Effects of self-administered exercises based on Tuina techniques on musculoskeletal disorders of professional orchestra musicians: a randomized controlled trial

BACKGROUND： Musicians are frequently affected by playing-related musculoskeletal disorders （PRMD）. Common solutions used by Western medicine to treat musculoskeletal pain include rehabilitation programs and drugs, but their results are sometimes disappointing. OBJECTIVE： To study the effects of self-administered exercises based on Tuina techniques on the pain intensity caused by PRMD of professional orchestra musicians, using numeric visual scale （NVS）. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： We performed a prospective, controlled single-blinded, randomized study with musicians suffering from PRMD. Participating musicians were randomly distributed into the experimental （n = 39） and the control （n = 30） groups. After an individual diagnostic assessment, specific Tuina self-administered exercises were developed and taught to the participants. Musicians were instructed to repeat the exercises every day for 3 weeks. MAIN OUTCOME MEASURES： Pain intensity was measured by NVS before the intervention and after 1, 3, 5, 10, 15 and 20 d of treatment. The procedure was the same for the control group, however the Tuina exercises were executed in points away from the commonly-used acupuncture points. RESULTS： In the treatment group, but not the control group, pain intensity was significantly reduced on days 1, 3, 5, 10, 15 and 20. CONCLUSION： The results obtained are consistent with the hypothesis that self-administered exercises based on Tuina techniques could help professional musicians controlling the pain caused by PRMD. Although our results are very promising, further studies are needed employing a larger sample size and double blinding designs.

BACKGROUND： Rheumatoid arthritis（RA） is a systemic inflammatory disease characterized by functional disability and pain. Although acupuncture is widely used, until now Western acupuncture studies on RA have not shown conclusive positive results. Acupuncture is regarded as a reflex therapy that has effects on the human autonomic nervous system. By establishing a traditional Chinese medicine（TCM） diagnosis first, the practitioner is able to choose acupoints according to the state of each individual patient.METHODS/DESIGN： We are interested if acupuncture, using a classical diagnostic procedure to allocate acupoints to the patient according to the Shang Han Lun theory, can be effective in relieving pain, improving hand function and increasing health-related quality of life in RA.The authors intend to harmonize TCM diagnosis according to clinical and genetic profiles. Patients with the TCM diagnosis of a so-called Turning Point syndrome will be followed up in a randomized, prospective, double-blind, placebo-controlled, multicenter and three-armed parallel-group study with a standardized treatment in order to optimize potential therapeutic effects of acupuncture on pain, strength and muscle function of patients with RA as well as the influence on inflammation and quality of life.DISCUSSION： The findings of this study will provide important clinical information about the feasibility and efficacy of acupuncture treatment for RA patients. Association between EGF +61A/G polymorphism and gastric cancer in Caucasians

AIM: To investigate the association between epidermal growth factor (EGF) +61A/G polymorphism and susceptibility to gastric cancer, through a cross-sectional study. METHODS: Polymerase chain reaction resctriction fragment lenght polymorphism analyses were used to genotype EGF +61 in 207 patients with gastric lesions (162 patients with gastric adenocarcinomas, 45 with atrophy or intestinal metaplasia) and 984 controls. All subjects were Caucasian. RESULTS: Genotype distribution was 23.5% for GG and 76.5% for GA/AA in the control group, 18.4% for GG and 68.6% for GA/AA in the entire group with gastric lesions and 17.9% for GG and 82.1% for GA/AA in the group with gastric adenocarcinoma. No statistically significant associations were found between EGF +61 variants and risk for developing gastric cancer [odds ratios (OR) = 1.41, 95% confidence intervals (CI): 0.90-2.21, P = 0.116]. However, the stratification of individuals by gender revealed that males carrying A alleles (EGF +61A/G or AA) had an increased risk for developing gastric cancer as compared to GG homozygous males (OR = 1.55, 95% CI: 1.05-2.28, P = 0.021). CONCLUSION: In summary, we found that males who were A carriers for EGF +61 had an increased risk for developing gastric cancer. This result may be explained by the suggestion that women secrete less gastric acid than men.

Assessment of Short-term Acupuncture Effect Through Electro-conductivity Variation of Yuan-Source Acupoints

Objective: To demonstrate that acupuncture has an instant effect in equilibrating the imbalanced electrical conductivity of Yuan-Source acupoints after intervention on the Luo-Connecting acupoint(one-side) of the same channel.Methods: The experiment was designed as a randomized blinded control trial, with 56 volunteers. Reactive Electrical Permeable Points(REPP-mostly Yuan points) were measured in terms of electrical conductivity. Patients with a split-meridian(difference between left and right measurement) were randomly included in the control or experimental group. After five minutes of needle insertion in a Luo acupoint, the needle was removed and REPP were re-measured twice within a five minutes interval.Results: From the 56 volunteers, 32 had a valid Split-Meridian Difference(SMD > 20 μA) and were included in the study. The results indicate that after the third measurement(15 minutes-M3), 100% of the experimental group had SMD 20 μA, at M3, when compared with the initial measurement(M1). Results obtained between the experimental and control group are significantly different(p< 0.001).Conclusions: The present exploratory work indicates that there is evidence of instant body response to acupuncture that can be detected by changes in the electro-conductivity of specific acupoints.

