Characteristics and quality of clinical practice guidelines for diabetic kidney disease:a systematic review

OBJECTIVE:To assess the quality of Clinical practice guidelines(CPGs)in the context of diabetic kidney disease(DKD)and determine whether any factors affect the ***:We searched eight databases along with five international and national organizations to develop or archive guidelines from their inception to July 2023,with an additional search of *** the authoritative organizations related to *** and consensus statements created using direct differential diagnosis or therapy for DKD were included without language *** quality was evaluated by four reviewers using the Appraisal of Guidelines for Research and EvaluationⅡ(AGREEⅡ)*** with the item and domain scores,the guideline was also allocated an overall quality score,which ranged from 1(lowest possible quality)to 7(highest possible quality).Moreover,an overall recommendation for use was also assigned(“recommended”,“recommended with modifications”or“not recommended”).RESULTS:A total of 16 CPGs were included,of which 14 were from Asia and the remaining two from *** two CPGs were updated in the third *** CPGs were recommended for use because their primary domains scored in the medium or high ***,five CPGs were recommended with modifications as the stakeholder involvement,applicability,and editorial independence domains were evaluated as low *** all domains,the lowest average score was for rigour of development(33%),followed by application(36%),and stakeholder involvement(51%).The highest average score was for scope and purpose(79%),followed by clarity of presentation(75%).None of the CPGs considered the patient's viewpoint,and six of 16 CPGs did not use any grading system to translate the evidence into ***,only three of 16 CPGs shared search strategy,and eight of 16 CPGs did not declare a funding ***:According to the AGREE II evaluation,more than one in four CPGs for DKD had poo

The pancreas exerts endocrine and exocrine functions in energy *** neural innervation and immune milieu are both crucial in supporting pancreatic *** neuronal network connects the pancreas with the central nervous system(CNS)and the enteric nervous system(ENS)and sustains metabolic *** nerves in the pancreas are categorized as spinal sensory afferent fibers,vagal sensory afferent nerves,autonomic fibers of both sympathetic and parasympathetic divisions,and fibers from the ENS and intrapancreatic *** innervate different regions and various cell types,which collectively determine physiological *** have established that the diverse pathological conditions,including pancreatitis,diabetes,and pancreatic tumor,are attributed to aberrant immune reactions;however,it is largely not clear how the neuronal network may influence the disease *** by the recent advances illuminating the organ-wide neuronal architecture and the dysfunctions in pancreatic disorders,this review will highlight emerging opportunities to explore the cellular interrelationship,particularly the neuroimmune components in pancreatic health and diseases.

Background Leprosy is a chronic infectious disease that can lead to deformities and disabilities. Better detection and early diagnosis of leprosy is a public health priority that requires attention. This study aimed to identify the cluster of leprosy symptoms with a higher risk to provide reference of leprosy *** Leprosy symptomatic surveillance was conducted in ten districts of Shenzhen from 2010 to 2023 to detect suspected leprosy cases at all public medical institutions. Data on socio-demographic characteristics, patient symptoms, and relevant test results were collected. Cluster analysis and logistic regression were *** A total of 3459 leprosy patients were notified in Shenzhen, with the prevalence of 3.87%(134/3459). Eight common symptoms of leprosy and positivity rates were identified:(NO.1) chronic skin diseases that have not been diagnosed or cured for a long time(86.9%, 3007/3459);(NO.2) local dryness or paraesthesia of the skin(12.8%, 444/3459);(NO.3) thinning eyebrows with a drunken face(7.4%,256/3459);(NO.4) swelling or nodules on the face or earlobes(4.2%, 144/3459);(NO.5) edema or nodular erythema lesion(5.4%, 188/3459);(NO.6) coarse, painful or tender nerves(2.0%, 69/3459);(NO.7) painless wounds, sores, or easy burns on the hands or feet(3.1%, 107/3459);(NO.8) the eyes are not closed, the mouth angle is skewed, the palm finger muscle is atrophied or the claw hand, foot and other visible deformities of the face, hands and feet(0.8%, 29/3459). The rate of leprosy positivity found in Cluster 1(NO.2,NO.7 and NO.8), Cluster 2(NO.1 and NO.3), and Cluster3(NO.4,NO.5and NO.6) was 3.24%, 3.93%, and 3.70% respectively. The effects of selenium on the translation efficiency of selenoproteins Gpx1， Gpx4， and Txnrd1 in oxLDL-induced ***926 cells

Selenium is a crucial trace element that contributes to physiological processes in the body as selenoproteins. Selenoproteins serve as an integral role in the body in controlling the redox state of cells and protecting against damage induced by oxidative stress. This study aimed to investigate the effects and possible mechanism of selenium on selenoproteins expression in ***926 cells induced by oxLDL. The impact of selenium on the viability of ***926 cells was detected by the MTT method， and intracellular ROS level and mitochondrial membrane potential were assessed by fluorescent probe DCFH-DA and JC-1， respectively. RNA-seq， quantitative real-time PCR (RT-qPCR)， and western blot were used to investigate the selenoprotein expression. Selenoprotein mRNA translation efficiency was analyzed by ribosome profiling (Ribo-Seq) coupled with transcriptomics. Our data showed that selenium supplementation (0.5 μmol/L) significantly decreased ROS production， reduced mitochondrial inner membrane potential and increased the proliferative activity of ***926 cells induced by oxLDL. Moreover， The protective effects of selenium against oxLDL-induced ***926 cell injury were associated with the upregulation of the expressions of selenoproteins GPX1， GPX4， and TXNRD1. Furthermore， the expressions of selenoproteins GPX1 and GPX4 were hierarchically controlled， but the expressions of selenoproteins TXNRD1 were mainly regulated by oxLDL. Finally， Ribo-Seq coupled with transcriptomics results demonstrated that the expressions of selenoproteins GPX1， GPX4， and TXNRD1 were regulated at the translation process level. These findings suggested that selenium could have preventive effects in oxLDL induced ***926 cell injury by regulating the selenoprotein expression， and the selenoproteins expressions at the translation level in vascular endothelial cells need further study.
