Ethnic variations in ulcerative colitis: Experience of an international hospital in Thailand

AIM: To investigate the clinical characteristics, treatment, medication use, and treatment response in patients with ulcerative colitis(UC) across ethnic ***: This study retrospectively analyzed medical records of all 268465 patients who visited the Bumrungrad international Digestive Disease Center during 2005-2010. The demographics, clinical characteristics, medication use, results of investigations, and medical and surgical management for patients with UC were evaluated. Evaluation included sigmoidoscopy and colonoscopy performed in compliance with the American Society of Gastrointestinal Endoscopy practice guidelines. Patient ethnicities were categorized into seven groups: Thai, Oriental, South Asian(SA), Middle Eastern(ME), Caucasian, African, and Hispanic. UC pathological severity was classified into inactive, mild, moderate, and severe. Associations between categorical variables were analyzed using the χ2 or Fischer's exact test. Associations between categorical and interval variables were analyzed usingStudent's t-test and/or analysis of ***: UC was diagnosed in 371 of the 268465 patients: male 56.33%; ME 42%, Caucasian 23%, and Thai 19%. Annual incidence of UC was 82 cases per 100000 with wide ethnic variation, ranging from 29 to 206 cases per 100000 in Oriental and ME patients, respectively. Of the patients with UC, 16.71% had severe UC with highest incidence among the patients from ME(20.39%) and lowest among the Caucasian population(11.90%). ME had highest proportion of pancolitis(52.90%), followed by Caucasian(45.35%) and Asian(34.40%). Only 20.93% of Caucasian patients received steroid, compared with 26.40% and 27.10% of Asian and Middle Eastern, respectively(P = 0.732). Overall, 13.72% of UC patients did not respond to steroid therapy, with non-significantly higher proportions of non-responders among Asian and Middle Eastern patients(15.22% and 15.04%, respectively)(P = 0.781). Prevalence of Coinfection Malaria-Covid-19 at the international hospital Center of Kinshasa during the 3^rd Wave of the Pandemic

Background: Since 2019, Covid-19 pandemic has afflicted the world and countries of Africa. Despite the limited resources, these countries already disturbed by multiple diseases that have not yet been controlled such as malaria, must face this pandemic whose success in the management depends on the early detection of the disease. The objective of this study was to determine the prevalence of Malaria-Covid-19 coinfection in our environment. Methods: This was a retrospective analysis of patients’ data with Covid-19 infection from May to July 2021 at the international hospital center of Kinshasa “CHIK”. We collected data and analysis was performed on the sociodemographic parameters, the notion of anticovid-19 vaccination as well as the duration of the symptomatology before the consultation, the clinical manifestations and the laboratory data available while including the data of the thick drop. Results: A total of 84 patients were registered with an average age of 35.23 ± 12.74 years. The male sex was predominant (82.1%). The Indian community was the most affected (44.2%). The average of days elapsed before the consultation of 3.63 days. The anti-Covid-19 vaccination rate was 20.3%. The prevalence of Malaria-Covid-19 coinfection was 29.76%. In coinfected patients, fever and cough were more reported (64%). Regarding biological and inflammatory parameters, 31.8% of coinfected patients had a platelet count less than 150,000 elements/mm3 compared to 11.6% in non-Co-infected (p = 0.046). Conclusion: The Malaria-Covid-19 comorbidity prevalence is high in Malaria endemic country like Democratic Republic of Congo (DRC). It is necessary to make better distinction, to detect early the comorbidity in order to better guide care and not be limited to treating malaria, letting the Covid-19 evolve.

Background: Intensity Modulated Radiation Therapy (IMRT) is currently employed as a major arm of treatment in multiforme glioblastoma (GBM). The present study aimed to compare 3D-CRT with IMRT to assess tumor volume coverage and OAR sparing for the treatment of malignant gliomas. Materials and methods: We assessed 22 anonymized patients datasets with High Grade Glioblastoma who had undergone post-operative Intensity Modulated Radiotherapy (IMRT) and 3D Conformal Radiotherapy (3D-CRT), This study will compare and contrast treatment plans Rapidarc and 3D-CRT to determine which technology improves significantly dosimetric parameters. Results: Plans will be assessed by reviewing the coverage of the PTV using mean, maximum and minimum doses while the OAR doses will be compared using the maximal doses for each, as set out in the QUANTEC dose limits. Consensus on glycemic management for patients with coronary heart disease and type 2 diabetes

The prevalence of patients with coronary heart disease(CHD)and diabetes mellitus is notably high,posing sig-nificant residual cardiovascular risks even after routine interventions such as antihypertensive,lipid-lowering,and antithrombot-ic *** studies have demonstrated that certain glucose-lowering medications confer cardiovascular benefits for pa-tients with type 2 ***,a survey indicates that cardiologists may not be fully acquainted with the optimal screen-ing timing,indicators,and diagnostic criteria for type 2 diabetes,and there is insufficient awareness and a low rate of prescrip-tion of novel glucose-lowering medications with proven cardiovascular efficacy,such as glucagon-like peptide-1 receptor agon-ists(GLP-1 RAs)and sodium-glucose co-transporter-2 inhibitors(SGLT-2i).In this context,based on domestic and international guidelines or consensus and the latest evidence-based evidence,this consensus aims to standardize the glycemic management for patients with acute coronary syndrome,chronic coronary syndrome,and perioperative management for percutaneous coronary *** highlights the key points of screening and diagnosis of type 2 diabetes,and the comprehensive management of cardiovascular risk in patients with *** consensus elaborates on the principles and algorithms of glycemic management for CHD patients,without involving acute complications of diabetes,clarifies the clinical practice of glucose-lowering medications with cardiovascular benefits,and promotes the standardized use of these medications in cardiovascular and other related spe-cialty ***,it addresses the glucose-lowering treatment to comprehensively reduce cardiovascular risks.

BACKGROUND Nutritional support for patients hospitalized in the intensive care unit(ICU)is an important part of clinical treatment and care,but there are significant implementation *** To introduce a modified nutritional support management system for ICU patients based on closed-loop information management and psychological *** The division of functions,personnel training,system construction,development of an intelligent decision-making software system,quality control,and improvement of the whole process were carried out to systematically manage nutritional support for ICU *** Following the implementation of the whole process management system,the scores of ICU medical staff’s knowledge,attitudes/beliefs,and practices regarding nutritional support were comprehensively *** proportion of hospital bed-days of total enteral nutrition(EN)in ICU patients increased from 5.58%to 11.46%,and the proportion of EN plus parenteral nutrition increased from 42.71%to 47.07%.The rate of EN initiation within 48 h of ICU admission increased from 37.50%to 48.28%,and the EN compliance rate within 72 h elevated from 20.59%to 31.72%.After the implementation of the project,the Self-rating Anxiety Scale score decreased from 61.07±9.91 points to 52.03±9.02 points,the Self-rating Depression Scale score reduced from 62.47±10.50 points to 56.34±9.83 points,and the ICU stay decreased from 5.76±2.77 d to 5.10±2.12 *** The nutritional support management system based on closed-loop information management and psychological counseling achieved remarkable results in clinical applications in ICU patients.

Significance of oxidative stress and antioxidant capacity tests as biomarkers of premature ovarian insufficiency: A case control study
