Comparison of resistive index and shear-wave elastography in the evaluation of chronic kidney allograft dysfunction

BACKGROUND Detection of early chronic changes in the kidney allograft is important for timely intervention and long-term *** and novel ultrasound-based investigations are being increasingly used for this purpose with variable *** To compare the diagnostic performance of resistive index(RI)and shear wave elastography(SWE)in the diagnosis of chronic fibrosing changes of kidney allograft with histopathological *** This is a cross-sectional and comparative study.A total of 154 kidney transplant recipients were included in this study,which was conducted at the Departments of Transplantation and Radiology,Sindh institute of Urology and Transplantation,Karachi,Pakistan,from August 2022 to February *** consecutive patients with increased serum creatinine levels and reduced glomerular filtration rate(GFR)after three months of transplantation were enrolled in this *** and RI were performed and the findings of these were evaluated against the kidney allograft biopsy results to determine their diagnostic *** The mean age of all patients was 35.32±11.08 *** these,126(81.8%)were males and 28(18.2%)were *** mean serum creatinine in all patients was 2.86±1.68 mg/dL and the mean estimated GFR was 35.38±17.27 mL/min/1.73 *** allograft biopsy results showed chronic changes in 55(37.66%)*** sensitivity,specificity,positive predictive value(PPV),and negative predictive value(NPV)of SWE for the detection of chronic allograft damage were 93.10%,96.87%%,94.73%,and 95.87%,respectively,and the diagnostic accuracy was 95.45%.For RI,the sensitivity,specificity,PPV,and NPV were 76.92%,83.33%,70.17%,and 87.62%,respectively,and the diagnostic accuracy was 81.16%.CONCLUSION The results from this study show that SWE is more sensitive and specific as compared to RI in the evaluation of chronic allograft *** can be of great help during the routine follow-up of kidney transplant recipients for screening a

Bunch-length measurement at a bunch-by-bunch rate based on time–frequency-domain joint analysis techniques and its application

Assessments and diagnoses of aquatic ecosystem integrity based on integrity requirements of ecosystem service targets

This paper critically reviews the progress in ecosystem integrity(health)assessment of inland waters and provides future directions for *** biotic integrity assessments mainly use multimetric indices and predictive models to evaluate overall health status;the criteria largely rely on pristine reference sites,but such an approach is not applicable to unique natural waters and irreversibly modified *** diagnostic assessments are still in the exploratory stage and can only diagnose possible stressor types and wide-ranges of their intensities through empirical models linking stressors and *** chemical integrity assessments mainly use criteria determined by small-scale ecotoxicity tests,rather than quantitative relationships developed between chemical factors and biotic effects in real ecosystems,thus potentially under or overestimating pollutant *** physical integrity assessments focus on overall habitat quality,rather than quantitative habitat requirements,and thus cannot provide quantitative support for ecological restoration and *** hydrological integrity(environment flow)assessments largely depend on quantitative relationships between hydrological regimes with a few species and single groups,rather than with whole communities,and fail to comprehensively diagnose hydrological causes of biotic resource *** the future,integrity assessments need to be based on ecosystem integrity requirements of ecosystem service targets:first,there is a need to build quantitative relationships between biotic integrity and ecosystem services,assess requirements of community structure and function for service goals,and establish biotic integrity assessment methods;second,we must build quantitative relationships between biotic integrity and abiotic integrity in real ecosystems,assess chemical,physical,and hydrological integrity requirements of biotic needs,and establish diagnostic assessment methods.

An edge Lyman-alpha-based beam emission spectroscopy(LyBES)diagnostic,using a heating NBI(neutral beam injection)system,is currently under development on the HL-2A *** 20-channel edge LyBES,which is intended to measure the density fluctuation in plasma edge(from R=1960 mm to R=2026 mm)with an improved spatial resolution of 3.3 mm,is a complement to the existing conventional beam emission spectroscopy(BES)*** this article,we introduce the progress of LyBES diagnostic,including the collection optics,the monochromator,and the detector *** reflectance of the collection mirrors is measured to be~82%at 122 nm,and the aberration geometrical radius of the collection optics is tested to be~150μm in the aimed *** linear dispersion of the LyBES monochromator is designed to be~0.09 nm mm^(-1).The bandwidth of the detector system with the 5×10^(7)V A^(-1)preamplifier gain is measured to be~280 kHz,and the peak-to-peak noise of the detector system is tested to be~16 *** finalized design,components development and testing of the LyBES diagnostic have been completed at present,and an overall performance of the LyBES diagnostic is to be confirmed in the next HL-2A campaign.
