Objective:To explore the effects of combining application of diagnosis and treatment technologies of Event Related Potentials(ERP)and Psychic Rehabilitation Intervention(PRI)on rehabilitation of ***:PRI and ERP were used in schizophrenia rehabilitation in this research.232 patients were divided into study group(SG)and control group(CG),and the rehabilitation effects were ***:(1)After the implementation of the PRI,more score of the Brief Psychiatric Rating Scale(BPRS)was reduced on SG than the score on CG;more decreased scores of insight and on Social Disability Screening Schedule(SDSS),and more increased scores of treatment compliance questionnaire scores were observed on SG,comparing with CG(P<0.05 or 0.01).(2)After the implementation of the PRI,more decreased factor scores of Symptom Checklist 90 were observed on SG patients family members(P<0.05 or 0.01).(3)PRI significantly improved the patients’social functions,family members’relationship and prevented the recurrence of schizophrenia.(4)Improved P2,P3 amplitudes were also detected on SG compared with CG(P<0.05).Conclusions:(1)PRI interventions may improve schizophrenia patients’social function.(2)ERP may be used to evaluate the schizophrenia patients’rehabilitation,and is worth being popularized.

背景:抑郁与焦虑经常出现在慢性躯体疾病患者中,通常这会加深这些躯体疾病所造成的损失,但是在中低等收入国家中这一问题却很少受到关注。目标:评估非专业临床人员和志愿者进行以社区为基础的心理干预对缓解慢性躯体疾病患者抑郁和焦虑症状的疗效。方法:将共计10,164名接受糖尿病或高血压治疗的上海社区居民任意分配到常规治疗组(n=2042)或干预组(n=8122),对干预组的干预包括社区范围的心理健康教育、同伴支持小组和个人咨询。采用自评患者健康问卷(Patient health Questionnaire,PHQ-9)、广泛性焦虑量表(Generalized Anxiety Disorder scale,GAD-7)和12项健康状况调查问卷(12-item Short-Form health Survey,SF-12)来评定基线和干预6个月后的抑郁症状、焦虑症状和生活质量。结果:8813人完成了基线评估,其中16%的人有轻度或较严重的抑郁或焦虑症状(PHQ-9或GAD-7>5),并有4%的人伴有中度或重度抑郁或焦虑症状(PHQ-9或GAD-7>10)。本研究有效实施了干预内容中的健康教育部分,但是在符合条件成为同伴支持小组的成员中仅31%的对象接受了干预措施,接受个人咨询的仅9%。本研究脱落率较高(51%),并且在完成和没有完成随访评估的人群之间存在显著差异。经过这些混杂因素的调整后,在完成两项评估的对象中,结果表明抑郁症状(F=9.98,p<0.001)、焦虑症状(F=12.85,p<0.001)以及SF-12中的心理部分总分(F=16.13,p<0.001)均得到显著改善。然而,自我报告未受控制的糖尿病或高血压的率没有显著变化。结论:这些结果支持了以社区为基础的干预措施的可行性,以降低在精神科人力资源有限的中低等收入国家中慢性疾病患者抑郁和焦虑症状的严重程度。然而,在确认该措施广泛大规模实施前还有大量方法学上的挑战需在未来研究中解决。

Thirty-year trends of anxiety disorders among adolescents based on the 2019 Global Burden of Disease Study

Bipolar disorder (BD) is a chronic, recurrent, disabling disease, even when given currently-available pharmacological and psychological treatments. Currently, the etiology and pathogenesis of BD remain unclear. As a consequence, patients with BD are frequently unrecognized, misdiagnosed, and inappropriately treated, which often yields a low treatment response and poor outcome.

Community capacity for sustainable community—based dengue prevention and control:study of a sub—district in Southern Thailand

Objective:To assess the level of community capacity for dengue prevention and control and to study household environments and larval indices in southern ***:A crosssectional survey was designed for the study,enrolling two communities with higher dengue incidence rate than the standard over the past five *** gathering was conducted by the dengue leader group(DLG),including 15 leaders and 15 non-leaders trained by the research *** dengue community capacity assessment tool(DCCAT) for leaders(115 items.14 domains) and non-leaders(83 items,11 domains).Participants were selected by the DLG based on their communities’ dengue *** the low-dengue incidence(LDI) community,32 leaders and 177 non-leaders were selected;while in the high-dengue incidence(HDI) community,31 leaders and 199 non-leaders were ***:The leaders from the LDI and the HDI communities demonstrated high levels of dengue community-capacity(360.47±58.82,416.22±57.72). Non-leaders in the LDI community demonstrated a moderate level of dengue community capacity competence(205.90±60.76),while the non-leaders in the HDI community had a high level (254.78±50.34).Conclusions:These initial levels of dengue community capacity serves as a baseline for diagnosing each *** a community that needed to improve its capacity, the DCCAT is essential tool to conduct a pre-post intervention assessment or a serial assessment. A participatory approach is taken to enable local communities to carry out anti-dengue efforts on their own,rather than have intervention by an outsider.

