Results of the Evaluation of the Respiratory Condition of Permanent Workers in an Oil Mill in Benin

Introduction: The process of extracting oil from cotton seeds can create dusty work atmospheres that can cause respiratory problems. The main objective of this study was to determine the prevalence of respiratory problems among permanent workers in an oil mill in Benin. Methods: This cross-sectional study of 52 workers in an oil mill took place in January 2017 as part of the annual medical check-ups of workers. A questionnaire was administered and spirometry using Spirobank II and chest radiography were performed. The spirometry results were interpreted by an occupational physician and a pulmonologist. Data were entered and analyzed using Epidata software. Results: The mean age was 42.7 ± 6.4 years, and 43 of the 52 workers were men. Of these, 58% were in technical production positions and 42% in administrative positions. Most of them had more than 10 years of seniority. The prevalence of respiratory symptoms among production workers was 4 (13%) versus 2 (9%) among administrative workers. A total of 8 (15.4%) abnormal spirometry was identified with 4 obstructive syndrome, 3 restrictive syndrome, 1 a mixed pattern. There were 5 (16.6%) workers in production versus 3 (13.6%) in administration who had abnormal spirometry. The means 25/75 forced expiratory flow (FEF) value of production workers was significantly lower than that of administration workers. Abnormal chest radiographs were 5 (17%) in production workers compared to 3 (14%) in administration workers. Conclusion: Oil mill workers had few respiratory symptoms. However, production workers had more ventilatory disorders than administrative workers. A spirometric follow-up of this group of workers is therefore necessary.

Sectoral changes of the peripapillary choroidal thickness in patients with unilateral branch retinal vein occlusion

AIM: To investigate sectoral changes in the mean peripapillary choroidal thickness(PCT) in patients with unilateral branch retinal vein occlusion(BRVO). METHODS: This retrospective, interventional study included 41 patients with acute, unilateral BRVO without macular edema. All patients completed at least a 6-month follow-up period. The PCT was measured at eight locations(temporal, superotemporal, superior, superonasal, nasal, inferonasal, inferior, and inferotemporal). In addition to calculating the average of all locations, the peripapillary choroidal area was divided into four sectors: superior(average of superotemporal PCT, superior PCT, and superonasal PCT), temporal, inferior(average of inferotemporal PCT, inferior PCT, and inferonasal PCT), and nasal. RESULTS: In the BRVO-affected eyes, the mean PCT was 177.7±69.8 μm(range, 70.1-396.0 μm) at baseline and 127.8±54.8 μm(range, 56.4-312.1 μm) at 6 mo(P<0.001). In the non-affected contralateral eyes, the mean PCT was 192.5±60.6 μm(range, 61.4-365.0 μm) at baseline and 165.9±61.1 μm(range, 56.8-326.8 μm) at 6 mo(P<0.001). In sectoral analysis, the mean PCT in each sector was significantly reduced in over 6 mo in the BRVO-affected eyes(all P<0.001). In the non-affected contralateral eyes, the mean PCT was not significantly changed in any sector over the 6-month follow-up period(superior sector, P=0.143; temporal sector, P=0.825; inferior sector, P=0.192; and nasal sector, P=0.599).CONCLUSION: Sectoral analysis shows that the mean PCTs in all sectors are reduced significantly over 6 mo in the BRVO-affected eyes, but not in the non-affected contralateral eyes.

AIM: To compare two different anti-vascular endothelial growth factor(anti-VEGF) treatment regimens'-a priori pro re nata(PRN) and PRN regimen following^(th)e loading phaseanatomical and functional results in neovascular agerelated macular degeneration(n AMD) patients. METHODS: Totally 544 n AMD patients followed and treated with aflibercept(n=135) and ranibizumab(n=409)at 9 different centers between 2013 and 2015 were enrolled into^(th)is retrospective multicenter study. Patients with initial best corrected visual acuity(BCVA) interval of 1.3-0.3(log MAR) and a minimum follow-up of 12 mo were included. Patients under two different regimens-a priori pro re nata(1+PRN) or 3 consecutive intravitreal injections followed by a PRN regimen(3+PRN)-were compared in BCVA at 3^(th), 6^(th) and 12^(th) months, and in central macular^(th)ickness(CMT) at 6^(th) and 12^(th) months. The total study group, intravitreal ranibizumab(IVR) and intravitreal aflibercept(IVA) groups were evaluated separately. RESULTS: The mean CMT decreased in^(th)e 1+PRN(n=101) regimen from 407 to 358 and 340 μm and in^(th)e 3+PRN(n=443) group from 398 to 318 and finally to 310 μm at months 6 and 12, respectively. Anatomically,^(th)e CMT reduction at 6^(th) month(48.5 vs 76.4;P<0.05) was statistically significant in favor of 3+PRN group. BCVA changed in 1+PRN group from 0.77 to 0.78, 0.75 and 0.75;in 3+PRN group from 0.81 to 0.69, 0.72, and 0.76 at months 3, 6, and 12, respectively. Visual gain was statistically better in 3+PRN group at 3^(th) month(-0.01 vs 0.12;P<0.001). In IVR group, CMT reduction was in greater in 3+PRN at 6^(th)(44 vs 72) and 12^(th) month(61 vs 84), but statistically insignificant. The 3+PRN group revealed statistically better visual results at 3^(th) month(-0.02 vs 0.11, P<0.05). In IVA group, although statistically insignificant, CMT reduction(61 vs 89, 6^(th) month;85 vs 97, 12^(th) month) and visual gain(0.02 vs 0.16;0.02 vs 0.14;0.05 vs 0.11) was found in favor of 3+PRN group