Molecular-assisted precision oncology gained tremendous ground with high-throughput next-generation sequencing(NGS),supported by robust bioinformatics.The quest for genomicsbased cancer medicine set the foundations for improved patient stratification,while unveiling a wide array of neoantigens for immunotherapy.Upfront pre-clinical and clinical studies have successfully used tumor-specific peptides in vaccines with minimal off-target effects.However,the low mutational burden presented by many lesions challenges the generalization of these solutions,requiring the diversification of neoantigen sources.Oncoproteogenomics utilizing customized databases for protein annotation by mass spectrometry(MS)is a powerful tool toward this end.Expanding the concept toward exploring proteoforms originated from post-translational modifications(PTMs)will be decisive to improve molecular subtyping and provide potentially targetable functional nodes with increased cancer specificity.Walking through the path of systems biology,we highlight that alterations in protein glycosylation at the cell surface not only have functional impact on cancer progression and dissemination but also originate unique molecular fingerprints for targeted therapeutics.Moreover,we discuss the outstanding challenges required to accommodate glycoproteomics in oncoproteogenomics platforms.We envisage that such rationale may flag a rather neglected research field,generating novel paradigms for precision oncology and immunotherapy.

Immediate effects of Tuina techniques on working-related musculoskeletal disorder of professional orchestra musicians

BACKGROUND：Musicians are a prone group to suffer from working-related musculoskeletal disorder（WRMD）.Conventional solutions to control musculoskeletal pain include pharmacological treatment and rehabilitation programs but their effi ciency is sometimes disappointing.OBJECTIVE：The aim of this research is to study the immediate effects of Tuina techniques on WRMD of professional orchestra musicians from the north of Portugal.DESIGN,SETTING,PARTICIPANTS AND INTERVENTIONS：We performed a prospective,controlled,single-blinded,randomized study.Professional orchestra musicians with a diagnosis of WRMD were randomly distributed into the experimental group（n=39）and the control group（n=30）.During an individual interview,Chinese diagnosis took place and treatment points were chosen.Real acupoints were treated by Tuina techniques into the experimental group and non-specifi c skin points were treated into the control group.Pain was measured by verbal numerical scale before and immediately after intervention.RESULTS：After one treatment session,pain was reduced in 91.8%of the cases for the experimental group and 7.9%for the control group.CONCLUSION：Although results showed that Tuina techniques are effectively reducing WRMD in professional orchestra musicians of the north of Portugal,further investigations with stronger measurements,double-blinding designs and bigger simple sizes are needed.

Evaluation of Analgesic Properties of Piper Nigrum Essential Oil: a Randomized, Double-blind,Placebo-controlled Study

Objective: Essential oils are complex mixtures of chemical compounds, extracted from a wide range of plants. The volatile fraction of essential oils is responsible for their characteristic aroma and presents diverse biological properties that have been studied over the years. In Traditional Chinese Medicine, Piper nigrum is considered to be pungent and hot. Although its chemical constituents and respective pharmacological properties have been described by several authors, the volatile fraction is still underestimated as a therapeutic agent. The aim of this study was to evaluate the analgesic properties of the volatile fraction of Piper nigrum essential oil, in patients presenting different types of pain.Methods: Fifty-four patients presenting pain, were included in a randomized, double-blind, placebo-controlled study, over a 9-week period.The patients were randomly divided into two groups, and asked to inhale a vial containing Piper nigrum essential oil, or a vial containing a placebo(sesame oil), for 15 minutes. A numerical pain scale was applied before and after the inhalation.Results: Results showed a statistically significant decrease in pain intensity in the patients that inhaled the black pepper essential oil, while the placebo group patients showed no significant change in pain intensity.Conclusion: Although the results are preliminary due to the limited sample size and short inhalation time, the volatile fraction of the Piper nigrum essential showed promising results in reducing pain. In the Chinese medicine perspective, these results support the use of black pepper in different types of pain, since it warms the center and disperses cold.

Potential of four marine-derived fungi extracts as anti-proliferative and cell death-inducing agents in seven human cancer cell lines

Objective:To evaluate the in vitro anticancer activity of crude ethyl acetate extracts of the culture of four marine-derived fungi Aspergillus similanensis KUFA 0013(E1),Neosartorya paulistemis KUFC 7897(E2),Neosartorya siamensis KUFA 0017(E4) and Talaromyces trachyspermus KUFC 0021(E3) on a panel of seven human cancer cell lines.Methods:Effects on cell proliferation,induction of DNA damage and cell death were assessed by MTT and clonogenic assays,comet assay and nuclear condensation assay,respectively.Results:The proliferation of HepG2,HCTl 16 and A375 cells decreased after incubation with the extracts E2 and E4.The anti-proliterative effect was confirmed by morphologic alterations and by clonogenic assay.Both extracts also induced cell death in HepG2 and HCT116 cells.Doxorubicin was used as a positive control and showed in vitro anticancer activity.Conclusions:This study demonstrated,for the first time,that extracts of Neosartorya paulistensis and Neosartorya siamensis have selective anti-proliferative and cell death activities in HepG2,HCT16 and A375 cells.The bioactivity of these extracts suggests a potential for biotechnological applications and substantiates that both should be further considered for the elucidation of the molecular targets and signal transduction pathways involved.
