Circulating inflammatory factors associated with worse long-term prognosis in colorectal cancer

AIM To investigate association of circulating inflammatory factors at the time of colorectal cancer(CRC) surgery with *** Plasma levels from 174 CRC patients(69 females and 105 men), with median age 70 years(range 29-90), localized in the colon(n = 105) or rectum(n = 69), with stage Ⅰ(n = 24), stage Ⅱ(n = 54), stage Ⅲ(n = 67) and stage Ⅳ(n = 29) were measured using commercially available Bio-Plex Pro? Human Chemokine Panel 40-Plex, including 40 different chemokines, cytokines and interleukins. The prognostic association of each inflammatory factor was analysed as CRC-specific and total *** Out of 174 patients, 66 died during the follow-up, 40 because of CRC specific mortality. High tertile levels of 8 factors were significantly associated with increased CRC-specific mortality, of which CCL1, CCL20, CCL24, CX3CL1, IL-4 and TNF-α remained significant in a multivariate Cox regression analysis. High tertile levels of 14 factors were associated with increased total mortality, of which CCL1, CCL15, CCL20, CX3CL1, CXCL13, IFN-γ, IL-2, IL-4 and IL-10 remained significant after adjustment for clinical covariates. For most of the inflammatory factors the association between higher tertile levels and an increased mortality in general appeared two years after surgery. High tertile levels of TNF-α and CCL24 were exclusively associated with CRC-specific mortality. The distribution of these factors were not associated with TNM stage with exception for *** High plasma levels of inflammatory factors are associated with increased risk of mortality among CRC patients and could be potential biomarkers for revealing prognosis.

Working With News Literacy Among Pre-teens： From Selection to Reflection

This article presents the outcome of the Swedish pilot study CESEM （named after the initials of the girls＇ first names） where pre-teen girls produced their own news program and showed it to their classmates. The methodological approach was the action research model where the process of producing news was followed from selecting news to reflecting on their learning outcomes. The knowledge from the pilot study is useful in designing active projects within the scope of media literacy. One of the learning outcomes for the girls in this study was the awareness of how news programs in TV are staged. A positive response was the initiative from one of the participating schools to start their own news project in one of the classes. A possible outcome in longer run projects is better knowledge for developing pedagogical tools in media education.

The present study aims to investigate the effect of friction stir processing(FSP)conditions on the corrosion characteristic of AZ31B magnesium *** made of AZ31B alloy were friction stir processed under various processing conditions,and their microstructure and corrosion behavior were *** corrosion behavior was studied by potentiodynamic polarization,electrochemical impedance spectroscopy(EIS),and immersion test in 3.5%sodium chloride(NaCl)*** results showed a substantial improvement in the corrosion resistance of the friction stir processed AZ31B *** improvement is likely a result of more stable corrosion products and microstructure refinement formed after friction sir processing.

Reactive Search Optimization;Application to Multiobjective Optimization Problems

During the last few years we have witnessed impressive developments in the area of stochastic local search techniques for intelligent optimization and Reactive Search Optimization. In order to handle the complexity, in the framework of stochastic local search optimization, learning and optimization has been deeply interconnected through interaction with the decision maker via the visualization approach of the online graphs. Consequently a number of complex optimization problems, in particular multiobjective optimization problems, arising in widely different contexts have been effectively treated within the general framework of RSO. In solving real-life multiobjective optimization problems often most emphasis are spent on finding the complete Pareto-optimal set and less on decision-making. However the com-plete task of multiobjective optimization is considered as a combined task of optimization and decision-making. In this paper, we suggest an interactive procedure which will involve the decision-maker in the optimization process helping to choose a single solution at the end. Our proposed method works on the basis of Reactive Search Optimization (RSO) algorithms and available software architecture packages. The procedure is further compared with the excising novel method of Interactive Multiobjective Optimization and Decision-Making, using Evolutionary method (I-MODE). In order to evaluate the effectiveness of both methods the well-known study case of welded beam design problem is reconsidered.

Images of sorrow—Experiences of losing a co-twin in old age

What it is like when a lifelong twin relationship ends through death in later life is the focus of this study. It draws on interview data from seven twins who are part of a longitudinal Swedish twin study (SATSA) and who lost their co-twins in old age. Data were analyzed using qualitative latent content analysis. The results showed that the experience of loss of the co-twin was profound, including an emotional as well as a behavioral dimension. Loss and loneliness were expressed as the dominant feelings related to the quality of the missing relationship as well as the loss of twin identity. However, the grief experiences in this study were primarily related to the closeness and quality of the twin relationship, rather than identity. Behavioral adjustments included the use of outside as well as internal cognitive resources to cope with life after the loss. Despite the devastating experience of losing a co-twin after a lifelong relationship, the participants engaged actively in their own grief processes. It was concluded that twin loss is unique, in the sense of losing the relational twin identity, as well as it is characterized by similar features as the loss of a close relationship among non-twins.

Promoting sense of security in old-age care

Purpose: The concept of security is related to the experience of health but has ever so often been argued from a risk perspective rather than from a promotional perspective. The experience of older persons’ sense of security in private homes and in nursing homes seems to be missing when it comes to promote aging well throughout the life span. This study aimed to describe and analyze factors related to the sense of security of older persons receiving care in nursing homes in Sweden. Design and Methods: The study was based on a questionnaire from a total of 495 persons aged ≥ 65 in private homes (n = 350) or nursing homes (n = 145) in Sweden. Results: Secure relationships, sense of control, and perceived health were significantly related to the subjects’ sense of security. No significant relationships were found between sense of security and having a personal emergency response alarm. Implications: Experience of sense of security from the older person perspective differs depending on the context. To promote the sense of security within the care of older persons, methods on how to establish secure relations as well as the sense of control and knowledge need to be further tested, developed and analyzed together with older persons.