Introduction: Because of the specificity of their care activities, hospital wastewater contains various contaminants such as germs, disinfectants, metals, pharmaceuticals and chemical reagents, potentially infectious or toxic, which can be harmful to living organisms, or create ecological damage. This study aimed to analyze the quality of wastewaters produced by university hospitals of cotonou in Benin. Methods: We conduct an analytical cross-sectional study including 30 wastewater samples from selected hospitals. An analysis of variance was performed at the significance level of 0.05. Results: Microbiological analysis showed the presence of germs such as total coliform types, faecal coliforms, fecal streptococcus, staphylococcus, yeasts and Clostridium perfringens in hospital wastewater samples. There was a significant difference (p Conclusion: The wastewater produced by the university hospitals of cotonou is of poor quality and therefore requires adequate prior treatment before discharge into the environment.

Frequency of Hypovitaminosis D in 6 - 59 Month Children with Severe Malaria in the Pediatrics Unit of the Teaching Hospital of Parakou (CHUD/BA) in Benin in 2016

Introduction: Vitamin D’s action outside of bone, especially on immunity, is widely reported in the international scientific literature over the last years. Objective: Calculate the frequency of hypovitaminosis D in children aged 6 to 59 months suffering from severe malaria in the CHUD-P pediatric unit in 2016. Setting and Methods: This research work is a cross-sectional study with descriptive and analytical purposes. Data gathering was prospective. The study involved children aged 6 to 59 months hospitalized for severe malaria in the CHUD-P pediatric unit. The said children were HIV-uninfected, eutrophic and had not received vitamin D supplementation during the last 6 months. Vitamin D dose was measured using the High Performance Liquid Chromatography (HPLC) technique. Results: A total of 80 subjects were involved in the survey. Mean age was 26.08 months, sex ratio was 0.8 and average weight was 10.80 kg. Hypovitaminosis D frequency was 83.8% (67 cases out of 80 children investigated during the survey) with an average plasma concentration of vitamin D estimated at 21.57 ng/ml ± 7.34 with two extremes (11.24 - 42.32) ng/ml. The minimum parasitaemia was 202 P/μl and the maximum was 580,000 P/μl. Conclusion: Hypovitaminosis D is common in children suffering from severe malaria;this result suggests conducting a large-scale community-based study to decide on vitamin D inclusion in national supplementation policies and severe malaria management.

Management of Breast Cancer in Visceral Surgery of CNHU-HKM of cotonou in Benin

Breast cancer affects more females than males. It occurs earlier with the black females and is often diagnosed at a stage of complication. This study aims to describe its epidemiological characteristics, the changing stages, and to analyze the treatment and means of interdisciplinary and international cooperation in its care and treatment. Our study was retrospective, descriptive and analytic on breast cancer patients received at CNHU-HKM of cotonou between January 1, 2001 and December 31, 2013. Breast cancer accounts for 24.24% of mammary pathologies and most affects the left breast. Females aged from 40 to 59 years old are the most affected. For males, the average age was 47.5 years old. Patients mostly consult at a later stage where the tumor is already palpable with nodal metastasis. Breast echography and mammography had been done only for 23.85% of the cases. In an anatomical pathology perspective, the invasive ductal carcinoma was the most common type with 87.2%. For the therapeutical aspect, a radical mastectomy according to Patey was realized in 81.65% of the cases;48.65% underwent a chemotherapy and 14.7% were able to do a radiotherapy. The chance at life for patients was decreasing while the life time is increasing. Also, the average life span of patients was increased by the combination of diverse types of treatments notably when radiotherapy is done. The incidence of breast cancers in Visceral Surgery A and B department of CNHU-HKM-cotonou is increasing over years.

Treatment of ocular rosacea: comparative study of topical cyclosporine and oral doxycycline

AIM: To compare the effectiveness of topical cyclosporine A emulsion with that of oral doxycycline for rosacea associated ocular changes and dry eye ***: One hundred and ten patients with rosacea were screened. Thirty-eight patients having rosacea associated eyelid and ocular surface changes and dry eye complaints were included in the study. Patients were randomly divided into two groups: nineteen patients were given topical cyclosporine twice daily and nineteen patients were given oral doxycycline 100 mg twice daily for the first month and once daily for the following two months. Symptom and sign scores, ocular surface disease index questionnarie and tear function tests were evaluated at baseline and monthly for 3mo. Three months after results were compared with that of ***: Mean values of symptom, eyelid sign and corneal/conjunctival sign scores of each treatment group at baseline and 3mo after treatments were compared and both drugs were found to be effective on rosacea associated ocular changes(P <0.001). Cyclosporine was more effective in symptomatic relief and in the treatment of eyelid signs(P =0.01). There was statistically significant increase in the mean Schirmer score with anesthesia and tear break up time scores in the cyclosporine treatment group compared to the doxycycline treatment group(P <0.05).CONCLUSION: Cyclosporine as a topical drug can be used in the treatment of rosacea associated ocular complications because it is more effective than doxycycline. In addition ocular rosacea as a chronic disease requires long term treatment and doxycycline has various side effects limiting its long term usage.
