Designing a Scale to Assess Family Nursing Practice among Public Health Nurses in Japan

Background: The promotion of family nursing by public health nurses (PHN) presupposes an accurate assessment of the family nursing support they already provide. However, as there is no assessment tool for this purpose, this study aimed to develop a scale to assess family nursing currently provided by PHN. Methods: We developed the Family Nursing Practice Scale (FNPS) for PHN based on the results of a previous study. The content validity of the FNPS was established through discussion with three other researchers. A pilot study was conducted to confirm face validity. To confirm reliability and validity, an anonymous, self-reported questionnaire was sent to PHN working in municipal offices. The statistical analyses included the Kaiser-Meyer-Olkin (KMO), Barlett’s Test of Sphericity, exploratory factor analysis (EFA), Cronbach’s alpha, correlation coefficient and t-test. Results: Seven hundred fifty-four PHN participated in this study. Cronbach’s alpha of FNPS was 0.94. The KMO measure was 0.948, and Bartlett’s Test of Sphericity was p < 0.01. Two factors together accounted for 63.2% of the variance in EFA. No items were excluded because of low loadings. Construct validity was confirmed through comparison with categories from a previous study. The correlation coefficient of FNPS and selected items of the Practice of Breastfeeding Support Scale was r = 0.56 (p < 0.01). The result of the t-test showed that the FNPS score of PNH who had received training in family nursing was significantly higher than that of PHN who had not (t = -2.0;p < 0.05). Conclusion: The reliability and validity of the FNPS were confirmed. The FNPS comprised 15 items and two factors. The score for “Active support for the family” was lower than “Support given with awareness of the family’s situation.” The findings of this study strongly suggested that the FNPS would be effective in clarifying the current state of family nursing provided by PHN and factors related to this activity and thus greatly ass

Modified docetaxel, cisplatin and capecitabine for stage Ⅳ gastric cancer in japanese patients: A feasibility study

AIM To evaluate the feasibility of chemotherapy including fluoropyrimidine, platinum and taxane with modified dosages for unresectable gastric cancer in japanese *** We performed a feasibility study of a modified docetaxel, cisplatin and capecitabine (DCX) regimen for stage Ⅳ gastric cancer. In particular, 30 or 40 mg/m^2 of docetaxel on day 1, 60 mg/m^2 of cisplatin on day 1, and 2000 mg/m^2 of capecitabine for 2 wk were administered every three *** Three patients were treated with modified DCX(m DCX) with 30 mg/m^2 docetaxel, and five patients were treated with this regimen with 40 mg/m^2 docetaxel. Grade 3 or 4 neutropenia was observed in six of the eight patients; no patients exhibited febrile neutropenia. Partial response was achieved in four of the eight patients. Three patients underwent gastrectomy, which achieved R0 resection without residual tumors in dissected lymph nodes. In one of these three patients, resected specimens revealed pathological complete response in the primary lesion and in lymph *** m DCX was well tolerated by japanese patients with stage Ⅳ gastric cancer. This regimen might be useful for allowing gastric cancer patients with distant lymph node metastasis to undergo conversion surgery.

Self-expanding metal stent placement and pathological alterations among obstructive colorectal cancer cases

BACKGROUND Experimental studies suggest that self-expanding metal stents(SEMSs)enhance the aggressive behavior of obstructive colorectal *** influence of SEMS placement on pathological alterations remains to be *** To determine whether SEMS placement is associated with molecular or pathological features of colorectal carcinoma *** Using a nonbiased molecular pathological epidemiology database of patients with obstructive colorectal cancers,we examined the association of SEMS placement with molecular or pathological features,including tumor size,histological type,American Joint Committee on Cancer(AJCC)-pTNM stage,and mutation statuses in colorectal cancer tissues compared with the use of transanal tubes.A multivariable logistic regression model was used to adjust for potential *** SEMS placement was significantly associated with venous invasion(P<0.01),but not with the other features examined,including tumor size,disease stage,mutation status,and lymphatic *** both the univariable and multivariable models with adjustment for potential factors including tumor location,histological type,and AJCC-pT stage,SEMS placement was significantly associated with severe venous invasion(P<0.01).For the outcome category of severe venous invasion,the multivariable odds ratio for SEMS placement relative to transanal tube placement was 19.4(95%confidence interval:5.24–96.2).No significant differences of disease-free survival and overall survival were observed between SEMS and transanal tube *** SEMS placement might be associated with severe venous invasion in colorectal cancer tissue,providing an impetus for further investigations on the pathological alterations by SEMSs in colorectal cancer development.

Portal vein thrombosis in liver cirrhosis

Portal vein thrombosis(PVT) is considered to be a frequent complication of liver cirrhosis. However, unlike PVT in patients without cirrhosis, very few data are available on the natural history and management of PVT in cirrhosis, despite its association with potentially life-threatening conditions, such as gastroesophageal bleeding and acute intestinal ischemia. Moreover, no consensus regarding PVT in cirrhosis exists. Suggested causes of PVT in cirrhosis include reduced portal blood flow velocity, multiple congenital or acquired thrombophilic factors, inherited or acquired conditions, and derangement of liver architecture. However, the understanding of PVT in cirrhosis is incomplete. In addition, information on the management of PVT in cirrhosis is inadequate. The aims of this review are to:(1) assemble data on the physiopathological mechanism, clinical findings, diagnosis and management of PVT in cirrhosis;(2) describe the principal factors most frequently involved in PVT development; and(3) summarize the recent knowledge concerning diagnostic and therapeutic procedures.

on February 6 th, 2018, the Symposium of Commemorating the 40 th Anniversary of Sino-japanese Treaty of Peace and Friendship organized by Chinese association for International Understanding was staged at Wanshou Hotel, Beijing. Liu Hongcai, Vice President of Chinese association for International Understanding and former Vice Minister of International Department of Central Committee of CPC attended and addressed the meeting. More than 30 representatives, respectively form Chinese social organizations and institutions such as All-China Youth Federation, All-China Women's Federation, Chinese association for japanese Studies, People's China, visiting representatives of Broad National Coalition of Liberty, Peace and Democracy, as well as from the Beijing branch of japanese non-governmental organizations and institutions such as Council of Local Authorities for International Relations, Beijing. Representatives from both China and Japan made heated discussion and actively contributed to themes such as giving full play to the role of nongovernmental organizations, enhancing bilateral communication and improving bilateral relations. Peritoneovenous shunt for intractable ascites due to hepatic lymphorrhea after hepatectomy

A peritoneovenous shunt has become one of the most effi cient procedures for intractable ascites due to liver cirrhosis.A case of intractable ascites due to hepatic lymphorrhea after hepatectomy for hepatocellular carcinoma that was successfully treated by the placement of a peritoneovenous shunt is presented.A 72-year-old japanese man underwent partial resection of the liver for hepatocellular carcinoma associated with hepatitis C viral *** hepatectomy,a considerable amount of ascites ranging from 800-4600 mL per day persisted despite conservative therapy,including numerous infusions of albumin and plasma protein fraction and administration of *** the patient's general condition deteriorated,based on the diagnosis of intractable hepatic lymphorrhea,a subcutaneous peritoneovenous shunt was *** patient's postoperative course was uneventful and the ascites decreased rapidly,with serum total protein and albumin levels and hepatic function improving *** intractable ascites due to hepatic lymphorrhea after hepatectomy,we recommend the placement of a peritoneovenous shunt as a procedure that can provide immediate effectiveness without increased surgical risk.
