Seminal plasma miR-192a： a biomarker predicting successful resolution of nonobstructive azoospermia following varicocele repair

This study was performed to investigate a potential marker for the presence of spermatozoa in the ejaculate following varicocelectomy in Chinese men with nonobstructive azoospermia and varicoceles. The micro-RNA （miR）-192a levels in seminal plasma and testicular tissue were evaluated by quantitative real-time polymerase chain reaction from 60 men with nonobstructive azoospermia and varicoceles （Group A： 27 men with spermatozoa found in the ejaculate after surgery; Group B： 33 men without spermatozoa found in the ejaculate after surgery） and 30 controls. The seminal plasma and testicular tissue miR-192a levels were higher in Group B than in Group A and the controls （P〈 0.001）, and there was no significant difference between Group A and the controls （P〉 0.05）. Apoptosis and proliferation assays with miR mimics and inhibitors showed that miR-192a induced GC-2 cell apoptosis through the activation of Caspase-3 protein. Thus, seminal plasma miR-192a appears to be a potential marker for successfully indicating spermatozoa in the ejaculate following microsurgical varicocelectomy in men with nonobstructive azoospermia and varicoceles. Seminal plasma miR-192a may be a useful clinical marker for prescreening to determine which patients with nonobstructive azoospermia and varicoceles would benefit from varicocelectomy.

Objective:To uncover the underlying mechanism of Hewei Jiangni granule(HWJNG)on non-erosive reflux disease(NERD)treatment by examining histological changes,gastrointestinal neurochemicals release and visceral hypersensitivity-related receptor expression in NERD model ***:A NERD rat model was established via a combination of basal sensitization and acid *** treatments at different doses were then *** changes to tissues,mast cell(MC)activation,serum levels of esophageal visceral hypersensitivity-related neurochemicals,and transient receptor potential(TRP)receptor mRNA and protein levels were ***:Compared with the control group,the expression of tryptase in MCs,the changes of intercellular space,and the serum levels of substance P(SP),calcitonin gene-related peptides(CGRP)and proteinaseactivated receptor 2(PAR2)increased in the model group(all P<.05).The expression of TRP vanilloid 1(Trpv1)mRNA decreased in esophagus and dorsal root ganglia(DRG)of the model group(P=.030&P=.013),and the expression of TRP melastatin channel subfamily member 8(Trpm8)mRNA decreased in the esophagus of model group(P<.01).The level of esophageal TRPV1 protein increased in the model group(P<.01)and the level of TRPM8 protein decreased in esophagus and DRG of the model group(both P<.05).Compared with the model group,the serum levels of SP,CGRP,and PAR2 in the mediumdose HWJNG group showed significant decreases(all P<.05).The expression of Trpv1 mRNA in esophagus and DRG of the HWJNG groups and the Omeprazole group remarkably decreased(all P<.05),as was the expression of Trpm8 mRNA in esophagus of the HWJNG groups(all P<.05).Conclusion:HWJNG alleviated visceral hypersensitivity in NERD model rats by regulating TRP-mediated *** results indicate that HWJNG has potential as a therapeutic agent for NERD.

Oxaliplatin, an anticancer drug commonly used to treat colorectal cancer and other tumors, has a number of serious side effects, most notably neuropathy and ototoxicity. To gain insights into its ototoxic profile, oxaliplatin was applied to rat cochlear organ cultures. Consistent with it neurotoxic propensity, oxaliplatin selectively damaged nerve fibers at a very low dose 1 μM. In contrast, the dose required to damage hair cells and spiral ganglion neurons was 50 fold higher (50 μM). Oxailiplatin-induced cochlear lesions initial-ly increased with dose, but unexpectedly decreased at very high doses. This non-linear dose response could be related to depressed oxaliplatin uptake via active transport mechanisms. Previous studies have demon-strated that axonal degeneration involves biologically active processes which can be greatly attenuated by nicotinamide adenine dinucleotide (NAD+). To determine if NAD+would protect spiral ganglion axons and the hair cells from oxaliplatin damage, cochlear cultures were treated with oxaliplatin alone at doses of 10 μM or 50 μM respectively as controls or combined with 20 mM NAD+. Treatment with 10 μM oxaliplatin for 48 hours resulted in minor damage to auditory nerve fibers, but spared cochlear hair cells. However, when cochlear cultures were treated with 10 μM oxaliplatin plus 20 mM NAD+, most auditory nerve fibers were intact. 50 μM oxaliplatin destroyed most of spiral ganglion neurons and cochlear hair cells with apop-totic characteristics of cell fragmentations. However, 50 μM oxaliplatin plus 20 mM NAD+treatment great-ly reduced neuronal degenerations and hair cell missing. The results suggested that NAD+provides signifi-cant protection against oxaliplatin-induced neurotoxicity and ototoxicity, which may be due to its actions of antioxidant, antiapoptosis, and energy supply.

