Male gender and increased body mass index independently predicts clinically relevant morbidity after spleen-preserving distal pancreatectomy

AIM To identify risk factors for clinically relevant complications after spleen-preserving distal pancreatectomy(SPDP). No previous studies explored potential predictors of morbidity after *** The data of 41 patients who underwent a SPDP in a single surgical center between 2000 and 2015 were retrospectively reviewed from a prospectively maintained electronic database established in our Department of surgery. The database included demographic, clinical, bioumoral, pathological, intraoperative and postoperative parameters. Uni-and multivariate ana-lyses were performed to assess potential predictors of clinically relevant morbidity. Postoperative morbidity was defined as in-hospital complications and mortality was assessed at 90 d. Clinically relevant morbidity was defined as complication ≥ grade 2 *** Overall morbidity rate was 34.1%(14 patients): grade Ⅰ(6 patients, 14.6%), grade Ⅱ(2 patients, 4.8%), grade Ⅲa(1 patient, 2.4%), and grade Ⅲb(5 patients, 12.2%). A number of 5 patients(12.2%) required re-laparotomy for postoperative complications. There was no postoperative mortality. Thus, at least one clinically relevant complication occurred in 8 patients(19.5%). Univariate analysis identified male gender(P = 0.034), increased body mass index(P = 0.002) and neuroendocrine pathology(P = 0.013) as statistically significant risk factors. Multivariate analysis identified male gender [odds ratio(OR): 1.29, 95%CI: 1.07-1.55, P = 0.005] and increased body mass index(OR: 23.18, 95%CI: 1.72-310.96, P = 0.018) as the only independent risk factors of clinically relevant morbidity after *** Male gender and increased body mass index are independently associated with increased risk of clinically relevant morbidity after SPDP. These findings may assist a surgeon in clinical decision-making to better select patients suitable for SPDP.

Predicting morbidity and mortality in acute pancreatitis in an Indian population:a comparative study of the BISAP score,Ranson’s score and CT severity index

Objective:Our aim was to prospectively evaluate the accuracy of the bedside index for severity in acute pancreatitis(BISAP)score in predicting mortality,as well as intermediate markers of severity,in a tertiary care centre in east central India,which caters mostly for an economically underprivileged ***:A total of 119 consecutive cases with acute pancreatitis were admitted to our institution between November 2012 and October *** scores were calculated for all cases,within 24 hours of ***’s score and computed tomography severity index(CTSI)were also *** respective abilities of the three scoring systems to predict mortality was evaluated using trend and discrimination *** optimal cut-off score for mortality from the receiver operating characteristics(ROC)curve was used to evaluate the development of persistent organ failure and pancreatic necrosis(PNec).Results:Of the 119 cases,42(35.2%)developed organ failure and were classified as severe acute pancreatitis(SAP),47(39.5%)developed PNec,and 12(10.1%)*** area under the curve(AUC)results for BISAP score in predicting SAP,PNec,and mortality were 0.962,0.934 and 0.846,***’s score showed a slightly lower accuracy for predicting SAP(AUC 0.956)and mortality(AUC 0.841).CTSI was the most accurate in predicting PNec,with an AUC of *** sensitivity and specificity of BISAP score,with a cut-off of≥3 in predicting mortality,were 100%and 69.2%,***:The BISAP score represents a simple way of identifying,within 24 hours of presentation,patients at greater risk of dying and the development of intermediate markers of *** risk stratification method can be utilized to improve clinical care and facilitate enrolment in clinical trials.

Visualization of the protective role of estrogen against female liver fibrosis via an ER viscosity NIR fluorescent probe

Endoplasmic reticulum(ER) viscosity has emerged as a new potential biomarker for hepatic fibrosis(HF). Herein, we report the first ER-targeting viscosity-responsive near-infrared(NIR) fluorescent probe HBT-PP for investigations of HF *** twisted intramolecular charge transfer(TICT) and the excited state intramolecular proton transfer(ESIPT) are combined to endow HBT-PP with the selective response towards viscosity(～62-fold) and a super large stokes shift(～320 nm). HBT-PP mainly targets organelle ER and is able to image the drug-or alcohol-induced viscosity elevation in living cells. Significantly,HBT-PP was successfully applied to visualize the gender difference in HF and the protective role of estrogen in restraining HF progression. Of note, after 10 min of stain with HBT-PP, HF lesions showed a stage-based response, indicating the potential of this probe in fast and early diagnosis of HF. These results clearly demonstrated the promising application of HBT-PP as a new tool for exploring ER viscosity-involved physiological and pathological processes.

