Outcomes and variables that impact pneumatic retinopexies

●AIM:To evaluate the efficacy of pneumatic retinopexy(PR)in patients undergoing PR as primary treatment for rhegmatogenous retinal detachment(RRD)and analyze the factors associated with success and failure in the studied population.●METHODS:A retrospective chart review was done of patients with RRD treated with PR as primary management method treated at New York Eye and Ear Infirmary of Mount Sinai between January 2017 and December 2021.Primary outcome measured success or failure of PR.Secondary outcome measured best corrected visual acuity(BCVA)after PR.A separate risk analysis was done to identify and stratify risks associated with success and failure of PR.●RESULTS:A total of 179 eyes from 179 patients were included for final analysis.The 83 patients(46.37%)achieved anatomical reattachment of the retina after primary PR with no need for additional surgery.The 96 patients(53.63%)had a failed primary PR and required a PPV and 6 of them required pars plana vitrectomy(PPV)with scleral buckle(SB).In total,19 cases(10.61%)were done as temporizing pneumatics,18(94.74%)underwent PPV,and 1(5.26%)did not require further intervention.The visual acuities at postoperative month 1(POM1)for patients who underwent primary PR successfully and for those that underwent PPV after,were 0.58(20/80)and 1.03(20/200)respectively.Patients who met Pneumatic Retinopexy Versus Vitrectomy for the Management of Primary Rhegmatogenous Retinal Detachment Outcomes Randomized Trial(PIVOT)criteria had a statistically significant decreased risk of primary PR failing(hazard ratio 0.29,P=0.00).Majority of missed or new breaks were found superotemporally.●CONCLUSION:PR is a good treatment option for treating RRDs in patients that meet PIVOT criteria and can be conducted as a temporizing measure.PIVOT criteria and fovea on status decrease the risk of PR failure.

Necrotizing otitis externa(NOE)is an aggressive and fast‐evolving infection of the external auditory canal.Late diagnoses and untreated cases can lead to severe,even fatal consequences and so early diagnosis and treatment are paramount.NOE is a notoriously challenging diagnosis to make.It is therefore important to understand what diagnostic modalities are available and how otolaryngologists can use them to accurately treat such an aggressive disease.This review aims to evaluate the different diagnostic options available in NOE and discuss their advantages and limitations,thus,providing an up‐to‐date picture of the multimodal approach required in the diagnosis of this disease.

Application of UiO-66 and its composites for remediation and resource recovery of typical environmental contaminants:a review

UiO-66 series metal-organic framework materials（MOFs） are typical porous materials assembled by Zr4+with a large mass-to-nucleus ratio and terephthalic acid ligands,which form tetrahedral and octahedral cages arranged in a periodic triangular window pattern.Due to the strong interaction between Zr and O,UiO-66 series MOFs exhibit high thermal stability,structural stability,and chemical stability.This article mainly reviews the applications of UiO-66 and its composites in adsorption,photocatalysis,and resource utilization,while exploring the harm of pollutants to human health and the environment.In the first part,the differences in adsorption and removal mechanisms of liquid organic pollutants,heavy metals,and volatile organic compounds（VOCs） are investigated.The results show that organic pollutants are mainly removed by physical adsorption,electrostatic interactions,hydrogen bonding,and π-π interactions,while heavy metals are mainly removed by chemical adsorption,electrostatic interactions,reduction,and chelation.VOCs are mainly removed by the pore volume and pore size structure of the material.Heterojunction catalysis can achieve non-toxic treatment of pollutants,and this study mainly focuses on UiO-based composite materials constructed by strategies such as semiconductor composites,ion doping,and metal/dye encapsulation.In the second part,the synergistic effect between the components of UiO-based composite materials promotes the oriented and rapid separation and transfer of earriers at the material interface,thereby promoting the generation of active species such as h+,·O2-and ·OH,and achieving rapid degradation of pollutants and detoxification of heavy metals.In the third part,heterojunctions can realize the resource utilization of pollutants in water and air,producing energy-type substances such as hydrogen and methanol while solving environmental problems.In addition,this article also summarizes the harm of common typical pollutants to the environment and human health.Finally,the development prospects and un

