Plasma metabolic biomarkers for syndrome of phlegm and blood stasis in hyperlipidemia and atherosclerosis

OBJECTIVE:To explore the plasma metabolite profiles in patients with the syndrome of phlegm and blood stasis in hyperlipidemia and atherosclerosis(As),and to search for the metabolic biomarkers of the syndrome.METHODS:The plasma metabolite profiles of 31 patients with the syndrome of phlegm and blood stasis in hyperlipidemia and As,6 patients with syndromes without phlegm and blood stasis,and 10 healthy subjects were analyzed by gas chromatography-mass spectrometry(GC-MS).Partial least squares-discriminant analyses(PLS-DA) were used to carry out the pattern-recognition analyses of the data.The plasma metabolic biomarkers of patients were obtained by variable importance plot value(VIP value) and Student's t-test.The structures of biomarkers were defined by the National institute of Standards andTechnology(NIST) database.RESULTS:PLS-DA score plots of plasma metabolomes did not show overlap between the phlegm-blood stasis syndrome group and syndromes without phlegm and blood stasis group,whereas significant differences in the concentrations in the plasma of 5 metabolites were found(P< 0.05).They were identified as urine,isoleucine,glucuronic acid,palmitic acid and glycerol by searching in NIST database.The concentrations of four metabolites in the plasma of patients with syndrome of phlegm and blood stasis were higher than those with syndromes without phlegm and blood stasis,whereas the glycerol concentration was lower.CONCLUSION:Compared with patients with syndromes without phlegm and blood stasis,five metabolites showed abnormal levels in patients with the syndrome of phlegm and blood stasis.These metabolites could be diagnostic and prognostic biomarkers.

Objective： To identify the commonalities between rheumatoid arthritis （RA） and diabetes mellitus （DM） to understand the mechanisms of chinese medicine （CM） in different diseases with the same treatment. Methods： A text mining approach was adopted to analyze the commonalities between RA and DM according to CM and biological elements. The major commonalities were subsequently verified in RA and DM rat models, in which herbal formula for the treatment of both RA and DM identified via text mining was used as the intervention. Results： Similarities were identified between RA and DM regarding the CM approach used for diagnosis and treatment, as well as the networks of biological activities affected by each disease, including the involvement of adhesion molecules, oxidative stress, cytokines, T-lymphocytes, apoptosis, and inflammation. The Ramulus Cinnamomi-Radix Paeoniae Alba-Rhizoma Anemarrhenae is an herbal combination used to treat RA and DM. This formula demonstrated similar effects on oxidative stress and inflammation in rats with collagen-induced arthritis, which supports the text mining results regarding the commonalities between RA and DM. Conclusion： Commonalities between the biological activities involved in RA and DM were identified through text mining, and both RA and DM might be responsive to the same intervention at a specific stage.

OBJECTIVE: To systematically evaluate the efficacy and safety of blood pricking and cupping in the treatment of nonspecific low back pain(NLBP).METHODS:The China National Knowledge Infrastructure Database(CNKI), Wanfang Med Online Database(Wanfang), China Science and Technology Journal Database(VIP), Pub Med, Web of Science,EMBase, and Cochrane Library databases were searched from inception to October 31, 2021, for randomized controlled trials(RCTs) of the treatment of NLBP by blood pricking and cupping. The quality of each included study was evaluated according to the Cochrane Systematic Assessor’s Manual evaluation criteria, while the Meta-analysis was performed using RevMan 5.4software.RESULTS: A total of 13 thirteen RCTs included a total of 1088 subjects. The visual analogue scale scores indicated that blood pricking and cupping was superior to the other treatments at relieving NLBP pain [mean difference(MD) =-1.43;95% confidence interval(CI):-2.31 to 0.54;Z = 3.15;P = 0.002). The Oswestry Disability Index score of blood pricking and cupping was superior to that of the control group in terms of improving NLBP dysfunction(MD =-6.25;95%CI:-8.37 to-4.31);Z = 5.77;P < 0.000 01). 7 RCTs mentioned no adverse reactions, while one study reported mild syncope [7%(n = 3), all in the 17-30 years group] that was gradually relieved after rest. CONCLUSIONS: Blood pricking and cupping therapy can safely and effectively reduce pain and improve functional impairment in patients with NLBP. However, this study included limited high-grade studies with a small overall sample size;therefore, further high-quality large-sample RCTs are needed to improve clinical evidence.

