背景：膝关节前髁解剖形态存在明显性别差异的观点已使存全膝关节假体设计时专门为女性患者设计了薄型前髁假体。我们通过对自己的患者进行观察，发现前髁的解剖形态变异很大，不仅取决于患者的性别，还取决于患者的身材、身高和人种。鉴于所发现的膝关节前髁解剖存在较大变异，我们试图明确是否真正存在前髁解剖的性别差异。方法：我们对212例随机挑选患者的MRI（男112例，女100例）进行评估。前髁定义为膝关节间隙以上10mm的股骨前侧皮质前方的骨性区域。前髁的内、外侧高度直接从MRI两个层面的标记值读出，以毫米为单位。经计算得出的髁高横径比用来确定患者的身材对前髁的大小是否有影响。

There exist technical challenges in the total hip arthroplasty for high dislocation of the hip in DDH to locate the true acetabulum without subtrochantric femoral shortening *** purpose of this study was to describe and share our experience of total hip arthroplasty without subtrochanteric femoral shortening osteotomy in 28 hips with Crowe Ⅲ and Ⅳ dislocation,by strong traction accompanying hip and knee flexion,continuing to push the artificial femur head,and rocuronium 0.9 mg/ kg(three times ED95) injected into veins one minute before *** of them had dislocation,prosthesis loosen,or other severe *** 4 patients resulted in the split fracture,and 4 patients whose limbs had been lengthened more than 4cm suffered from the numb of calf skin and recovered one month *** is safe and feasible for reduction of arthroplasty for Crowe Ⅲ or Ⅳ dislocation of the hip DDH by this technique.

The replacement of high dislocation hip(DDH) is easy to injure the sciatic nerve in ture acetabulum and without the femur shortening *** study is to explore the posture of lower extremities that may avoid the sciatic nerve *** prepared four adult dogs’fresh corpses whose sciatic nerve tension was measured with tension sensor and whose displacement of proximal end of sciatic nerve was measured accompanying the hip and knee *** found that the sciatic nerve tension is 25N～39N(30±7N) at the hip and knee full extension, that the sciatic nerve tension is zero Newton at flexion hip 60°and