The term hepatolithiasis describes the presence of biliary stones within the intrahepatic bile ducts,above the hilar confluence of the hepatic ducts.The disease is more prevalent in Asia,mainly owing to socioeconomic and dietary factors,as well as the prevalence of biliary parasites.In the last century,owing to migration,its global incidence has increased.The main pathophysiological mechanisms involve cholangitis,bile infection and biliary strictures,creating a self-sustaining cycle that perpetuates the disease,frequently characterised by recurrent episodes of bacterial infection referred to as syndrome of“recurrent pyogenic cholangitis”.Furthermore,long-standing hepatolithiasis is a known risk factor for development of intrahepatic cholangiocarcinoma.Various classifications have aimed at providing useful insight of clinically relevant aspects and guidance for treatment.The management of symptomatic patients and those with complications can be complex,and relies upon a multidisciplinary team of hepatologists,endoscopists,interventional radiologists and hepatobiliary surgeons,with the main goal being to offer relief from the clinical presentations and prevent the development of more serious complications.This comprehensive review provides insight on various aspects of hepatolithiasis,with a focus on epidemiology,new evidence on pathophysiology,most important clinical aspects,different classification systems and contemporary management.

Combined aqueous misdirection and persistent choroidal effusions following implantation of a Preserflo Micro Shunt

AIM:To describe a case of aqueous misdirection complicated by subsequent persistent choroidal effusions following implantation of a Preserflo Micro Shunt(PMS)device to treat advanced closed angle glaucoma.METHODS:A 67-year-old caucasian female with advanced primar y angle-closure glaucoma on four medications with an intraocular pressure(IOP)of 26 mm Hg was listed for a PMS insertion with mitomycin C(MMC).RESULTS:Past ocular history was significant for pseudophakia and previous yttrium aluminum garnet(YAG)peripheral iridotomy.Surgery was uneventful but on the first postoperative day,she developed aqueous misdirection complicated by subsequent development of persistent uveal effusions.Conventional treatment strategies including atropine drops,YAG hyaloidotomy and choroidal effusion drainage proved ineffective.A combination of oral steroids and pars plana vitrectomy(PPV)along with an irido-zonulohyloidectomy(IZH)proved efficacious.CONCLUSION:To the best of the author’s knowledge,this is the first published case of aqueous misdirection complicated with the presence of significant,unresolving choroidal effusions,highlighting the possibility and sequelae of comorbid pathology in nanophthalmic eyes.

BACKGROUND Pancreaticoduodenectomy(PD)is a technically complex operation,with a re-latively high risk for complications.The ability to rescue patients from post-PD complications is as a recognized quality measure.Tailored protocols were instituted at our low volume facility in the year 2013.AIM To document the rate of rescue from post-PD complications with tailored protocols in place as a measure of quality.METHODS A retrospective audit was performed to collect data from patients who experienced major post-PD complications at a low volume pancreatic surgery unit in Trinidad and Tobago between January 1,2013 and June 30,2023.Stan-dardized definitions from the International Study Group of Pancreatic Surgery were used to define post-PD complications,and the modified Clavien-Dindo classification was used to classify post-PD complications.RESULTS Over the study period,113 patients at a mean age of 57.5 years(standard deviation[SD]±9.23;range:30-90;median:56)underwent PDs at this facility.Major complications were recorded in 33(29.2%)patients at a mean age of 53.8 years(SD:±7.9).Twenty-nine(87.9%)patients who experienced major morbidity were salvaged after aggre-ssive treatment of their complication.Four(3.5%)died from bleeding pseudoaneurysm(1),septic shock secondary to a bile leak(1),anastomotic leak(1),and myocardial infarction(1).There was a significantly greater salvage rate in patients with American Society of Anesthesiologists scores≤2(93.3%vs 25%;P=0.0024).CONCLUSION This paper adds to the growing body of evidence that volume alone should not be used as a marker of quality for patients requiring PD.Despite low volumes at our facility,we demonstrated that 87.9%of patients were rescued from major complications.We attributed this to several factors including development of rescue protocols,the competence of the pancreatic surgery teams and continuous,and adaptive learning by the entire institution,cul-minating in the development of tailored peri-pancreatectomy protocols.

The importance of performing an early primary survey in‘silver trauma’patients in the detection of injuries has been well documented in reducing the associated morbidity and mortality.[1]In the past,when whole body computed tomography(WBCT)was not commonly available,following initial examination patients would undergo chest radiograph and pelvic X-ray and then proceed to selective computed tomography(CT).With the relatively widespread availability of CT,the use of these X-rays has diminished.The reliance on clinical examination alone for the detection of underlying injuries is another matter of debate.