Partial Amputation of the Penis with Total Rupture of the Urethra: About a Case in the Urology Department of the Bonamoussadi district Medical center

Penis amputations are rare, they are most often observed in a criminal context or in the context of self-mutilation. We present the case of a partial amputation of the shaft in the context of a domestic accident. This is a 53-year-old patient received in the emergency department for partial amputation of the penis occurred, whose mechanism of occurrence would be the fall of a sheet metal from the roof of his house, with reception on the proximal end of the shaft leading to a partial rupture of it—here, with total urethral section. He has no medical-surgical history. The physical examination finds a good general condition, a partial tearing of the ventral face of the penis associated with a total rupture of the urethra followed by a hemorrhage, the rest of the examination was without particularity. The emergency assessment carried out was without particularity. The treatment consisted initially of catheterizing the urethra by a urinary tube ch 20, then a urethral replacement, followed by a Peno plastie. The surgical suites were enamelled with parietal suppuration plus necrosis at J 14 post-operative. A more bidaily dressing debridement followed with a favorable evolution at 2 months. Subsequently, a penile graft was performed more urethrolastic, with removal of the flap on the inner side of the thigh, the surgical suites were favorable on the sexual plane gradual resumption of night erections.

Comparison of sleep timing of people with different chronotypes affected by modern lifestyle

Circadian rhythm is an endogenous rhythmic behavior of organisms which can be entrained by daily light–dark *** timing of human sleep-cycle is regulated by endogenous circadian rhythm and homeostatic processes. Light exposure affects both sleep timing and circadian rhythm. Now humans can extend lighting time by turning on artificial lights and wake up time is usually triggered by alarm clocks to meet social schedules. This modern lifestyle is believed to be related with a temporal mismatch between sleep and circadian rhythmicity(social jet-lag) and insufficient sleep, which lead to ill mental and physical health outcomes. At present, the impacts of self-selection of light exposure and social constrains on sleep timing is far from clear. According to preferred sleep-wake schedule, there are three different chronotypes. In this paper, we apply a mathematical model to get a quantitative comparison of sleep timing of people with different chronotypes with the effects of modern light consumption and social constrains. The results show that the prolonged day light and evening light exposure both delay preferred sleep timing with the sleep duration almost unchanged. People of evening-type or with longer intrinsic periods are most expected to be vulnerable to evening light. Increasing light exposure can offset the effect of evening light to some extent, but it is most difficult for evening-type people. Social constrains cause the largest social jet-lag in people of evening-type, which increases with evening light intensity or intrinsic periods. Morning-type people's sleep symptoms worsens, while that of evening-type people improves with age. This study provides a theoretical reference for preventing and treating sleep disorder and social jet-lag for individuals with different chronotypes.

Early distinction of bipolar disorder(BD)from major depressive disorder(MDD)is difficult since no tools are available to estimate the risk of *** this study,we aimed to develop and validate a model of oxidative stress injury for predicting *** were collected from 1252 BD and 1359 MDD patients,including 64 MDD patients identified as converting to BD from 2009 through 2018.30 variables from a randomly-selected subsample of 1827(70%)patients were used to develop the model,including age,sex,oxidative stress markers(uric acid,bilirubin,albumin,and prealbumin),sex hormones,cytokines,thyroid and liver function,and glycolipid *** analyses and the Least Absolute Shrinkage and Selection Operator were applied for data dimension reduction and variable *** logistic regression was used to construct a model for predicting bipolar disorder by oxidative stress biomarkers(BIOS)on a *** validation was assessed in the remaining 784 patients(30%),and independent external validation was done with data from 3797 matched patients from five other hospitals in China.10 predictors,mainly oxidative stress markers,were shown on the *** BIOS model showed good discrimination in the training sample,with an AUC of 75.1%(95%CI:72.9%–77.3%),sensitivity of 0.66,and specificity of *** discrimination was good both in internal validation(AUC 72.1%,68.6%–75.6%)and external validation(AUC 65.7%,63.9%–67.5%).In this study,we developed a nomogram centered on oxidative stress injury,which could help in the individualized prediction of *** better real-world practice,a set of measurements,especially on oxidative stress markers,should be emphasized using big data in psychiatry.