Nomogram to predict prolonged postoperative ileus after gastrectomy in gastric cancer

The conventional perception of astrocytes as mere supportive cells within the brain has recently been called into question by empirical evidence, which has revealed their active involvement in regulating brain function and encoding behaviors associated with ***, astrocytes in the basolateral amygdala have been found to play a role in the modulation of anxiety-like behaviors triggered by chronic stress. Nevertheless, the precise molecular mechanisms by which basolateral amygdala astrocytes regulate chronic stress–induced anxiety-like behaviors remain to be fully elucidated. In this study, we found that in a mouse model of anxiety triggered by unpredictable chronic mild stress, the expression of excitatory amino acid transporter 2 was upregulated in the basolateral amygdala. Interestingly, our findings indicate that the targeted knockdown of excitatory amino acid transporter 2 specifically within the basolateral amygdala astrocytes was able to rescue the anxiety-like behavior in mice subjected to stress. Furthermore, we found that the overexpression of excitatory amino acid transporter 2 in the basolateral amygdala, whether achieved through intracranial administration of excitatory amino acid transporter 2agonists or through injection of excitatory amino acid transporter 2-overexpressing viruses with GfaABC1D promoters, evoked anxiety-like behavior in mice. Our single-nucleus RNA sequencing analysis further confirmed that chronic stress induced an upregulation of excitatory amino acid transporter 2 specifically in astrocytes in the basolateral amygdala. Moreover, through in vivo calcium signal recordings, we found that the frequency of calcium activity in the basolateral amygdala of mice subjected to chronic stress was higher compared with normal *** knocking down the expression of excitatory amino acid transporter 2 in the basolateral amygdala, the frequency of calcium activity was not significantly increased, and anxiety-like behavior was ob

Total mesopancreas excision for pancreatic head cancer: analysis of 120 cases

Objective： To evaluate the feasibility and safety of total mesopancreas excision （TMpE） in the treatment of pancreatic head cancer. Methods： The clinical and pathological data of 120 patients with pancreatic head cancer who had undergone TMpE in our center from May 2010 to January 2014 were retrospectively analyzed. Results： The mean operative time was （275.0±50.2） min and the average intra-operative blood loss was （390.0±160.5） mL. Post-operative complications were reported in 45 patients, while no peri-operative death was noted. The specimen margins were measured in three dimensions, and 86 patients （71.6%） achieved R0 resection. Conclusions： TMpE is safe and feasible for pancreatic head cancer and is particularly helpful to increase the R0 resection rate.

Rechargeable zinc-air batteries(ZABs) have recently drawn great attention in energy research due to their high theoretical capacity,low costs, and inherently safe nature [1–3]. However, the sluggish cathode reactions necessitate the development of bifunctional oxygen electrocatalysts with lower ΔE indicator values. The ΔE indicator is commonly employed to quantitatively evaluate the electrocatalytic activity of a bifunctional oxygen electrocatalyst,representing the overall overpotential from oxygen reduction reaction(ORR) to oxygen evolution reaction(OER).

Morphological Mechanism and Experimental Verification of Self-healing of Basalt-fiber Modified Water-soaked Asphalt

This study investigates the mechanism of action of representative molecules of basalt fibers on the healing of water-soaked *** parameters,morphological characteristics,interfacial healing energy,and interfacial healing strength were analyzed using molecular dynamics and macroscopic tests under different time,temperature,and water conditions to evaluate the specific states and critical conditions involved in *** results indicate that basalt-fiber molecules can induce rearrangement and a combination of water-soaked asphalt at the healing *** groups with different bonding states increase the interfacial adsorption capacity of water-soaked *** interaction between basalt fiber molecules and water molecules leads to a"hoop"phenomenon,while aromatics-2 molecules exhibit a"ring band aggregation"*** former reduces the miscibility of water and asphalt molecules,while the latter causes slow diffusion of the ***,a micro-macro dual-scale comparison of interfacial healing strength was conducted at temperatures of 297.15 and 312.15 K to identify the strength transition point and critical temperature of 299.4 K during the self-healing process of basalt-fiber modified water-soaked asphalt.
