Objective：To explore the dose-effect relationship of Astragalus granule（AG） on improving the quality of life（QOL） of the patients with chronic heart failure（CHF）.Methods：Ninety CHF patients of Fei（肺）-qi deficiency and/or Xin（心）-Shen（肾） yang-deficiency syndromes were equally randomized divided with a random number table into three groups;they received the high（7.5 g）,moderate（4.5 g）,and low dosage（2.25 g） of AG orally taken twice a day,respectively,and 4 mg of perindopril tablet once a day for 30 successive *** heart function grade,patients＇ left ventricular ejection fraction（LVEF） and walking distance in 6 min（6mWD） were measured before and after treatment,and the patients＇ QOL was scored by the Minnesota Questionnaire for QOL evaluation in the patients with CHF at the same ***：The heart function grades of all the three groups after treatment were improved compared with those before treatment,but the improvements in high-dose group and moderate dose group were better than that in the low dose group（P0.05）.LVEFs were increased significantly in all the three groups,but the improvements in the high-dose group（59.42%±7.50%） and moderate dose group（61.98%±6.82%） were better than that in the low dose group（51.45%±6.80%,P0.01）; the 6mWDs in the all groups were also significantly increased（P0.01）,up to 419.80136.23 m,387.15134.13 m, and 317.69±39.97 m,respectively;and Minnesota scores in them were lowered to 29.59±4.69 scores,35.74±5.89 scores,and 42.78±6.06 scores,respectively;comparisons in aspects on 6mWD and Minnesota score showed that the effectiveness with high dose is the most effective,moderate dose as the second,and low dose as the lowest（P0.01）.Conclusions：AG was sufficient to display an optimal effect on improving heart contraction at the moderate *** aspects of improving the QOL of CHF patients,the effectiveness of AG showed a dosedependent *** should be applied discriminatively depending on the actual condition of patients and the aim of

Objective:To determine post-discharge mortality and associated factors of the first-wave multicenter Turkish Thoracic Society(TTD)-TURCOVID ***:In this retrospective cohort study,we analyzed the data of 18 of 26 centers included in the first TTD-TURCOVID study,and 1112 cases diagnosed with COVID-19 between 11 March and 31 July 2020 participated in the *** causes of death after COVID-19 discharge were ***:The mean age of the patients was(51.07±16.93)years,with 57.6%male *** the cohort group,89.1%of COVID-19 treatment locations were hospital wards,3.6%were intensive care units(ICUs),and 7.2%were community *** the longterm follow-up,the in-hospital mortality rate was 3.6%(95%CI 2.6-4.8),the post-discharge mortality rate was 2.8%(95%CI 1.9-3.9),and the total mortality was 6.3%(95%CI 5.0-7.8).After discharge,63.3%of mortality overall occurred during the first six *** rates in post-discharge follow-ups were 12.7%(95%CI 8.0-30.6)in cancer patients,10.8%(95%CI 6.3-22.9)in chronic obstructive pulmonary disease patients,11.1%(95%CI 4.4-22.7)in heart failure patients,7.8(95%CI 3.8-14.3)in atherosclerotic heart disease patients,and 2.3%(95%CI 0.8-5.6)in diabetes mellitus *** smokers/ex-smokers,the all-mortality rates were higher than in ***:This multicenter study showed that patients over 65 years of age,males,former/active smoker,ICU stay,lung,heart disease,and malignancy should be followed up for at least the first six months after discharge due to COVID-19.

Traumatic brain injury induces potent inflammatory responses that can exacerbate secondary blood-brain barrier（BBB） disruption, neuronal injury, and neurological dysfunction. Dexmedetomidine is a novel α2-adrenergic receptor agonist that exert protective effects in various central nervous system diseases. The present study was designed to investigate the neuroprotective action of dexmedetomidine in a mouse traumatic brain injury model, and to explore the possible mechanisms. Adult male C57 BL/6 J mice were subjected to controlled cortical impact. After injury, animals received 3 days of consecutive dexmedetomidine therapy（25 μg/kg per day）. The modified neurological severity score was used to assess neurological deficits. The rotarod test was used to evaluate accurate motor coordination and balance. Immunofluorescence was used to determine expression of ionized calcium binding adapter molecule-1, myeloperoxidase, and zonula occluden-1 at the injury site. An enzyme linked immunosorbent assay was used to measure the concentration of interleukin-1β（IL-1β）, tumor necrosis factor α, and IL-6. The dry-wet weight method was used to measure brain water content. The Evans blue dye extravasation assay was used to measure BBB disruption. Western blot assay was used to measure protein expression of nucleotide-binding oligomerization domain-like receptor family pyrin domain-containing 3（NLRP3）, caspase-1 p20, IL-1β, nuclear factor kappa B（NF-κB） p65, occluding, and zonula occluden-1. Flow cytometry was used to measure cellular apoptosis. Results showed that dexmedetomidine treatment attenuated early neurological dysfunction and brain edema. Further, dexmedetomidine attenuated post-traumatic inflammation, up-regulated tight junction protein expression, and reduced secondary BBB damage and apoptosis. These protective effects were accompanied by down-regulation of the NF-κB and NLRP3 inflammasome pathways. These findings suggest that dexmedetomidine exhibits neuroprotective effects

A 72-year-old man with abdominal aortic aneurysm and angina pectoris underwent coronary artery bypass grafting (CABG) prior to abdominal aortic surgery. Perioperatively, he developed thrombocytopenia ((29,000 mm3), which was suggested as heparin-induced thrombocytopenia. Cardiac status was also aggravated at the same time, suggestive of bypass graft thrombosis. The results of platelet factor 4 (pf4) antibody test were negative, but platelet aggregation test was positive for heparin. Heparin was immediately discontinued, and replaced by argatroban;the patient’s cardiac status improved. One month later, he underwent abdominal aortic surgery using argatroban without issues related to anticoagulation and hemostasis. Autologous donated fresh blood prepared by the “switch-back” method was also used in this case, and its value was confirmed.

Aim: The ongoing COVID-19 pandemic is an acute medical, social, political and economic problem. Acute myocarditis is a rare complication of the widely used mRNA-based vaccines. Case Presentation: 20-year old male with no preexisting disease or cardiovascular risk factor presented in October 2021 with chest pain after receiving the second dose of the Moderna COVID-19 vaccine 2 days previously. He discharged from ward care only a few days after his initial presentation. Conclusion: Despite the meta-analysis results suggesting a higher risk of myocarditis with COVID-19 vaccination, vaccination should still be recommended because benefits of the vaccine likely outweigh its harms.
