Chimeric antigen receptor-modified T cells for the immunotherapy of patients with EGFR-expressing advanced relapsed/refractory non-small cell lung cancer

The successes achieved by chimeric antigen receptor-modified T （CAR-T） cells in hematological malignancies raised the pos- sibility of their use in non-small lung cancer （NSCLC）. In this phase I clinical study （NCT01869166）, patients with epidermal growth factor receptor （EGFR）-positive （〉50% expression）, relapsed/refractory NSCLC received escalating doses of EGFR-targeted CAR-T cell infusions. The EGFR-targeted CAR-T cells were generated from peripheral blood after a 10 to 13-day in vitro expansion. Serum cytokines in peripheral blood and copy numbers of CAR-EGFR transgene in peripheral blood and in tissue biopsy were monitored periodically. Clinical responses were evaluated with RECISTI.1 and im- mune-related response criteria, and adverse events were graded with CTCAE 4.0. The EGFR-targeted CAR-T cell infusions were well-tolerated without severe toxicity. Of 11 evaluable patients, two patients obtained partial response and five had stable disease for two to eight months. The median dose of transfused CAR＋ T cells was 0.97x 10^7 cells kg J （interquar- tile range （IQR）, 0.45 to 1.09x 10^7 cells kg 1）. Pathological eradication of EGFR positive tumor cells after EGFR-targeted CAR-T cell treatment can be observed in tumor biopsies, along with the CAR-EGFR gene detected in tumor-infiltrating T cells in all four biopsied patients. The EGFR-targeted CAR-T cell therapy is safe and feasible for EGFR-positive advanced re- lapsed/refractory NSCLC.

To acquire the phage-displayed mimotopes which mimic the specificity of hepatitis B virus surface antigen (HBsAg), a random peptide library expressing linear peptide with 12 amino asids in length were used to screen with the serum from a hepatitis B virus infected patient in the recovery phase. After 3 rounds of biopanning, the positive phages were confirmed by competitive ELISA using HBsAg/P33. Two phagotopes were identified and one of them was confirmed as mimotope by competition experiment. Based on the mimotpe, a multiple antigenic peptide with four branches was synthesized by solid phase peptide synthesis. The antiginicity and specificity of the synthesized antigen was tested in BALB/c mice compared with the native epitope-based antigen. The results showed that the mimotope-based antigen could evoke higher titer of antibodies with the same specificity of the epitope-based antigen. Those findings indicate mimotopes can be used in antigen and vaccine design.

In this paper, superhigh reproductive rate strains of MEV with titre more than HA8192* or TCID50 log9.7 10 have been achieved both by cultivation in cell lines with different susceptibility to MEV and by isolating and identifying in field by the author. The systematic tests proved that S18 and L12 strains of MEV are the best strains for vaccine preparation. In this study, the best means for the tissue cultivation of MEV and the most advanced technological process for the production and detection of serum-free cell-cultured MEV fluids with super-high HA titre in batches in large quantities have been established for the first time. Optimum conditions for MEV inactivation were determined, and safe and effective inactivated vaccines with mineral oil or A1(OH)3 gel adjuvant were successfully prepared with serum-free cell-cultured MEV fluids. Both vaccines with different adjuvants can be manufactured in batches in large quantities and have been widely used all over China since 1986. The change laws of the immune response kinetics of the MEV vaccines have been determined by the tests, and the indexes and laboratory animal models for monitoring humoral immune efficiency and immune protective rate of parvovirus vaccines have been established for the first time. The manufacturing and monitoring operation rules of the MEV vaccines with different adjuvants have been laid down by the author.

In multiple attribute group decision making （MAGDM） problems based on linguistic information, the granularities of linguistic label sets are usually different due to the differences of thinking modes and habits among decision makers. In order to deal with this inconvenience, the transformation relationships among multigranular linguistic labels （TRMLLs）, which are applied to unify linguistic labels with different granularities into a certain linguistic label set with fixed granularity, are presented in this paper. Furthermore, the reference tables are made according to TRMLLs so that the interrelated calculation will be less complicated, and the method of how to use them is explained in detail. At length, the TRMLLs are illustrated through an application example.

The cobalt-based alloy coating with different Co contents was deposited on 45 steel by electro-spark deposition with the self-made electrode. The coating has a compact and uniform microstructure with low porosity and no visible microcracks. When Co content increases grad- ually, oxygen content of coating samples 1-5 decreases first and then increases in the range of 2.52 wt%-3.05 wt%; sample 3 has the lowest oxygen content of 2.52 %. Mi- crohardness of the coating is improved remarkably com- pared with the substrate （HV 230.18）. With Co content increasing, microhardness of the coating samples 1-5 first rises slightly and then declines rapidly in the range of HV 580.61-1052.33. Sample 3 gets the maximum of HV 1052.33, which is about 4.6 times that of the substrate. The coating presents excellent wear resistance, which first increases and then decreases when Co content increases. Sample 3 shows the best wear resistance of about 6.4 times that of the substrate. Main wear mechanism of the coating is abrasive wear and fatigue wear, along with oxidation wear under high speed or heavy load conditions.

