Individualized red-cell transfusion strategy for non-cardiac surgery in adults:a randomized controlled trial

Introduction: Experimental Clinical studies have shown that loss of up to 77.0% of red cell mass can be tolerated as long as it kept the blood volume. However, the blood volume loss of about 34.0% is fatal. The initial attention in patients with bleeding should be given to maintaining blood volume and oxygen transport. Thus, transfusions of allogeneic blood products are associated with an increased risk of morbidity and mortality in cardiac surgery. Some transfusion risk scores have been proposed for cardiac surgery patients. Objective: To review and discuss literature blood transfusion (blood components gradient) in cardiovascular surgery versus polytrauma, gastrointestinal bleeding, and neoplastic diseases. Methods: The model followed for the review was PRISMA. We used the databases as Scielo, Lilacs, Google Scholar, PubMed articles and works of scientific and doctoral theses master. Results: According to the discussion in the literature, the acute bleeding of large volumes is challenging to emergency services. Accordingly, the need to support hemotherapeutics led to the development of protocols for more rational utilization of the monitored blood components. Conclusion: Hemotherapy services have adopted a very dynamic approach to the clinical condition presented by patients, the need for four or more red blood cell units.

The feasibility, effectiveness and safety of totally thoracoscopic resection for right atrial myxoma

Background Myxoma is the most frequent cardiac benign tumor which requires surgical removal even though in right atrium. Minimally invasive surgery has become the standard approach for removal of myxoma in our center. Herein, we report our initial experience of cardiac myxoma removal through mini-port totally thoracoscopic particular with focus on its feasibility, efficacy and short-term outcomes, as compared with median sternotomy. Methods From April 2015 to March 2019, 31 consecutive cases were ultimately identified and divided into two groups(Mini-port thoracoscopic group, n=14;versus conventional median sternotomic group, n=17). An echocardiography was routinely performed prior to discharge, at 1 st month, 6 th month, 12 th month and every year postoperatively. Retrospective analyses of perioperative data, postoperative outcomes and complications,and follow up data were performed. Results All the 31 patients successfully underwent right atrial myxoma removal surgery with no perioperative death, re-exploration for bleeding, residual mass or central nervous event in both groups. Intraoperative data from two group showed absence of significant difference except the intraoperative blood loss(150±50.01 m L in mini-port thoracoscopic group vs. 255.88±93.35 m L in conventional median sternomtomic group, P=0.01). There was no significant difference in perioperative blood transfusion and postoperative outcomes between the two groups. But the mini-port thoracoscopic group had a much less 24-hour postoperative drainage(136.71±148.20 m L vs. 341.17±211.29 m L, P=0.03). No significance difference could be identified between two groups about the incidence of postoperative morbidities and follow-up adverse events. Conclusion Totally mini-port video-assisted thoracoscopic approach for right atrial myxoma resection on peripheral cardiopulmonary bypass is feasible, effective with favorable safety in experienced cardiac center.[S Chin J Cardiol 2019;20(4):236-244]

目的评价国产重组人血小板生成素(rhTPO)治疗实体肿瘤患者化疗后血小板减少的临床疗效和安全性。方法采用随机交叉自身对照研究,154例实体瘤患者随机分为A、B两组,每组77例,接受方案和剂量相同的两周期化疗。A组:第1个周期(用药周期)注射rhTPO,第二个周期(对照周期)不注射rhTPO;B组:第1个周期(对照周期)不注射rhTPO,第2个周期(用药周期)注射rhTPO。两组患者均于用药周期化疗结束后6～24 h皮下注射rhTPO 1.0μg·kg-1·d-1,连续用药最长14 d。监测血尿便常规、肝肾功能、凝血功能、胸片、心电图及血清抗rhTPO抗体。结果 A、B两组用药周期与对照周期血小板减少程度和持续时间差异无显著性。154例实体瘤患者用药周期与对照周期比较:(1)化疗后血小板下降的最低值分别为(64.4±45.4)×109/L和(52.4±30.9)×109/L(P=0.000),血小板计数恢复后的最高值分别为(263.9±142.5)×109/L和(148.9±67.7)×109/L(P=0.000);(2)化疗后血小板<50×109/L的持续时间分别为(2.5±3.9)和(3.7±5.7)d(P=0.04);(3)用药周期化疗后血小板恢复至75×109/L、100×109/L以上所需天数为(10.3±8.7)和(14.0±8.9)d,而对照周期为(15.9±10.5)和(21.1±9.5)d(P=0.000);(4)血小板平均输注量两周期比较无差异;(5)化疗后血红蛋白、白细胞?

The authors report a case of bilateral pulmonary embolism (PE) with intermediate risk at the University Hospital center of Treichville (CHUT). This is a postpartum PE in a 37-year-old obese, multiparous woman with postpartum hemorrhage from uterine rupture after vaginal delivery initiated by injectable oxytocin. This postpartum haemorrhage was managed by massive transfusion and hysterectomy. The initiation of thromboprophylaxis was delayed in view of its coagulation record, the first 3 days. Later, the patient presented respiratory distress for which the completion of a pulmonary angioscanner made it possible to make the diagnosis of PE whose clinical evolution under heparinotherapy was favorable.

河北汉族人群NAT2基因多态性分析

目的　了解河北汉族人群中氮乙酰基转移酶 2 (NAT2 )基因分布。方法　采用聚合酶链式反应限制性内切酶片段长度多态性 (PCR RFLP)方法检测 2 37名河北籍健康汉族人的NAT2基因型。结果　在河北健康汉族人群中 ,NAT2各种等位基因频率分别为Wt =0 .4 874 ,M1=0 .0 5 0 6 ,M2 =0 .15 6 1,M3=0 .30 5 9,M4 =0。快速乙酰化基因型 16 9例 (71.31% ) ,其中纯合的个体 6 2例 (2 6 .16 % ) ,杂合个体 10 7例 (4 5 .15 % ) ,并以Wt/M 3为主 (6 9例 ) ,占样本的 2 9.11% ;慢速乙酰化基因型 6 8例 ,占 2 8.6 9% ,其中纯合的个体 2 4例 (10 .13% ) ,杂合个体 4 4例 (18.5 7% ) ,并以M2 /M3基因型为主 ,共 31例 ,占样本的 13.0 8%。该结果经Hardy Weinberg吻合度检验符合Hardy Weinberg定律 ,χ2=7.2 5 33,υ=8,P>0 .0 5。结论　在河北省汉族人群NAT2突变基因中 ,以M3为主 ,但主要以Wt/M3形式存在 ,M1突变最少 ,未发现M4。这可能是该地区慢速乙酰化基因型较少的原因之一。
