Objective:We assessed the longitudinal risk of developing cervical intraepithelial neoplasia(CINs)with self-sampling human papillomavirus(HPV)tests,based on polymerase chain reaction(PCR)and signal amplification(care HPV),to explore the appropriate intervals for cervical cancer ***:A prospective study was conducted in China during *** were invited for PCR and care HPV tests with self-samples at *** positive in either HPV test underwent colposcopy and biopsy if *** with baseline CIN grade one(CIN1)or less were followed up over 3 *** absolute risk was assessed by the immediate risk(IR)and cumulative risk(CR),and the relative risk was assessed by the hazard ratio(HR)with a 95%confidence interval(CI).Results:A total of 8,126 women were included in the final *** positive for the PCR HPV test had comparable IRs of CIN2+and CIN3+to those positive on the care HPV *** triage by HPV genotyping,women with HPV 16/18 infection had the highest IRs of CIN2+(21.15%)and CIN3+(9.67%).For CR,women negative for PCR HPV test had a lower risk of CIN2+than that reported in women negative on care HPV test(0.57%versus 0.98%,HR=0.58,95%CI:0.38,0.87),but no significant difference was found in the CRs of CIN3+between them(0.25%versus 0.39%,HR=0.64,95%CI:0.34,1.20).Among women with CIN1 or less at baseline,women who were persistent or recurrent positive on care HPV or PCR HPV test had a higher risk of developing CIN3+(11.36%-14.59%),compared with women remained HPV negative from baseline throughout follow-up(≤0.28%).Conclusions:Routine screening with 3-year intervals is acceptable for self-sampling HPV tests based on PCR or care HPV *** positive on HPV16/18 triaging at baseline or with CIN1 or less at baseline while being per-sistent or recurrent positive on care HPV or PCR HPV test during 3-year follow-up require immediate colposcopy or treatment.

Metabolic syndrome: a risk factor for high intraocular pressure in the Israeli population

AIM: To evaluate the association among elevated intraocular pressure(IOP), the metabolic syndrome(Met S), body mass index(BMI), and some of their components in the Israeli ***: We retrospectively reviewed the charts of12 747 soldiers of the Israeli Defense Forces, aged 35 y or older, who underwent a routine periodical medical examination between 1991 and 2004. None of the subjects received medical treatment for either glaucoma or ocular hypertension. High IOP(>21 mm Hg) was correlated with age, sex, arterial blood pressure, total blood cholesterol levels, triglyceride levels, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, smoking, BMI and Met ***: A statistically significant difference was found between the IOP of subjects with a low risk and higher risk for the development of Met S(P <0.0001 for males, P =0.0026 for females). A statistically significant positive correlation was found in male subjects between high BMI and elevated IOP(r =0.11677, P <0.0001).CONCLUSION: Met S and BMI were significantly more prevalent in subjects with increased IOP levels. We suggest that both should be taken into consideration in the assessment of glaucoma suspects.

The cornerstone of antimalarial treatment, artemisinin, has reduced malaria associated morbidity and mortality worldwide. However, Plasmodium falciparum parasites with reduced sensitivity to artemisinin have emerged, and this threatens malaria control and elimination efforts. In this minireview, we describe the initial development of artemisinin as an antimalarial drug,its use both historically and currently, and our current understanding of its mode of action and the mechanisms by which malaria parasites achieve resistance.

Do peripartum and postmenopausal women with primary liver cancer have a worse prognosis? A nationwide cohort in Taiwan

Pulmonary vein isolation implemented by second-generation cryoballoon for treating hypertrophic cardiomyopathy patients with symptomatic atrial fibrillation:a case-control study

Background Atrial fibrillation(AF)is a generally acknowledged turning-point of the natural history of hypertrophic cardiomyopathy(HCM);however,data from the cryoballoon ablation(CBA)for AF in HCM patients are relatively *** study aimed to evaluate the efficacy and safety of CBA in HCM patients with *** We retrospectively analyzed HCM patients among 1253 patients with symptomatic AF who underwent CBA for pulmonary vein isolation in a single *** study analyzed the AF recurrence and assessed the CBA indexes,including nadir temperature,time-to-isolation,CBA failure,pulmonary vein potentials(PVPs),and redo *** A total of 108 patients were included(mean age:59.0±6.9 years),27 patients(25%)had HCM,with the median follow-up duration of 25.5 *** one-year AF-free rates were 79.0%vs.63.0%(non-HCM ***),while the two-year AF-free rates were 77.8%vs.55.1%[hazard ratio(HR)=2.758,log-rank P=0.024].Patients with persistent AF had poor AF-free rates compared to those with paroxysmal AF(P<0.001).The CBA failure was the most common in the right inferior pulmonary veins,which had the lowest *** Cox regression analysis indicated that HCM and persistent AF were risk factors for AF recurrence(HR=2.74,95%CI:1.29–5.79,P=0.008;and HR=3.97,95%CI:1.85–8.54,P<0.001,respectively).Conclusions The CBA can be effectively and safely used to treat HCM patients with symptomatic *** freedom from AF for HCM patients after CBA is relatively low compared to that for non-HCM patients.

Inhibition of cathepsin B protects pancreatic acinar cells against apoptosis in early pancreatic trauma in rats

Objectives:To observe the protective effect of cathepsin B inhibition against apoptosis of acinar cells in the early management of pancreatic contusion and laceration in rats,which would provide evidence of a potential early therapeutic for pancreatic contusion and ***:Twenty-four rats were assigned to 2 groups:1)Model(n=12)with an induced pancreatic injury of severity I-II and 2)CA074-V(n=12):an induced pancreatic injury,severity I-II treated with the cathepsin B inhibitor CA074-me(0.01mg/g)by intravenous administration through the caudal vein at 5minutes post model *** mice in these two groups were further randomly divided into 4 subgroups containing 3 rats each that were sacrificed for quantitation of apoptosis,immunohistochemistry of cathepsin B,and serum amylase and lipase measurements at different time points after model establishment(0,3,6,and 12hours).Results:The percentage of apoptotic pancreatic acinar cells collected from the injured tissues were much lower in the CA074-V group than the Model group at 3hours[9.25±3.94%vs.64.76±26.47%,P<0.10]and 6hours[14.71±8.22%vs.66.60±13.54%,P<0.10]post model *** percentage of cathepsin B-positive pancreatic acinar cells were much lower in the CA074-V group than in the Model group at 3hours[31.07±12.02%vs.69.16±5.71%,P<0.10],6hours[24.84±0.93%vs.47.06±0.91%,P<0.10],and 12hours[28.33±9.14%vs.52.72±1.25%,P<0.10]post model ***:Early cathepsin B inhibition effectively blocked acinar cell apoptosis in an experimental rat model of pancreatic contusion and laceration.
