing relevant positivity *** this work we consider expanding the set of constraints to be applied during the fitting of the QTI *** show that the additional conditions,which introduce an upper bound on the diffusivity values,further improve the retrieved parameters on a publicly available human brain dataset as well as on data acquired from healthy volunteers using a scanner-ready protocol.

Background and purpose While extracranial carotid artery stenosis is more common among Caucasians and intracranial artery stenosis is more common among Asians,the differences in atherosclerotic plaque characteristics have not yet been extensively *** sought to investigate plaque location and characteristics within extracranial carotid and intracranial arteries in symptomatic Caucasians and Chinese using vessel wall *** Subjects with recent anterior circulation ischaemic stroke were recruited and imaged at two sites in the USA and China using similar *** extracranial carotid and intracranial arteries were reviewed to determine plaque location and *** The prevalence of extracranial carotid plaque in Caucasians and Chinese was 73.1%and 49.1%,respectively(p=0.055).Prevalence of intracranial plaque was 38.5%and 69.1%in Caucasians and Chinese,respectively(p=0.02).Furthermore,42% of Caucasians and 16%of Chinese had high-risk plaque(HRP)features(intraplaque haemorrhage,luminal surface disruption)in the extracranial carotid artery(p=0.03).The prevalence of HRP features in intracranial arteries was not significantly different between the two cohorts(4%vs 11%;p=0.42).Conclusions Differences in the location and characteristics of cerebrovascular atherosclerosis were identified by vessel wall MRI in US Caucasian and Chinese subjects with recent anterior circulation ischaemic *** carotid plaques with HRP features were more common in *** plaques were more common in Chinese subjects,but no significant difference between the two cohorts in intracranial HRP prevalence was *** studies using vessel wall imaging to investigate racial differences in cerebrovascular disease may inform underlying mechanisms of HRP development and may ultimately help guide appropriate therapy.

Clinical Value of Dual-energy CT in Detection of Pancreatic Adenocarcinoma: Investigation of the Best Pancreatic Tumor Contrast to Noise Ratio

Objective To quantitatively compare and determine the best pancreatic tumor contrast to noise ratio (CNR) in different dual-energy derived datasets. Methods In this retrospective, single center study, 16 patients (9 male, 7 female, average age 59.4±13.2 years) with pathologically diagnosed pancreatic cancer were enrolled. All patients received an abdominal scan using a dual source CT scanner 7 to 31 days before biopsy or surgery. After injection of iodine contrast agent, arterial and pancreatic parenchyma phase were scanned consequently, using a dual-energy scan mode (100 kVp/230 mAs and Sn 140 kVp/178 mAs) in the pancreatic parenchyma phase. A series of derived dual-energy datasets were evaluated including non-liner blending (non-linear blending width 0-500 HU; blending center -500 to 500 HU), mono-energetic (40-190 keV), 100 kVp and 140 kVp. On each datasets, mean CT values of the pancreatic parenchyma and tumor, as well as standard deviation CT values of subcutaneous fat and psoas muscle were measured. Regions of interest of cutaneous fat and major psoas muscle of 100 kVp and 140 kVp images were calculated. Best CNR of subcutaneous fat (CNR F ) and CNR of the major psoas muscle (CNR M ) of non-liner blending and mono-energetic datasets were calculated with the optimal mono-energetic keV setting and the optimal blending center/width setting for the best CNR. One Way ANOVA test was used for comparison of best CNR between different dual-energy derived datasets. Results The best CNR F (4.48±1.29) was obtained from the non-liner blending datasets at blending center -16.6±103.9 HU and blending width 12.3±10.6 HU. The best CNR F (3.28±0.97) was obtained from the mono-energetic datasets at 73.3±4.3 keV. CNR F in the 100 kVp and 140 kVp were 3.02±0.91 and 1.56±0.56 respectively. Using fat as the noise background, all of these images series showed significant differences (P<0.01) except best CNR F of mono-energetic image sets vs. CNR F of 100 kVp image (P=0.460). Similar r

This report describes a rare case of metastatic hepatocellular carcinoma (HCC) presenting a huge mass in the left external auditory canal (EAC). The patient was a 55-year-old man with hepatitis B virus-related HCC. He presented to our department with a three-month history of increasing left otalgia,and hearing loss with recent fresh aural bleeding. Histopathologic examination indicated that the tumor was secondary to HCC. Although external irradiation was not effective,the tumor was treated with surgical debulking and high dose rate 192 Ir remote afterloading system (RALS) for postoperative intracavitary irradiation. A review of the literature revealed only five other cases of HCC metastasis to the temporal bone,all of which mainly metastasized in the internal acoustic meatus. The present case is the first report of HCC metastasis to the EAC.

Background and Aims:This study aimed to determine the performance of the non-invasive score using noncontrastenhanced MRI(CHESS-DIS score)for detecting portal hy-pertension in ***:In this international multicenter,diagnostic study(***,NCT03766880),patients with cirrhosis who had hepatic venous pressure gradient(HVPG)measurement and noncontrast-enhanced MRI were prospectively recruited from four university hospitals in China(n=4)and Turkey(n=1)between December 2018 and April 2019.A cohort of patients was retrospectively recruited from a university hospital in Italy between March 2015 and November *** segmentation of the liver on fat-suppressed T1-weighted MRI maps,CHESS-DIS score was calculated automatically by an in-house developed code based on the quantification of liver surface ***:A total of 149 patients were included,of which 124 were from four Chinese hospitals(training cohort)and 25 were from two international hospitals(validation cohort).A positive correlation between CHESS-DIS score and HVPG was found with the correlation coefficients of 0.36(p<0.0001)and 0.55(p<0.01)for the training and validation cohorts,*** area under the receiver operating characteristic curve of CHESS-DIS score in detection of clinically significant portal hypertension(CSPH)was 0.81 and 0.9 in the training and validation cohorts,*** intra-class correlation coefficients for assessing the inter-and intra-observer agreement were 0.846 and 0.841,***:A non-invasive score using noncontrast-enhanced MRI was developed and proved to be significantly correlated with invasive ***,this score could be used to detect CSPH in patients with cirrhosis.
