Performance of tacrolimus in hospitalized patients with steroid-refractory acute severe ulcerative colitis

BACKGROUND Acute severe ulcerative colitis unresponsive to systemic steroid treatment is a lifethreatening medical condition requiring hospitalization and often *** the increasing choice of medical therapy options for ulcerative colitis, the condition remains a great challenge in the field of inflammatory bowel diseases(IBD). The performance of the calcineurin inhibitor tacrolimus in this clinical setting is insufficiently *** To evaluate the short and long-term outcomes of tacrolimus therapy in adult inpatients with steroid-refractory acute severe ulcerative *** We conducted a retrospective monocentric study enrolling 22 patients at a tertiary care center for the treatment of IBD. All patients who were admitted to one of the wards of the Department of Gastroenterology and Hepatology of the Heidelberg University Hospital with acute severe ulcerative colitis between 2007 and 2018, and who received oral or intravenous tacrolimus for steroid-refractory disease were included. Baseline characteristics and data on the disease courses were retrieved from entirely computerized patient charts. The primary study endpoint was clinical response to tacrolimus therapy, resulting in discharge from the hospital. Secondary study endpoints were colectomy rate and time to colectomy, achievement of clinical remission under tacrolimus therapy, and the occurrence of side *** the majority of the 22 included patients(68.2%), tacrolimus therapy was initiated intravenously and subsequently converted to oral administration. The treatment duration was 128 ± 28.5 d(mean ± SEM), and the patients were followed up for 705 ± 110 d after treatment initiation. Objective: Positive peritoneal lavege cytology(CY1) gastric cancer is featured by dismal prognosis, with high risks of peritoneal metastasis. However, there is a lack of evidence on pathogenic mechanism and signature of CY1and there is a continuous debate on CY1 therapy. Therefore, exploring the mechanism of CY1 is crucial for treatment strategies and targets for CY1 gastric ***: In order to figure out specific driver genes and marker genes of CY1 gastric cancer, and ultimately offer clues for potential marker and risk assessment of CY1, 17 cytology-positive gastric cancer patients and 31matched cytology-negative gastric cancer patients were enrolled in this study. The enrollment criteria were based on the results of diagnostic laparoscopy staging and cytology inspection of exfoliated cells. Whole exome sequencing was then performed on tumor samples to evaluate genomic characterization of cytology-positive gastric ***: Least absolute shrinkage and selection operator(LASSO) algorithm identified 43 cytology-positive marker genes, while Mut Sig CV identified 42 cytology-positive specific driver genes. CD3G and CDKL2 were both driver and marker genes of CY1. Regarding mutational signatures, driver gene mutation and tumor subclone architecture, no significant differences were observed between CY1 and negative peritoneal lavege cytology(CY0).Conclusions: There might not be distinct differences between CY1 and CY0, and CY1 might represent the progression of CY0 gastric cancer rather than constituting an independent subtype. This genomic analysis will thus provide key molecular insights into CY1, which may have a direct effect on treatment recommendations for CY1and CY0 patients, and provides opportunities for genome-guided clinical trials and drug development.

Line Patterns Segmentation in Blurred Images Using Contrast Enhancement and Local Entropy Thresholding

Finger vein extraction and recognition hold significance in various applications due to the unique and reliable nature of finger vein patterns. While recently finger vein recognition has gained popularity, there are still challenges associated with extracting and processing finger vein patterns related to image quality, positioning and alignment, skin conditions, security concerns and processing techniques applied. In this paper, a method for robust segmentation of line patterns in strongly blurred images is presented and evaluated in vessel network extraction from infrared images of human fingers. In a four-step process: local normalization of brightness, image enhancement, segmentation and cleaning were involved. A novel image enhancement method was used to re-establish the line patterns from the brightness sum of the independent close-form solutions of the adopted optimization criterion derived in small windows. In the proposed method, the computational resources were reduced significantly compared to the solution derived when the whole image was processed. In the enhanced image, where the concave structures have been sufficiently emphasized, accurate detection of line patterns was obtained by local entropy thresholding. Typical segmentation errors appearing in the binary image were removed using morphological dilation with a line structuring element and morphological filtering with a majority filter to eliminate isolated blobs. The proposed method performs accurate detection of the vessel network in human finger infrared images, as the experimental results show, applied both in real and artificial images and can readily be applied in many image enhancement and segmentation applications.