The host immune system plays an instrumental role in the surveillance and elimination of tumors by recognizing and destroying cancer cells. In recent decades, studies have mainly focused on adoptive immunotherapy using engineered T cells for the treatment of malignant diseases. Through gene engraftment of the patient's own T cells with chimeric antigen receptor(CAR),they can recognize tumor specific antigens effectively and eradicate selectively targeted cells in an MHC-independent *** date, CAR-T cell therapy has shown great clinical utility in patients with B-cell leukemias. Owing to different CAR designs and tumor complex microenvironments, genetically redirected T cells may generate diverse biological properties and thereby impact their long-term clinical performance and outcome. Meanwhile some unexpected toxicities that result from CAR-T cell application have been examined and limited the curative effects. Diverse important parameters are closely related with adoptively transferred cell behaviors, including CAR-T cells homing, CAR constitutive signaling, T cell differentiation and exhaustion. Thus, understanding CARs molecular design to improve infused cell efficacy and safety is crucial to clinicians and patients who are considering this novel cancer therapeutics. In this review, the developments in CAR-T cell therapy and the limitations and perspectives in optimizing this technology towards clinical application are discussed.

An expert consensus on the evaluation and treatment of acute thoracolumbar spine and spinal cord injury in China

This is an expert consensus on the evaluation and treatment of thoracolumbar spinal injury, estab- lished from February 2009 to July 2010. The expert consensus consists mainly of six parts with a total of 54 recommendations including the overview （one item）; pre-hospital care （one item）; evaluation and diagnosis （13 items）; treatment （23 items）; prevention and treatment of major com- plications （12 items）; and rehabilitation （four items）. This is the first time that Chinese experts have published a consensus on spine and spinal cord injury. The expert consensus was established based on Delphi methods, literature analysis, and clinical experiences. Each recommendation is supported by and was interpreted using multi-level evidences. The level of agreement with the rec- ommendation among the panel members was assessed as either low, moderate, or strong. Each panel member was asked to indicate his or her level of agreement on a 5-point scale, with ＂1＂ cor- respondJng to neutrality and ＂5＂ representJng maxJmum agreement. Scores were aggregated across the panel members and an arithmetic mean was calculated. This mean score was then translated into low, moderate, or strong. After all of the votes were collected and calculated, the results showed no low-level recommendations, 10 moderate-level recommendations, and 44 strong-level recom- mendations. An expert consensus was reached and was recognized by Chinese spine surgeons. Wide-scale adoption of these recommendations is urgent in the management of acute thora- columbar spine and spinal cord injury in a broader attempt to create a standard evaluation and treatment strategy for acute thoracolumbar spine and spinal cord injury in China.

Effects of Sea Spray Evaporation and Dissipative Heating on Intensity and Structure of Tropical Cyclone

To examine effects of sea spray evaporation and dissipative heating on structure and intensity of a real tropical cyclone, the sea spray flux parameterization scheme was incorporated into the fifth-generation Penn- sylvania State University National Center for Atmospheric Research Mesoscale Model （MM5）. Sensitivity tests were performed with varying the spray source function intensities and with and without dissipation heating. The numerical results indicate that sea spray evaporation increases the interfaeial sensible heat flux, which is increased by 16% for the moderate spray and 47% for the heavy spray, but has little effect on the interfaeial latent heat flux. The net effect of sea spray evaporation is to decrease the total sensible heat flux and to increase the total latent heat flux. The total enthalpy flux is increased by 1% and 12% with moderate and strong spray amounts, respectively. Consistent with these results, the intensity of the tropical cyclone is increased by 5% and 16% in maximum 10-m wind speed, respectively, due to sea spray evapora- tion. Sea spray evaporation and dissipative heating modify the tropical cyclone structure in important but complex ways. The effect of sea spray on the near-surface temperature and moisture depends on the spray amounts and its location within the tropical cyclone. Within the high-wind region of a tropical cyclone, the lower atmosphere becomes cooler and moister due to the evaporation of sea spray. However, the dissipative heating offsets the cooling due to sea spray evaporation, which makes the lower atmosphere warmer.

With development of web services technology, the number of existing services in the internet is growing day by day. In order to achieve automatic and accurate services classification which can be beneficial for service related tasks, a rough set theory based method for services classification was proposed. First, the services descriptions were preprocessed and represented as vectors. Elicited by the discernibility matrices based attribute reduction in rough set theory and taking into account the characteristic of decision table of services classification, a method based on continuous discernibility matrices was proposed for dimensionality reduction. And finally, services classification was processed automatically. Through the experiment, the proposed method for services classification achieves approving classification result in all five testing categories. The experiment result shows that the proposed method is accurate and could be used in practical web services classification.