Coexistence of 2.4 GHz sensor networks in home environment

The IEEE 802.15.4 is one of the low-layer communication standards for personal area networks （PANs） and wireless sensor networks （WSNs）, which may be interfered by other wireless devices in the industrial, scientific and medical （ISM） frequency bands, especially in home environment, such as devices of IEEE 802.1 lb, Bluetooth, cordless telephone, and microwave oven radiation. This article examines the mutual interference effects of 2.4 GHz devices widely deployed at home, via both theoretical analysis and real-life experiment. An analytical model is proposed to estimate the packet error rate （PER） of radio frequency （RF） coexistent networks. The model is verified through a series of experiments. The experimental results also show that Bluetooth has little impact of interference on IEEE 802.15.4 sensor networks, and that the effect of microwave oven radiation on IEEE 802.15.4 sensor devices is tolerable if the device is a few meters away from the oven. Whereas, IEEE 802.1 lb wireless networks can cause problems to IEEE 802.15.4, however the effects can be significantly reduced by a proper channel selection. This article also proposes the interference duration model, which will be helpful in modeling of coexistence simulation. Simulation results show that the stationary scenario obeys the experiments result very well.

Developing and Establishing the Psychometric Properties of an Ethos towards Wellness Questionnaire (EtWeQ)

Aim: Validated instruments exist measuring parts of personal health, ill-health and lifestyle, but there are few, if any, instruments “grasping the big picture” of wellness and ethos. Accordingly the aim was to develop and establish the psychometric properties of a 74-item Ethos towards Wellness Questionnaire (EtWeQ) with regards to content and construct validity, as well as homogeneity and stability reliability. Methods: A questionnaire based on a methodological and developmental design was sent out twice with a four week gap between, on 221 healthy middle-aged participants in four steps: item generating, content validating, statistical analyzing and establishing the final questionnaire. Results: The substantial base, consisting of content validity resulted in, besides basic characteristics, six well-defined and sufficient indexes comprising of;two health indexes (healthiness and ill-healthiness), three life context indexes (work, family, spare time), and one brief comprehensive index (ethos). The three life context as well as the ethos indexes at an ordinal scale level, identified overall satisfactory communalities of >0.30, factor loadings > 0.30, and factor total variance > 50% with regard to construct validity. The homogeneity reliability, in terms of Cronbach’s alpha coefficient > 0.70, at both occasions with a four week gap between, as well as the stability reliability in terms of intraclass correlation coefficient > 0.70, were also considered satisfactory in the same indexes. Conclusions: This newly developed, and likely the only questionnaire focusing on “grasping the big human picture”, was based on both a philosophical reasoning and empirical recommendations of wellness, is shown to be a valid and reliable measurement in screening or in follow-up of healthy people’s wellness and ethos.

Factors influencing female patients’ recovery after their first myocardial infarction as experienced by cardiac rehabilitation nurses

Background: In the developed part of the world, coronary heart disease is the major cause of death and is one of the leading causes of disease burden. In Sweden, more than 30,000 people per year are affected by myocardial infarction and out of these approximately 40% are women. Nearly 70% of the women survive and after a myocardial infarction a recovery process follows. Today’s health care focuses more on treatment, symptoms and risk factors than on the individuals’ perceptions of the recovery process. Aim: To explore cardiac rehabilitation nurses’ experiences of factors influencing female patients’ recovery after their first myocardial infarction. Methods: Twenty cardiac rehabilitation nurses were interviewed. The study was conducted using qualitative content analysis. Results: The cardiac rehabilitation nurses experienced that women’s recovery after a first myocardial infarction was influenced whether they had a supportive context, their ability to cope with the stresses of life, if they wanted to be involved in their own personal care and how they related to themselves. Conclusions: Women’s recovery after a myocardial infarction was influenced by factors related to surroundings as well as own individual factors. The underlying meaning of women’s recovery can be described as the transition process of a recovery to health. Our findings suggest that a focus on person-centered nursing would be beneficial in order to promote the every woman’s personal and unique recovery after a myocardial infarction. Finally, the cardiac rehabilitation nurses’ experiences of factors influencing male patients’ recovery after their first myocardial infarction should be important to investigate.

Development and psychometric evaluation of the radiographers’ competence scale

Assessing the competence of registered radiographers’ clinical work is of great importance because of the recent change in nursing focus and rapid technological development. Self-assessment assists radiographers to validate and improve clinical practice by identifying their strengths as well as areas that may need to be developed. The aim of the study was to develop and psychometrically test a specially designed instrument, the Radiographers Competence Scale (RCS). A cross sectional survey was conducted comprising 406 randomly selected radiographers all over Sweden. The study consisted of two phases;the development of the instrument and evaluation of its psychometric properties. The first phase included three steps: 1) construction of the RCS;2) pilot testing of face and content validity;and 3) creation of a web-based 54-item questionnaire for testing the instrument. The second phase comprised psychometric evaluation of construct validity, internal consistency reliability and item reduction. The analysis reduced the initial 54 items of the RCS to 28 items. A logical two-factor solution was identified explaining 53.8% of the total variance. The first factor labelled “Nurse initiated care” explained 31.7% of the total variance. Factor 2 labelled “Technical and radiographic processes” explained 22.1% of the total variance. The scale had good internal consistency reliability, with a Cronbach’s alpha of 0.87. The RCS is a short, easy to administer scale for capturing radiographers’ competence levels and the frequency of using their competence. The scale was found to be valid and reliable. The self-assessment RCS can be used in management, patient safety and quality improvement to enhance the radiographic process.