Dy^(3+) and Lu^(3+) co-doped CaLaGa_(3)O_(7) phosphors were prepared via high-temperature solid-phase *** electronic structures of LuxCaLa_(0.98-x)Ga_(3)O_(7) were investigated by the first-principles *** influence of Lu^(3+) on yellow light emission was studied using X-ray diffraction(XRD)and photoluminescence(PL) *** XRD results indicate that compared to Dy^(3+):CaLaGa_(3)O_(7),the cell parameters of Dy^(3+):LuCaLaGa_(3)O_(7) tend to decrease,which is due to the smaller radius of Lu^(3+).When pumped by blue GaN laser diode(LD),the emission peaks of phosphors with different Lu^(3+)doping concentrations in the visible region are similar,with the strongest peak at 574 nm in the yellow light region,which is sensitive to human *** optimal doping concentration of Lu^(3+) is confirmed to be1 at%,when all emission spectra and measured fluorescence lifetimes are taken into ***,the optimal phosphor composition Dy^(3+):Lu_(x)CaLa_(0.98-x)Ga_(3)O_(7)(x=0.01) has an internal quantum efficiency(IQE) of 46.94% and an external quantum efficiency(EQE) of 15.19%.Most notably,the prepared phosphor demonstrates excellent thermal stability and a high activation energy(0.203 eV).In addition,the International Illumination Commission color coordinates of the Dy^(3+):LuxCaLa_(0.98-x)Ga_(3)O_(7) phosphors are in the yellow light *** above analysis indicates that the Dy^(3+):LuxCaLa_(0.98-x)Ga_(3)O_(7)(x=0.001)phosphor has promising application prospects in yellow light-emitting devices.

The exfoliation of bulk 2H-molybdenum disulfide(2H-MoS_(2))into few-layer nanosheets with 1T-phase and controlled layers represents a daunting challenge towards the device applications of MoS_(2).Conventional ion intercalation assisted exfoliation needs the use of hazardous n-butyllithium and/or elaborate control of the intercalation potential to avoid the decomposition of the MoS_(2).This work reports a facile strategy by intercalating Li ions electrochemically with ether-based electrolyte into the van der Waals(vdW)channels of MoS_(2),which successfully avoids the decomposition of MoS_(2)at low *** co-intercalation of Li+and the ether solvent into MoS_(2)makes a first-order phase transformation,forming a superlattice phase,which preserves the layered structure and hence enables the exfoliation of bulk 2H-MoS_(2)into bilayer nanosheets with *** with the pristine 2H-MoS_(2),the bilayer 1T-MoS_(2)nanosheets exhibit better electrocatalytic performance for the hydrogen evolution reaction(HER).This facile method should be easily extended to the exfoliation of various transition metal dichalcogenides(TMDs).

Bietti crystalline corneoretinal dystrophy is an inherited retinal disease caused by mutations in CYP4V2,which results in blindness in the working-age population,and there is currently no available ***,we report the results of the first-in-human clinical trial(NCT04722107)of gene therapy for Bietti crystalline corneoretinal dystrophy,including 12 participants who were followed up for 180-365 *** open-label,single-arm exploratory trial aimed to assess the safety and efficacy of a recombinant adeno-associated-virus-serotype-2/8 vector encoding the human CYP4V2 protein(rAAV2/8-hCYP4V2).Participants received a single unilateral subretinal injection of 7.5×10^(10)vector genomes of rAAV2/***,73 treatment-emergent adverse events were reported,with the majority(98.6%)being of mild or moderate intensity and considered to be procedure-or corticosteroid-related;no treatment-related serious adverse events or local/systemic immune toxicities were *** with that measured at baseline,77.8%of the treated eyes showed improvement in best-corrected visual acuity(BCVA)on day 180,with a mean±standard deviation increase of 9.0±10.8 letters in the 9 eyes analyzed(p=0.021).By day 365,80%of the treated eyes showed an increase in BCVA,with a mean increase of 11.0±10.6 letters in the 5 eyes assessed(p=0.125).Importantly,the patients’improvement observed using multifocal electroretinogram,microperimetry,and Visual Function Questionnaire-25 further supported the beneficial effects of the *** conclude that the favorable safety profile and visual improvements identified in this trial encourage the continued development of rAAV2/8-hCYP4V2(named ZVS101e).
