Background:Despite immunotherapy advancements for patients with advanced or metastatic non-small-cell lung cancer(NSCLC),pivotal first-line trials were limited to patients with an Eastern Cooperative Oncology Group performance status(ECOG PS)0-1 and a median age of 65 years or *** aimed to compare the efficacy and safety of first-line atezolizumab monotherapy with single-agent chemotherapy in patients ineligible for platinum-based ***:This trial was a phase 3,open-label,randomised controlled study conducted at 91 sites in 23 countries across Asia,Europe,North America,and South *** patients had stage IIIB or IV NSCLC in whom platinum-doublet chemotherapy was deemed unsuitable by the investigator due to an ECOG PS 2 or 3,or alternatively,being 70 years or older with an ECOG PS 0-1 with substantial comorbidities or contraindications for platinum-doublet *** were randomised 2:1 by permuted-block randomisation(block size of six)to receive 1200 mg of atezolizumab given intravenously every 3 weeks or single-agent chemotherapy(vinorelbine[oral or intravenous]or gemcitabine[intravenous];dosing per local label)at 3-weekly or 4-weekly *** primary endpoint was overall survival assessed in the intention-to-treat *** analyses were conducted in the safety-evaluable population,which included all randomised patients who received any amount of atezolizumab or *** trial is registered with ***,NCT03191786.

目的研究磁场的不均匀性对乳腺脂肪抑制动态增强扫描(DCE-FS)的影响。材料与方法对10例病人的乳腺进行检查。使用1.5TMR设备,行DCE-FS,采用快速扰相梯度回波及频谱选择性脂肪抑制序列。在共振和去共振时,计算T1值为25～1200ms的信号强度,结果经测试实验证实。评价场不均匀性对临床检查的预期影响。结果位于中心横断面的乳腺磁场变化为(3.6±1.2)ppm,乳腺整体的磁场变化为(5.1±1.5)ppm。当由于磁场不均匀引起意外的水抑制时,或由于脂肪抑制失败导致对比剂(CA)摄取曲线变形时(因为像素内包含脂肪),计算机模块即会预测到动态变化范围的缩小。需要同时兼顾动态变化范围和脂肪饱和效果两方面的需求。全部临床检查中均存在脂肪抑制失败并产生了意外的水抑制。结论磁场不均匀可能通过两种机制降低DCE-FS的敏感性。其一,磁场不均匀可使脂肪抑制失败(由于像素包含脂肪)导致CA摄取曲线变形。其二,引起意外的水抑制,导致动态变化范围缩小。要点①磁场不均匀对乳腺MR检查有显著影响。②磁场不均匀会导致对比剂摄取曲线变形。③放射学专家必须清楚变形的可能,正确解释对比剂摄取曲线。④需同时兼顾对脂肪抑制效果和动态变化范围两方面的需求。

Managing panniculitis in alpha-1 antitrypsin deficiency: Systematic review of evidence behind treatment

AIM To systematically review literature for management of alpha-1 antitrypsin deficiency(AATD) panniculitis. METHODS Multiple databases were searched using combinations of pertinent terms. Articles were selected describing panniculitis treatment in patients with AAT < 11 μmol and/or PiZZ genotype, with no language limitation. All relevant articles were accessed in full text. Independent review of abstracts and full manuscripts was conducted by 2 reviewers, and quality assessment by one reviewer(checked by a second). Data extraction was conducted byone reviewer(checked by a second). Narrative synthesis only was conducted, as data were unsuitable for *** Thirty-two case reports and 4 case series were found. Augmentation therapy(infusions of plasma-derived AAT) was the most successful, with complete resolution of symptoms in all patients. Dapsone is a less expensive option, and it achieved clinical resolution in 62% of patients, but it is very poorly tolerated. Among other single-agent antibiotics, doxycycline was the most successful with complete clinical resolution seen in 33% of patients. Immunosuppressants were largely unsuccessful; 80% of patients exhibited no response. Liver transplantation and therapeutic plasma exchange displayed complete resolution in 66% of patients. Other strategies, such as non-steroidal anti-inflammatory drugs or antibiotics other than dapsone did not show sufficient response rates to recommend their use. Authors note the risk of bias imposed by the type of evidence(case reports, case series) available in this *** Dapsone is the recommended first line therapy for AATD panniculitis, followed by augmentation therapy. Plasma exchange may be an alternative in the setting of rapidly progressive disease.

摘要很多治疗癌症的方法都直接或间接地影响肿瘤血管。动态对比增强MRI(DCE-MRI)已经成为临床前和临床实验早期阶段研究肿瘤血管的重要方法。对于这些临床实验,需保证测量过程(即图像采集和分析)的有效进行以及在各个研究中心之间的一致性。随着技术进步,不仅能够潜在改善敏感性和生理相关性,而且在改善研究中心之间的技术差异方面也具有很大空间。

糖尿病相关的足病管理涉及多个学科,通用的专业词汇对于各学科间准确沟通交流至关重要。国际糖尿病足工作组(IWGDF)对相关文献进行了系统综述,构成了制定IWGDF指南的基础,并且明确了糖尿病相关的足病的一系列定义、标准及名词解释。本文介绍2023年IWGDF对这些定义和标准的更新,并建议在临床实践和研究中一致使用,以利于世界各地不同学科人员针对糖尿病相关的足病的专业性交流。
