Predictive value of C-reactive protein and the Pediatric Risk of Mortality Ⅲ Score for occurrence of postoperative ventilator-associated pneumonia in pediatric patients with congenital heart disease

Importance:Ventilator-associated pneumonia (VAP) is one of the most common complications after cardiac surgery in children with congenital heart disease (CHD).Early prediction of the incidence of VAP is important for clinical prevention and ***:To determine the value of serum C-reactive protein (CRP) levels and the Pediatric Risk of Mortality Ⅲ (PRISM Ⅲ) score in predicting the risk of postoperative VAP in pediatric patients with ***:We performed a retrospective review of clinical data of 481 pediatric patients with CHD who were admitted to our pediatric intensive care *** patients received mechanical ventilation for 48 hours or longer after corrective *** the basis of their clinical manifestations and laboratory results,patients were separated into two groups of those with VAP and those without *** levels were measured and PRISM Ⅲ scores were collected within 12 hours of admission to the pediatric intensive care *** Pearson correlation coefficient was used to evaluate the association of CRP levels and the PRISM score with the occurrence of postoperative VAP.A linear regression model was constructed to obtain a joint function and receiver operating curves were used to assess the predictive ***:CRP levels and the PRISM Ⅲ score in the VAP group were significantly higher than those in the non-VAP group (P heart surgery was more accurate than using either of them alone (CRP + the PRISM Ⅲ score:sensitivity:53.2%,specificity:85.7%).When CRP + the PRISM Ⅲ score was greater than 45.460,patients were more likely to have ***:Although using CRP levels plus the PRISM Ⅲ score to predict the incidence of VAP after congenial heart surgery is more accurate than using either of them alone,its predictive value is still limited.

Sexually transmitted infection (STI) management is considered rudimentary among rural medical practitioners (RMPs) in Bangladesh. We sought to understand the level of knowledge and skills in STI management and to assess the impact of a two-day training orientation among RMPs in Tangail district. Data were collected through a baseline survey of 225 practicing RMPs in the study area and a three-month follow-up survey of 99 RMPs who participated in a two-day STI/HIV orientation training. The level of formal training among RMPs ranged from none (22.7%), to paramedical training (14.7%) and local medical assistant training (62.6%). The baseline survey revealed a low level of STI/HIV knowledge and misconceptions about the transmission of STI/HIV among RMPs. RMPs mostly prescribed first line antibiotics for treatment of common reproductive tract infections (RTIs) including STIs, but they rarely prescribed the correct dosages according to the national RTI/STI management guidelines. Only 3% of RMPs were able to correctly answer all four HIV transmission (unprotected sexual intercourse, blood transfusion, needle sharing and mother to child transmission) questions at baseline, while 94.9% of RMPs answered all four correctly at three months following the training (p = 0.001). Only 10% of RMPs reported suggesting the recommended drug (azithromycin) and only 2% mentioned about the recommended dosage (2 gm single dose) for the treatment of urethritis/cervicitis;compared to 49.5% suggested azithromycin at follow-up with 39.4% mentioned the recommended 2 gm single dose (p = 0.001). Our study found low level of knowledge and poor practices related RTI/STI management among RMPs. Short orientation training and education intervention shown promise to increase knowledge and management skills for RTIs/STIs.

[Objectives]To establish a method for caffeic acid content determination and thin-layer chromatography identification of the Zhuang medicine Argyreia acuta Lour.[Methods]Silica gel GF254(thin-layer plate),toluene-formic acid-ethyl acetate(V∶V∶V=5∶1.2∶3)(developing agent)and 365 nm ultraviolet light were used for thin-layer chromatography *** the chromatographic conditions of column of Thermo SCIENTIFIC Hypersil C18(5μm,4.60 mm×250 mm),mobile phase of methanol-0.1%phosphoric acid(23∶77),detection wavelength of 320 nm,column temperature of 30℃,sample size of 10μL and flow rate of 1.0 mL/min,the content of caffeic acid in *** Lour was determined.[Results]Caffeic acid can be detected in thin layer chromatography,with strong specificity and clear *** the sample size of caffeic acid is within the range of 0.0151-0.4530μg(R^(2)=0.9999),the content of caffeic acid showed a relatively good linear relationship with the peak *** average recovery rate of *** Lour was 99.94%(RSD=2.68%),97.56%(RSD=1.57%)and 99.79%(RSD=2.05%),respectively.[Conclusions]This method can effectively identify *** Lour,and can accurately determine the content of caffeic acid in *** *** has characteristics of high accuracy,high precision,good color rendering stability,good reproducibility and fast analysis speed.

Exposure to multiple natural hazard-related disasters will become more common due to climate *** article reports on the development and validation of a cumulative measure of exposure to natural hazard-related disasters(2013–2017)at the area level,and an individual-level measure of disaster impact using data from the Longitudinal Cohort Study on the Filipino child and linked data from the international Disaster Database(EM-DAT).Caregiver reports of cumulative exposure to disasters had statistically significant associations with disasters reported by neighborhood officials and with disasters in *** ecometric techniques we generated a reliable community average measure of exposure to natural hazard-related *** on neighbor but not individual self-reports this exogenous measure of disaster exposure in the local area was more strongly related to EM-DAT and official neighborhood reports than individual *** capture household variation we developed an individual-level measure of disaster *** impact was associated with measures of exposure(individual and community average),community ratings by officials,and EM-DAT but only moderately associated with the community average *** the community average and disaster impacts measures were associated with household income and the adequacy of income in households.

Efron (1997) considered several approximations of p-values for simultaneous hypothesis testing. An extension of his approaches is considered here to approximate various probabilities of correlated events. Compared with multiple-integrations, our proposed method, the parallelogram formulas, based on a one-dimensional integral, not only substantially reduces the computational complexity but also maintains good accuracy. Applications of the proposed method to genetic association studies and group sequential analysis are investigated in detail. Numerical results including real data analysis and simulation studies demonstrate that the proposed method performs well.

AIM: To investigate whether the complement system is involved in a murine model of oxygen-induced retinopathy(OIR).METHODS: Forty C57BL/6J newborn mice were divided randomly into OIR group and control group. OIR was induced by exposing mice to 75% ±2% oxygen from postnatal 7d(P7) to P12 and then recovered in room *** the control group, the litters were raised in room *** the postnatal 17d(P17), gene expressions of the complement components of the classical pathway(CP),the mannose-binding lectin(MBL) pathway and the alternative pathway(AP) in the retina were determined by quantitative real-time polymerase chain reaction(RT-PCR). Retinal protein expressions of the key components in the CP were examined by Western blotting.· RESULTS: Whole mounted retina in the OIR mice showed area of central hypoperfusion in both superficial and deep layers and neovascular tufts in the *** expressions of C1 qb and C4 b genes in the OIR retina were significantly higher than those of the controls. The expression of retinal complement factor B(CFB) gene in OIR mice was significantly lower than those of the controls. However, the expressions of C3 and complement factor H(CFH) genes were higher. The protein synthesis of the key components involved in the CP(C1q, C4 and C3) were also significantly higher in OIR mouse retina. Although MBL-associated serine protease 1(MASP1) and MASP2 were detected in both the OIR and the control groups, the expressions were weak and the difference between the two groups was not ***: Our data suggest that the complement system CP is activated during the pathogenesis of murine model of OIR.
