Outcomes and patients' perspectives of transition from paediatric to adult care in inflammatory bowel disease

AIM: To describe the disease and psychosocial outcomes of an inflammatory bowel disease (IBD) transition cohort and their ***: Patients with IBD, aged > 18 years, who had moved from paediatric to adult care within 10 years were identified through IBD databases at three tertiary hospitals. Participants were surveyed regarding demographic and disease specific data and their perspectives on the transition process. Survey response data were compared to contemporaneously recorded information in paediatric service case notes. Data were compared to a similar age cohort who had never received paediatric IBD care and therefore who had not undergone a transition ***: There were 81 returned surveys from 46 transition and 35 non-transition patients. No statistically significant differences were found in disease burden, disease outcomes or adult roles and responsibilities between cohorts. Despite a high prevalence of mood disturbance (35%), there was a very low usage (5%) of psychological services in both cohorts. In the transition cohort, knowledge of their transition plan was reported by only 25/46 patients and the majority (54%) felt they were not strongly prepared. A high rate (78%) of discussion about work/study plans was recorded prior to transition, but a near complete absence of discussion regarding sex (8%), and other adult issues was recorded. Both cohorts agreed that their preferred method of future transition practices (of the options offered) was a shared clinic appointment with all key ***: Transition did not appear to adversely affect disease or psychosocial outcomes. Current transition care processes could be optimised, with better psychosocial preparation and agreed transition plans.

Approximately 40%of treatments of chronic and recurrent osteomyelitis fail in part due to bacterial *** aureus,the predominant pathogen in human osteomyelitis,is known to persist by phenotypic adaptation as small-colony variants(SCVs)and by formation of intracellular reservoirs,including those in major bone cell types,reducing susceptibility to *** infections with *** are difficult to treat;however,there are no evidence-based clinical guidelines addressing these infections in *** conducted a systematic review of the literature to determine the demonstrated efficacy of all antibiotics against intracellular *** relevant to osteomyelitis,including protein biosynthesis inhibitors(lincosamides,streptogramins,macrolides,oxazolidines,tetracyclines,fusidic acid,and aminoglycosides),enzyme inhibitors(fluoroquinolones and ansamycines),and cell wall inhibitors(beta-lactam inhibitors,glycopeptides,fosfomycin,and lipopeptides).The PubMed and Embase databases were screened for articles related to intracellular *** infections that compared the effectiveness of multiple antibiotics or a single antibiotic together with another treatment,which resulted in 34 full-text articles fitting the inclusion *** combined findings of these studies were largely inconclusive,most likely due to the plethora of methodologies ***,the reported findings in the context of the models employed and possible solutions for improved understanding are explored *** rifampicin,oritavancin,linezolid,moxifloxacin and oxacillin were identified as the most effective potential intracellular treatments,the scientific evidence for these is still relatively *** advocate for more standardized research on determining the intracellular effectiveness of antibiotics in *** osteomyelitis to improve treatments and patient outcomes.

Overnutrition causes hyperactivation of mTORC1-dependent negative feedback loops leading to the downregulation of insulin signaling and development of insulin *** osteoblasts(OBs),insulin signaling plays a crucial role in the control of systemic glucose *** utilized mice with conditional deletion of Rptor to investigate how the loss of mTORC1 function in OB affects glucose metabolism under normal and overnutrition dietary *** to the controls,chow-fed Rptorob−/−mice had substantially less fat mass and exhibited adipocyte ***,upon feeding with high-fat diet,mice with pre-and post-natal deletion of Rptor in OBs were protected from diet-induced obesity and exhibited improved glucose metabolism with lower fasting glucose and insulin levels,increased glucose tolerance and insulin *** leanness and resistance to weight gain was not attributable to changes in food intake,physical activity or lipid absorption but instead was due to increased energy expenditure and greater whole-body substrate ***-seq revealed an increase in glycolysis and skeletal insulin signaling pathways,which correlated with the potentiation of insulin signaling and increased insulin-dependent glucose uptake in Rptorknockout ***,these findings point to a critical role for the mTORC1 complex in the skeletal regulation of wholebody glucose metabolism and the skeletal development of insulin resistance.

In this paper, the numerical predictions of 3D hydrodynamics and power consumption in a vessel stirred by mul- tiple eccentrically located impellers are presented. The vessel is a fiat-bottomed cylindrical one equipped with six-curved bladed impellers. Aqueous solutions of xanthan gum are used, which have a shear thinning behavior with yield stress. The influence of several parameters on the mixing efficiency has been investigated, namely： the stirring rate, fluid theology, impeller number and impeller clearance from the tank bottom. Our predicted results are compared with other experimental data and a satisfactory agreement is found.

The paper describes a large-area analysis of the triggering zones of shallow landslides on a case of unsaturated layered volcanic air-fall(pyroclastic) soil deposits in Cervinara site(18 km^2),Southern *** physically-based model TRIGRS(Transient Rainfall Infiltration-Based Grid Regional Slope-Stability) is used,which is used with either saturated or unsaturated conditions and implemented in a GIS *** addition to using the TRIGRS model to simulate some recent landslides,a new simplified approach is also tested to take into account the actual layered soil *** consistency check of the model and of the input data is performed with reference to slope stable conditions before *** performances of the models are evaluated through the ROC curves and two other quantitative indexes taken from the literature referring to the slope failures caused by December 1999 *** the simplifications and limitations of the present work,both unsaturated conditions and layered stratigraphy are outlined as key factors for the slope stability of shallow deposits of unsaturated coarse-grained soils subjected to short heavy rainfall.

Zinc-ion batteries(ZIBs)have emerged as a promising candidate in the grid scale energy storage,offering an alternative to conventional lithium-ion ***,as research and development efforts in this field progress,it becomes increasingly important to address potential hazards associated with flammable gases and electrolyte *** future paper aims to raise awareness of safety issues that may be overlooked and underscore two primary temperature-induced hazards:long-term cycling H_(2) gas accu-mulation and flammable organic *** evaluate various electrolytes aimed at minimizing gas pro-duction and improving high-temperature ***,we emphasize the effectiveness of electrolyte modifications,especially co-solvent approach in addressing the above safety concerns and suggest pathways for formulating practical safe ***,apart from examining the inherent electrochemical stability window(EsW)of the electrolyte,we urge consideration of side reaction prod-ucts on electrode interfaces and high temperatures as factors influencing a battery's EsW.