Pancreatic surgery units undertake several complex operations,albeit with consi-derable morbidity and mortality,as is the case for the management of complicated acute pancreatitis or chronic pancreatitis.The centralisation of pancreatic surgery services,with the development of designated large-volume centres,has contribu-ted to significantly improved outcomes.In this editorial,we discuss the complex associations between diabetes mellitus(DM)and pancreatic/periampullary disease in the context of pancreatic surgery and overall management of complex pancreatitis,highlighting the consequential needs and the indispensable role of specialist diabetes teams in support of tertiary pancreatic services.Type 3c pan-creatogenic DM,refers to DM developing in the setting of exocrine pancreatic disease,and its identification and management can be challenging,while the glycaemic control of such patients may affect their course of treatment and outcome.Adequate preoperative diabetes assessment is warranted to aid identification of patients who are likely to need commencement or escalation of glucose lowering therapy in the postoperative period.The incidence of new onset diabetes after pancreatic resection is widely variable in the literature,and depends on the type and extent of pancreatic resection,as is the case with pancreatic parenchymal loss in the context of severe pancreatitis.Early involvement of a specialist diabetes team is essential to ensure a holistic management.In the current era,large volume pancreatic surgery services commonly abide by the principles of enhanced recovery after surgery,with inclusion of provisions for optimisation of the perioperative glycaemic control,to improve outcomes.While various guidelines are available to aid perioperative management of DM,auditing and quality improvement platforms have highlighted deficiencies in the perioperative management of diabetic patients and areas of required improvement.The need for perioperative support of diabetic patients by specialist diabetes teams is uniform

Microsurgical repair of severed thoracic spinal cord and clinical outcome:technical case report

Background:This report describes a case of successful repair of severed thoracic spine in a young man who presented with a penetrating stab injury to spine resulting in Brown-Séquard syndrome.Surgical technique and postoperative management is discussed.Case presentation:A 34-year-old fit and well healthy man was admitted with a history of stab injury to the thoracic spine at thoracic T2/3 level with ASIA impairment score(AIS)score D with an incomplete spinal cord affecting his left lower limb with complete paralysis and right lower limb paresis with impaired sensation below T6 level to L5.Neuroimaging confirmed a penetrating knife injury traversing the T2/3 level causing hemi-section of the spinal cord confirmed intraoperatively.He underwent an urgent exploratory surgery of his spine and a T2/3 laminectomy was performed to aid removal of the knife.The dura was noted to be contused and severed spinal cord was noted to be severed with associated cord oedema.A microsurgical repair of the severed cord was performed with duroplasty followed by intense neuro-rehabilitation.On a 3 month follow up his AIS score is E with lower limb power is 5/5 bilaterally and he is able to mobilise independently up to 8-10 steps without any supportive aid and with crutches he is independently functional and mobile.Conclusion:This is the first documented case of microsurgical repair of severed thoracic spinal cord secondary to traumatic knife injury.In the management of such scenario,apart from the removal of foreign body,repair of the cord with duroplasty should be carefully considered.The role of spinal neuroplasticity in healing following timely repair of the spinal cord along with intense rehabilitation remains the key.This had resulted in a good clinical and functional outcome with in a 18-month follow up.

Congenital diaphragmatic hernia survival in an English regional ECMO center

Introduction Congenital diaphragmatic hernia(CDH)remains a cause of neonatal death.Our aims are to describe contemporary rates of survival and the variables associated with this outcome,contrasting these with our study of two decades earlier and recent reports.Materials and methods A retrospective review of all infants diagnosed in a regional center between January 2000 and December 2020 was performed.The outcome of interest was survival.Possible explanatory variables included side of defect,use of complex ventilatory or hemodynamic strategies(inhaled nitric oxide(iNO),high-frequency oscillatory ventilation(HFOV),extracorporeal membrane oxygenation(ECMO),and Prostin),presence of antenatal diagnosis,associated anomalies,birth weight,and gestation.Temporal changes were studied by measuring outcomes in each of four consecutive 63-month periods.Results A total of 225 cases were diagnosed.Survival was 60%(134 of 225).Postnatal survival was 68%(134 of 198 liveborn),and postrepair survival was 84%(134 of 159 who survived to repair).Diagnosis was made antenatally in 66%of cases.Variables associated with mortality were the need for complex ventilatory strategies(iNO,HFOV,Prostin,and ECMO),antenatal diagnosis,right-sided defects,use of patch repair,associated anomalies,birth weight,and gestation.Survival has improved from our report of a prior decade and did not vary during the study period.Postnatal survival has improved despite fewer terminations.On multivariate analysis,the need for complex ventilation was the strongest predictor of death(OR=50,95%CI 13 to 224,p<0.0001),and associated anomalies ceased to be predictive.Conclusions Survival has improved from our earlier report,despite reduced numbers of terminations.This may be related to increased use of complex ventilatory strategies.