chinese tongue diagnosis was initially developed to quickly and efficiently diagnose and prescribe medicine, while at the same time allowing the doctor to have minimal contact with the patient. At the time of its compiling, the spread of Yersinia pestis, often causing septicaemia and gangrene of the extremities,may have discouraged doctors to come in direct contact with their patients and take the pulse.However, in recent decades, modern developments in the field of traditional chinese medicine, as well as the spread of antibiotics in conjunction with the advancements of microbiology, have overshadowed the original purpose of this methodology. Nevertheless, the fast approaching post-antibiotic era and the development of artificial intelligence may hold new applications for tongue diagnosis. This article focuses on the historical development of what is the world＇s earliest tongue diagnosis monograph, and discusses the directions that such knowledge may be used in future clinical research.

Mechanisms of ＂kidney governing bones＂ theory in traditional chinese medicine

Studies conducted by our group on the mechanism of ＂kidney governing bones＂ theory in traditional chinese medicine （TCM） are reviewed in this paper. Conclusions can be summarized as follows. （1） Neuroendocrine-immune network （NIN）-osteoclast regulatory pathway OPG-RANKL-RANK is one of the mechanisms of ＂kidney governing bones.＂ Although kidney-reinforcing therapy is regarded as one of the holistic regulatory mechanisms of the body, characteristic holistic regulation in TCM can be reflected through nonselective regulation of the NIN during kidney reinforcement therapy, which can be used to treat osteoporosis through microadjustments in the microenvironment of the bone marrow. （2） Marrow exhaustion in TCM, which is the state wherein lipocytes in the bone marrow increase whereas other cells decrease, serves as the pathogenesis of osteoporosis brought about by failure of the ＂kidney governing bones.＂（3） The kidney in TCM can be regarded as a complex system comprising multiple functional units in the body, including the unit ＂governing bones.＂ Kidney deficiency refers to a deficiency in only one or more units of the kidney system and not the whole system itself, which explains the kidney-reinforcing effect of many herbs; some herbs can treat osteoporosis, but some cannot. Although both classified as kidney-reinforcing agents, the former can resolve failure of the ＂kidney governing bones＂ unit while the latter regulates the failure of other units in the kidney system. Despite the current understanding on ＂kidney governing bones＂ theory, the mechanism of ＂kidney governing bones＂ remains complicated and unresolved. Thus, further studies in this area are warranted.

Effects of Total Saponins from Dioscorea Nipponica Makino on Monosodium Urate-Induced M1-Polarized Macrophages through Arachidonic Acid Signaling Pathway:An in vitro Study

Objective To investigate and reveal the underlying mechanism of the effect of total saponins from Dioscoreae nipponica Makino(TSDN)on the arachidonic acid pathway in monosodium urate(MSU)crystal-induced M1-polarized macrophages.Methods M1 polarization of RAW264.7 cells were induced by 1µg/mL lipopolysaccharide(LPS).The methylthiazolyldiphenyl-tetrazolium bromide method was then used to screen the concentration of TSDN.MSU(500µg/mL)was used to induce the gouty arthritis model.Afterwards,10µg/L TSDN and 8µmol/L celecoxib,which was used as a positive control,were added to the above LPS and MSU-induced cells for 24 h.The mRNA and protein expressions of cyclooxygenase(COX)2,5-lipoxygenase(5-LOX),microsomal prostaglandin E synthase derived eicosanoids(mPGES)-1,leukotriene B(LTB)4,cytochrome P450(CYP)4A,and prostaglandin E_(2)(PGE_(2))were tested by real-time polymerase chain reaction and Western blotting,respectively.The enzyme-linked immunosorbent assay was used to test the contents of M1 markers,including inducible nitric oxid synthase(NOS)2,CD80,and CD86.Results TSDN inhibited the proliferation of M1 macrophages and decreased both the mRNA and protein expressions of COX2,5-LOX,CYP4A,LTB4,and PGE_(2)(P<0.01)while increased the mRNA and protein expression of mPGES-1(P<0.05 or P<0.01).TSDN could also significantly decrease the contents of NOS2,CD80,and CD86(P<0.01).Conclusion TSDN has an anti-inflammation effect on gouty arthritis in an in vitro model by regulating arachidonic acid signaling pathway.

