BACKGROUND Experimental studies suggest that self-expanding metal stents(SEMSs)enhance the aggressive behavior of obstructive colorectal *** influence of SEMS placement on pathological alterations remains to be *** To determine whether SEMS placement is associated with molecular or pathological features of colorectal carcinoma *** Using a nonbiased molecular pathological epidemiology database of patients with obstructive colorectal cancers,we examined the association of SEMS placement with molecular or pathological features,including tumor size,histological type,American Joint Committee on Cancer(AJCC)-pTNM stage,and mutation statuses in colorectal cancer tissues compared with the use of transanal tubes.A multivariable logistic regression model was used to adjust for potential *** SEMS placement was significantly associated with venous invasion(P<0.01),but not with the other features examined,including tumor size,disease stage,mutation status,and lymphatic *** both the univariable and multivariable models with adjustment for potential factors including tumor location,histological type,and AJCC-pT stage,SEMS placement was significantly associated with severe venous invasion(P<0.01).For the outcome category of severe venous invasion,the multivariable odds ratio for SEMS placement relative to transanal tube placement was 19.4(95%confidence interval:5.24–96.2).No significant differences of disease-free survival and overall survival were observed between SEMS and transanal tube *** SEMS placement might be associated with severe venous invasion in colorectal cancer tissue,providing an impetus for further investigations on the pathological alterations by SEMSs in colorectal cancer development.

AIM To determine the predictive factors and impact of body weight loss on postgastrectomy quality of life(QOL). METHODS We applied the newly developed integrated questionnaire postgastrectomy syndrome assessment scale-45, which consists of 45 items including those from the Short Form-8 and Gastrointestinal Symptom Rating Scale instruments, in addition to 22 newly selected items. Between July 2009 and December 2010, completed questionnaires were received from 2520 patients with curative resection at 1 year or more after having undergone one of six types of gastrectomy for Stage Ⅰ gastric cancer at one of 52 participating institutions. Of those, we analyzed 1777 eligible questionnaires from patients who underwent total gastrectomy with Roux-en-Y procedure(TGRY) or distal gastrectomy with Billroth-I(DGBI) or Roux-en-Y(DGRY) procedures. RESULTS A total of 393, 475 and 909 patients underwent TGRY, DGRY, and DGBI, respectively. The mean age of patients was 62.1 ± 9.2 years. The mean time interval between surgery and retrieval of the questionnaires was 37.0 ± 26.8 mo. On multiple regression analysis, higher preoperative body mass index, total gastrectomy, and female sex, in that order, were independent predictors of greater body weight loss after gastrectomy. There was a significant difference in the degree of weight loss(P 25 kg/m2). Multiple linear regression analysis identified lower postoperative body mass index, rather than greater body weight loss postoperatively, as a certain factor for worse QOL(P < 0.0001) after gastrectomy, but the influence of both such factors on QOL was relatively small(R2, 0.028-0.080).CONCLUSION While it is certainly important to maintain adequate body weight after gastrectomy, the impact of body weight loss on QOL is unexpectedly small.

This study investigated self-efficacy in breastfeeding support among public health nurses (PHNs) working at maternal and child health (MCH) services in japanese municipalities, and its related factors such as education, working experience, breastfeeding experience, and postgraduate/continuous education. Directors of the MCH divisions were requested to select one PHN to answer the self-report questionnaires sent. The questionnaires were collected from October 2010 to January 2011. Of the 1750 questionnaires sent, 831 were returned (response rate: 47.5%);102 were excluded from analysis because of missing data (valid response rate: 41.7%). After adjusting the weights of PHN sampling and collection rates by prefecture and population size, Model 1 of the multiple regression analysis showed that the self-efficacy in breastfeeding support was significantly higher in large/urban municipalities (β = 0.13) and in the Kanto district. Model 2 revealed that self-efficacy was significantly associated with working experience (β = 0.13) and breastfeeding experience (β = 0.22) but not with educational background. Model 3 showed that self-efficacy was significantly associated with postgraduate education (β = 0.14) and continuous education (β = 0.12). This suggested that PHNs’ self-efficacy increased through self-enrichment. It is necessary to create training opportunities about breastfeeding support for PHNs—especially those working in remote and/or small municipalities— and to provide them with new and important information.

Studies have investigated the effects of androgen deprivation therapy(ADT)use on the incidence and clinical outcomes of coronavirus disease 2019(COVID-19);however,the results have been *** searched the PubMed,Medline,Cochrane,Scopus,and Web of Science databases from inception to March 2022;13 studies covering 84003 prostate cancer(PCa)patients with or without ADT met the eligibility criteria and were included in the *** calculated the pooled risk ratios(RRs)with 95%confidence intervals(CIs)to explore the association between ADT use and the infection risk of severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and severity of *** synthesizing the evidence,the pooled RR in the SARS-CoV-2 positive group was equal to 1.17,and the SARS-CoV-2 positive risk in PCa patients using ADT was not significantly different from that in those not using ADT(P=0.544).Moreover,no significant results concerning the beneficial effect of ADT on the rate of intensive care unit admission(RR=1.04,P=0.872)or death risk(RR=1.23,P=0.53)were ***,PCa patients with a history of ADT use had a markedly higher COVID-19 hospitalization rate(RR=1.31,P=0.015)than those with no history of ADT *** findings indicate that ADT use by PCa patients is associated with a high risk of hospitalization during infection with SARS-CoV-2.A large number of high quality studies are needed to confirm these results.
