Transcription factor CncC potentially regulates the expression of multiple detoxification genes that mediate indoxacarb resistance in Spodoptera litura

The xenobiotic transcription factor cap‘n’collar isoform C(CncC)is considered the central regulator of antioxidant and detoxification *** research indicated that CncC regulates three-phase enzymes responsible for insecticide *** this study,the SlituCncC gene from Spodoptera litura was obtained and *** polymerase chain reaction(qPCR)analysis showed that SlituCncC was expressed in all developmental stages and tissues,but was highly expressed in 3rd-and 4th-instar larvae,and in the Malpighian tubule,fat body,and *** addition,SlituCncC was up-regulated and more highly induced with indoxacarb treatment in the indoxacarb-resistant strains compared with the susceptible *** interference-mediated gene silencing of SlituCncC significantly increased mortality of *** when exposed to ***,comparative transcriptome analysis showed that 842 genes were down-regulated and 127 genes were up-regulated in SlituC"ncC knockdown *** analysis indicated that 18 three-phase enzymes were identified in the down-regulated genes,of which seven were associated with indoxacarb resistance in *** analysis confirmed that expression of six of these seven genes was consistent with RNA se-quencing *** six detoxification genes were induced by indoxacarb,and the expression patterns were similar to that of ***,the CneC-Maf binding site was predicted in all six gene *** study indicates that the transcription factor SlituCncC may regulate multiple detoxification genes that mediate indoxacarb resistance in ***.

Long-term Effectiveness of Antiepileptic Drug Monotherapy in Partial Epileptic Patients： A 7-year Study in an Epilepsy center in China

Background： It is important to choose an appropriate antiepileptic drug （AED） to manage partial epilepsy. Traditional AEDs, such as carbamazepine （CBZ） and valproate （VPA）, have been proven to have good therapeutic effects. However, in recent years, a variety of new AEDs have increasingly been used as first-line treatments for partial epilepsy. As the studies regarding the effectiveness of new drugs and comparisons between new AEDs and traditional AEDs are few, it is determined that these are areas in need of further research. Accordingly, this study investigated the long-term effectiveness of six AEDs used as monotherapy in patients with partial epilepsy. Methods： This is a retrospective, long-term observational study. Patients with partial epilepsy who received monotherapy with one of six AEDs, namely, CBZ, VPA, topiramate （TPM）, oxcarbazepine （OXC）, lamotrigine （LTG）, or levetiracetam （LEV）, were identified and followed up from May 2007 to October 2014, and time to first seizure after treatment, 12-month remission rate, retention rate, reasons for treatment discontinuation, and adverse effects were evaluated. Results： A total of 789 patients were enrolled. The median time of follow-up was 56.95 months. CBZ exhibited the best time to first seizure, with a median time to first seizure of 36.06 months （95% confidential interval： 30.64~4.07）. CBZ exhibited the highest 12-month remission rate （85.55%）, which was significantly higher than those of TPM （69.38%, P = 0.006）, LTG （70.79%, P= 0.001）, LEV （72.54%, P = 0.005）, and VPA （73.33%, P = 0.002）. CBZ, OXC, and LEV had the best retention rate, followed by LTG, TPM, and VPA. Overall, adverse effects occurred in 45.87% of patients, and the most common adverse effects were memory problems （8.09%）, rashes （7.76%）, abnormal hepatic function （6.24%）, and drowsiness （6.24%）. Hemodynamic effects and safety of pulmonary angiography in Chinese patients with pulmonary hypertension