Objective The aim of this study was to clarify the outcomes of stereotactic radiotherapy for the treatment of local residual and(or) local recurrent nasopharyngeal carcinoma and to promote scientific clinical treatment and research on it in China and other countries by creating a large data *** This Meta-analysis conducted a literature search using the China National Knowledge Infrastructure database for all clinical research articles on stereotactic radiotherapy for the treatment of local residual and(or) local recurrent nasopharyngeal carcinoma in *** on patient cohort numbers and other research factors were recorded for every relevant clinical research *** and analyzed these fact sheets to pave the way for the objective of this *** A total of 40 clinical research articles including 1,371 patients in China from 1998 to 2012 were *** average cohort size was 34 patients(range 9-98 patients).The average total radiation dose range was 16-33.6 *** average study or treatment duration was 3.51 *** calculated average follow-up time was 31.59 *** The study provided the largest resource for further research and Meta-analyses to determine the clinical pathway of stereotactic radiotherapy for the treatment of local residual and(or) local recurrent nasopharyngeal *** results indicated that variability in the total radiation dose,treatment or research time,and follow-up duration may have contributed to the complications and side effects of stereotactic radiotherapy for local residual and(or) local recurrent nasopharyngeal carcinoma in *** calculated average total radiation dose,follow-up time,and treatment and research durations may be referenced for future treatment and *** study also proposed worldwide cooperation for the metaanalysis of research articles on stereotactic radiotherapy for treating local residual and(or) local recurrent nasopharyngeal carcinoma.

With the development of medicine, the pros and cons of both medical systems become increasingly evident. How to integrate them and learn from each other will be the direction of future development of medicine. The formation and development of integrated medicine will, inevitably, usher in a new era for medicine.

Acuta Kudo(PF) to increase endometrial ***: Herbal medicines were extracted with boiling water and polysaccharides were *** examined the effect of PL, RG, and PF(PRP), a most effective herbal formula deduced from constitutive ingredient herbs of Antai Yin which is composed of PRP, on the leukemia inhibitory factor(LIF) expression and endometrial ***: The combination of the extracts from PRP induced the LIF expression in Ishikawa cells and increased the adhesion between Ishikawa and JAr cells. In addition, PRP-induced attachment of JAr cells onto Ishikawa cells and expression of adhesion molecules, ITGAV, ITGB5, CD44 s, and Lselectin, are significantly reduced by knock-down of LIF ***: Induced by the combination of the PRP extracts, the adhesion between trophoblast and endometrial cells are mediated by expression of LIF and adhesion molecules. Thus, we suggest the combination of the PRP extracts may be a novel therapy for enhancing embryo implantation rate.

Influence of Acupuncture on Insulin Resistance in Simple Obesity Patients

Objective： To investigate the regulatory effect of acupuncture on insulin resistance in simple obesity patients and its clinical effect. Methods： By the method ＂to benefit qi, strengthen the spleen, dissolve phlegm and remove turbidity＂, eight periumbilical acupoints, Tianshu （ST 25）, Huaroumen （ST 24）, Wailing （ST 26）, Yinjiao （CV 7） and Shuifen （CV 9）, and Guanyuan （CV 4） and Zusanli （ST 36） were used, in cooperation with ear-points-embedding method, and together with 30 healthy persons for comparison, in order to observe the changes in FBG, INS, ISI, TC, TG, LDLC and HDLC and to observe the changes in the waist circumference, hip circumference, waist-hip ratio, body weight and BMI in simple obesity patients before and after acupuncture treatments, and to observe the differences of the therapeutic effects between grade Ⅰ and grade Ⅱ obesity. Results： Hyperinsulinemia （HI） existed in the patients with simple obesity before the treatment, and ISI significantly decreased （P〈0.01） in comparison with the healthy group, and waist circumference, hip circumference, waist-hip ratio, body weight and BMI significantly decreased （P〈0.05, P〈0.01）, and FBG, INS, TC, TG and LDLC also significantly decreased （P〈0.01, P〈0.05）, and ISI and HDLC significantly increased （P〈0.05） after the treatment. Conclusion： Acupuncturecan not only effectively improve the body fat parameters but also regulate the abnormal fat metabolism and improve insulin resistance status in simple obesity patients.

Influence of Acupuncture on Remodeling of Swallowing Functions for Patients with Pseudobulbar Palsy after Cerebral Infarction

Objective： To explore the interfering effect of acupuncture treatment in remodeling the swallowing functions for the patients with pseudobulbar palsy after cerebral infarction. Methods： Seventy-two cases with pseudobulbar palsy after cerebral infarction were randomly allocated into an observation group and a control group, 36 cases in each group. The treatment neurological therapies, once a day, with one day rest group was treated with acupuncture based upon routine after continuous treatment for six days, and therapeutic effects were assessed after six courses of the treatments. The control group was treated with routine neurological therapies, with the therapeutic effects observed after the treatment for 42 d. Results： The total effective rate was 80.6% in the observation group and 58.3% in the control group, with statistical significance in comparison of difference between the two groups （P〈0.05）, and with statistical significance in comparison of the score of deglutition disorder between the observation group and control group （P〈0.01）. Conclusion： Nape acupuncture can obviously improve the clinical symptoms and signs for the patients with pseudobulbar palsy, with affirmative therapeutic effects, and is worthy of popularization.
