Background: Vitamin D has an important role in glucose metabolism and the associated hormones and metabolites in pregnant women, however its specific effect of vitamin D supplementation during pregnant time remains unclear. The present meta-analysis describes the effect of varying doses of vitamin D supplementation on glucose metabolism in pregnant ***: A literature search was conducted using PubMed, EMBASE, Web of Science, and Cochrane databases. The revised Jadad’s scale was used to evaluate the quality of each trial. Two reviewers independently performed a quality assessment of each study included. A random effects model was used to calculate the pooled ***: A total of 641 participants from 9 studies were pooled into our meta-analysis(intervention group: 320, control group: 321). Results from numerous populations(with and without GDM), suggest that vitamin D supplementation significantly reduced FPG, insulin, HOMA-IR, and HOMA-B in pregnant women with GDM [SMD=-0.87,-0.73,-0.68,-0.52, 95%CI:(-1.32,-0.42),(-1.01,-0.44),(-1.08,-0.28),(-0.92,-0.13), z= 3.76, 5.02, 3.34, 2.60, P= 0.00, 0.00, 0.00, 0.01, respectively]. Similarly, vitamin D supplementation significantly decreased insulin, HOMA-IR, and HOMA-B in pregnant women without GDM [SMD=-1.73,-2.52,-2.48, 95%CI:(-2.80,-0.65),(-2.52,-0.66),(-2.48,-0.67), z= 3.15, 3.34, 3.42, P = 0.00, 0.00, 0.00, respectively]. When vitamin D doses were varied,(low, moderate, high dosage group) the results fluctuated accordingly. There was no significant effect on glucose metabolism in the low dosage group. Moderate dosage of vitamin D supplementation significantly reduced FPG, insulin, HOMA-IR, and HOMA-B [SMD =-0.92,-0.97,-0.65,-0.48, 95%CI :(-1.44,-0.41),(-1.62,-0.32),(-1.13,-0.18),(-0.95,-0.02), z= 3.49, 2.91, 2.68, 2.02, P = 0.00, 0.00, 0.01, 0.04, respectively]. Chronic kidney disease severely deteriorates the outcome of gastrointestinal bleeding: A meta-analysis

AIM To understand the influence of chronic kidney disease(CKD) on mortality, need for transfusion and rebleeding in gastrointestinal(GI) bleeding *** A systematic search was conducted in three databases for studies on GI bleeding patients with CKD or endstage renal disease(ESRD) with data on outcomes of mortality, transfusion requirement, rebleeding rate and length of hospitalization(LOH). Calculations were performed with Comprehensive Meta-Analysis software using the random effects model. Heterogeneity was tested by using Cochrane's Q and I2 statistics. Mean difference(MD) and OR(odds ratio) were ***1063 articles(EMBASE: 589; PubM ed: 459; Cochrane: 15) were found in total. 5 retrospective articles and 1 prospective study were available for analysis. These 6 articles contained data on 406035 patients, of whom 51315 had impaired renal function. The analysis showed a higher mortality in the CKD group(OR = 1.786, 95%CI: 1.689-1.888, P group(OR = 2.530, 95%CI: 1.386-4.616, P = 0.002), and a rebleeding rate(OR = 2.510, 95%CI: 1.521-4.144, P clinical trials are crucially needed.

Topical delivery of nerve growth factor for treatment of ocular and brain disorders

Neurotrophins are a family of proteins that support neuronal proliferation, survival, and differentiation in the central and peripheral nervous systems, and are regulators of neuronal plasticity. Nerve growth factor is one of the best-described neurotrophins and has advanced to clinical trials for treatment of ocular and brain diseases due to its trophic and regenerative properties. Prior trials over the past few decades have produced conflicting results, which have principally been ascribed to adverse effects of systemic nerve growth factor administration, together with poor penetrance of the blood-brain barrier that impairs drug delivery. Contrastingly, recent studies have revealed that topical ocular and intranasal nerve growth factor administration are safe and effective, suggesting that topical nerve growth factor delivery is a potential alternative to both systemic and invasive intracerebral delivery. The therapeutic effects of local nerve growth factor delivery have been extensively investigated for different ophthalmic diseases, including neurotrophic keratitis, glaucoma, retinitis pigmentosa, and dry eye disease. Further, promising pharmacologic effects were reported in an optic glioma model, which indicated that topically administered nerve growth factor diffused far beyond where it was topically applied. These findings support the therapeutic potential of delivering topical nerve growth factor preparations intranasally for acquired and degenerative brain disorders. Preliminary clinical findings in both traumatic and non-traumatic acquired brain injuries are encouraging, especially in pediatric patients, and clinical trials are ongoing. The present review will focus on the therapeutic effects of both ocular and intranasal nerve growth factor delivery for diseases of the brain and eye.

AIM: To investigate recruitment, retention, and estimates for effects of formula supplementation with Lactobacillus rhamnosus GG(LGG) on inflammatory biomarkers and fecal microbial community in infants with colic. METHODS: A prospective, double-blind, placebocontrolled trial was conducted in otherwise healthy infants with colic. We screened 74 infants and randomized and analyzed results in 20 infants [9 receiving LGG(LGG+) and 11 not receiving LGG(LGG-)]. LGG was incorporated in the formula(Nutramigen~®)(minimum of 3 × 107 CFU/d) in the LGG+ group. Fecal microbiota and inflammatory biomarkers, including fecal calprotectin(FC), plasma cytokines, circulating regulatory T cells(Tregs), and crying + fussing time were analyzed to determine optimal time points and effect sizes for a larger trial. RESULTS: Recruitment in this population was slow, with about 66% of eligible infants willing to enroll; subject retention was better(75%). These rates were influenced by parents' reluctance to volunteer their infant for a clinical trial and by their tendency to change formulas. The maximal difference of crying + fussing time was observed at day 14, comparing the 2 groups, with a mean difference of-91(95%CI:-76, 259) min(P = NS). FC showed no significant difference, but the optimal time to determine a potential effect was at day 90 [with a mean difference of 121(95%CI:-48, 291) μg/g stool], observing a lower level of FC in the LGG+ group. The fecal microbial communities were chaotic, as determined by Shannon's diversity index and not apparently influenced by the probiotic. No significant change was observed in plasma inflammatory cytokines or Tregs, comparing LGG+ to LGG- groups. CONCLUSION: Designing future colic trials involving a probiotic-supplemented formula for infants in the United States will require consideration for difficult enrollment. Abnormal aggregation and accumulation of pathological amyloid proteins such as amyloid-β,Tau,and𝛼α-synuclein play key pathological roles and serve as histological hallmarks in different neurodegenerative diseases(NDs)such as Alzheimer’s disease(AD)and Parkinson’s disease(PD).In addition,various post-translational modifications(PTMs)have been identified on pathological amyloid proteins and are subjected to change during disease *** the central role of amyloid proteins in NDs,tremendous efforts have been made to develop amyloid-targeting strategies for clinical diagnosis and molecular classification of *** this review,we summarize two major strategies for targeting amyloid aggregates,with a focus on the trials in AD *** first strategy is a positron emission tomography(PET)scan of protein aggregation in the *** mainly focus on introducing the development of small-molecule PET tracers for specifically recognizing pathological amyloid *** second strategy is the detection of PTM biomarkers on amyloid proteins in cerebrospinal fluid and *** discuss the pathological roles of different PTMs in diseases and how we can use the PTM profile of amyloid proteins for clinical ***,we point out the potential technical challenges of these two strategies,and outline other potential strategies,as well as a combination of multiple strategies,for molecular diagnosis of NDs.