Background Pulmonary angiography is widely performed in pulmonary hypertension patients, but its immediate effects on right heart hemodynamics and safety are not well known. The objective of this study was to investigate the right heart hemodynamic effects and safety of pulmonary angiography in Chinese patients with pulmonary hypertension. Methods Between January 2008 and June 2009, pulmonary hypertension patients undergoing pulmonary angiography were consecutively enrolled. Pulmonary angiography was performed during breath-holding after deep breathing. The baseline clinical data, hemodynamic measurements before and after pulmonary angiography and complications occurring within 48 hours after angiography were recorded. Results Ninety-five patients were included. All received non-ionic contrast medium with a volume of （75.7±29.8） ml. Angiography reduced heart rate in patients with baseline mean pulmonary arterial pressure 〉 60 mmHg （change of heart rate：（-3.1±7.0） beats/min, P=0.005）, increased mean right atrial pressure, diastolic and end-diastolic right ventricular pressure in patients with baseline mean pulmonary arterial pressure 〈60 mmHg （all P 〈0.05）. Patients with decreased mean pulmonary arterial pressure （change of mean pulmonary arterial pressure 〈 -10 mmHg） had the highest total pulmonary resistance （P=0.009 vs. no change in mean pulmonary arterial pressure （change of mean pulmonary arterial pressure, -10 mmHg to 10 mmHg）; P=0.03 vs. increased mean pulmonary arterial pressure （change of mean pulmonary arterial pressure 〉 10 mmHg）） and the lowest cardiac output （P=-0.018 vs. no change in mean pulmonary arterial pressure; P=0.013 vs. increased mean pulmonary arterial pressure）. There were 7 complications （7%）, with 6 related to catheter and only 1 directly related to angiography. All complications were mild and no death occurred. Conclusion Pulmonary angiography has minimal effect on right heart hemodynamics and is safe in pulmonary hypertension patients.

Hepatic Cyp1a2 Expression Reduction during Inflammation Elicited in a Rat Model of Intermittent Hypoxia

Background： Intermittent hypoxia （IH） is a key element of obstructive sleep apnea （OSA） that can lead to disorders in the liver. In this study, IH was established in a rat model to examine its effects on the expression of hepatic cytochrome P450 （CYP） and CYP regulators, including nuclear receptors. Methods： Hematoxylin and eosin staining was conducted to analyze the general pathology of the liver of rats exposed to IH. The messenger RNA （mRNA） expression levels of inflammatory cytokines, CYPs, nuclear factor-κB （NF-κB）, and nuclear factors in the liver were measured by quantitative reverse transcription polymerase chain reaction. Results： We found inflammatory infiltrates in the liver of rats exposed to IH. The mRNA expression level of interleukin-1beta was increased in the liver of the IH-exposed rats （0.005 ± 0.001 vs. 0.038 ± 0.008, P = 0.042）, whereas the mRNA expression level of Cyp1a2 was downregulated （0.022 ± 0.002 vs. 0.0050 ± 0.0002, P = 0.029）. The hepatic level of transcription factor NF-κB was also reduced in the IH group relative to that in the control group, but the difference was not statistically significant and was parallel to the expression of the pregnane X receptor and constitutive androstane receptor. However, the decreased expression of the glucocorticoid receptor upon IH treatment was statistically significant （0.056 ± 0.012 vs. 0.032 ± 0.005, P = 0.035）. Conclusions： These results indicate a decrease in expression of hepatic CYPs and their regulator GR in rats exposed to IH. Therefore, this should be noted for patients on medication, especially those on drugs metabolized via the hepatic system, and close attention should be paid to the liver function of patients with OSA-associated IH.

Assessing the utility of the tipping point ratio to monitor HIV treatment programmes in the era of universal access to ART

Background Acute fatty liver of pregnancy(AFLP) can lead to fatal liver damage that occur in the third trimester or early postpartum *** effect of traditional therapy is not very ideal for facilitating hepatic *** safety and effect of non-bioartificial liver(NBAL) in application for treating AFLP are lacking systematic introduction. Aim Analysis of the timing and efficacy of non-bioartificial liver(NBAL) in the treatment of acute fatty liver of pregnancy(AFLP). Methods We observed the clinical data of 5 cases of AFLP treated with NBAL from 2007 to 2012 including symptoms,physical signs,adverse effects,and compared all relevant laboratory test results before and after NBAL. Results (1) Side effects during NBAL were slight and well-tolerated,and no patients interrupted treatment because of negligible side effects;Clinical symptoms,signs,and liver and renal functions can be ameliorated significantly after NBAL;(2) In the 5 cases treated,4 were cured,1 succeeded in bridging to transplantation;(3) In the 4 cases cured,the sooner a AFLP patient received NBAL the better the curative effect. Conclusion Treatment of AFLP by NBAL is safe and effective,and early application of NBAL in AFLP patients can effectively treat and reverse progress of the disease.