Background:The global outbreak of coronavirus disease 2019(COVID-19)has brought disastrous consequences to public health and medical systems,whereas no approved medications are currently available.Benefits of traditional chinese medicine(TCM)against COVID-19 have been observed,however,the underlying mechanistic actions remain unclarified.Due to high pathogenicity and infectivity of the new coronavirus(severe acute respiratory syndrome coronavirus 2[SARS-CoV-2]),the lack of access to SARS-CoV-2 and biosafety level 3(P3)facilities has impeded scientific investigations of TCM against COVID-19.Though low-pathogenic coronavirus and pseudoviral systems have been applied to substitute SARS-CoV-2 in fundamental studies,both models cannot imitate virological and clinical features associated with SARS-CoV-2.The virus-like particle(VLP)is a virological model that is safe and could be performed without biosafety protections.Aims and Objectives:To construct VLP of SARS-CoV-2 containing structural proteins of authentic viruses and resembling the morphology,partial life cycle,and immunoreactions of natural virions,and to introduce VLP into chinese medical research.Materials and Methods:Using mammalian expression system,we have currently constructed SARS-CoV-2 VLP containing four essential structural proteins.Results:Based on this model,we propose six aspects of research that could be carried out for TCM formulas in the fight against COVID-19.Conclusion:Application of the VLP model provides a safe methodology to strengthen the response systems of chinese medicine in preventing and controlling newly identified infectious diseases and offers collaborative opportunities for interdisciplinary deciphering of molecular and biological basis of anti-viral TCM formulas.

The Pi(Spleen)is responsible for the formation and transportation,and it is in the center.It is the source of qi and blood generation and transformation,so it has an essential position.If the qi and blood are biochemically passive,the functions of the human body will age prematurely.Mitochondria are for energy synthesis and have always been a hot topic in modern medical research.Many of their functions are similar to those of the Pi in chinese medicine.In chinese medicine,strengthening the Pi is to enhance the Pi’s ability to transport and transform,and to speed up the Pi,which is similar to the ability of mitochondria to maintain the ability to remove damaged cells.This can speed up the body’s ability to absorb nutrients and delay aging.

OBJECTIVE:To study the absorption and biotransformation of liquiritin,cinnamic acid,paeoniflorin,and glycyrrhizic acid in the Guizhi decoction (GZD) in the gastrointestinal tracts of rats.METHODS:A simple and reliable high-performance liquid chromatography method was established and validated for the analysis of the four components of GZD simultaneously in the gastrointestinal tracts of rats.Rats were randomly divided into in situ gastrointestinal loop model,in vitro anaerobic culture model,and blank control groups.All rats were fasted for 12 h and anesthetized using 20% urethane.Subsequently,the abdominal cavity of each rat was opened,and the stomach,duodenum,jejunum,ileum,cecum,and colon were ligated.For the in situ gastrointestinal loop model group,2.5 mL of GZD (1.0 g crude drug/mL,37 ℃)were injected into the gastrointestinal tract.The abdominal incision was covered with warm,wet cotton,and animals were maintained at 25 ℃.Then,we collected the gastrointestinal tract content after 1.5 h.For the in vitro anaerobic culture model group,the gastrointestinal tract contents of rats were collected and then cultured in 2.5 mL of GZD in an anaerobic environment at 25 ℃ for 24 h.For the blank control group,rats received the same volume of a normal saline solution instead of GZD.High performance liquid chromatography was used to detect the liquiritin,cinnamic acid,paeoniflorin,and glycyrrhizic acid concentrations in each group and calculate the absorption and biotransformation rates of each ingredient.RESULTS:Cinnamic acid (low polarity) was more easily absorbed by each gastrointestinal part than the higher-polarity glycosides.However,the absorption rate in the cecum was higher than that in other parts.The four compounds,cinnamic acid,liquiritin,paeoniflorin,and glycyrrhizic acid,were transformed completely within 24 h in the cecum and colon,whereas they were hardly transformed in the stomach,excluding glycyrrhizic acid.In addition,all ingredients had higher biotransformation rates in the distal small intestine than that
