Can patients with mild non-neoplastic lesions diagnosed at baseline screening be safely exempt from surveillance: evidence from multicenter community-based cohorts

Surveillance recommendations for gastric cancer（GC） in current guidelines focused on advanced precancerous lesions and were based on precise diagnosis of severity/extent of baseline lesions. We aimed to develop a less endoscopy-related equipment-dependent risk-stratification tool, and assessed whether mild-precursor-lesion patients can be safely exempt from surveillance. In the multicenter communitybased cohort, 75,051 participants receiving baseline endoscopy were enrolled during 2015–2017 and followed-up until 2021. Cumulative incidence rates（CIRs） of GC for precancerous-conditions were calculated by Kaplan-Meier method and compared by Log-rank tests. Mixedeffects Cox regression models were used to detect potential factors for progression towards GC. A risk score was calculated as counts of selected factors. An independent cohort, including 26,586 participants was used for external validation. During a median follow-up of 6.25 years, CIRs of GC were 0.302%, 0.436%, and 4.756% for normal group, non-neoplastic（atrophic gastritis/intestinal metaplasia） and neoplastic lesions（low-grade/high-grade dysplasia）, respectively(Ptrend<0.001). Four predictors, including male, ≥60 years, smoking, and limited vegetable consumption, were selected for risk-stratification. High-risk patients（≥3 risk factors） with non-neoplastic lesions showed higher GC risks（adjusted HR=7.73, 95%CI: 4.29–13.92）, and their four-year CIR reached the one-year CIR of neoplastic lesions. Further categorizing non-neoplastic lesions by histological grade, both patients with moderate-to-severe lesions（aHR=3.07, 95%CI: 1.67–5.64） and high-risk patients with mild lesions（aHR=7.29, 95%CI: 3.58–14.86） showed higher risks. Consistent trends were observed in validation cohort. High-risk mild-precursor-lesion patients should receive surveillance within 3–5 years after baseline screening. Our study provides evidence on supplementing current guideline recommendations.

In this paper, uncertainty has been measured in the form of fuzziness which arises due to imprecise boundaries of fuzzy sets. Uncertainty caused due to human’s cognition can be decreased by the use of fuzzy soft sets. There are different approaches to deal with the measurement of uncertainty. The method we proposed uses fuzzified evidence theory to calculate total degree of fuzziness of the parameters. It consists of mainly four *** first part is to measure uncertainties of parameters using fuzzy soft sets and then to modulate the uncertainties calculated. Afterward, the appropriate basic probability assignments with respect to each parameter are produced. In the last, we use Dempster’s rule of combination to fuse independent parameters into integrated one. To validate the proposed method, we perform an experiment and compare our outputs with grey relational analysis method. Also,a medical diagnosis application in reference to COVID-19 has been given to show the effectiveness of advanced method by comparing with other method.

A robust statistical prediction model for late-summer heavy precipitation days in North China

Recently, heavy precipitation(HP) events have occurred frequently in North China(NC), causing devastating economic losses and human fatalities. However, the short-term climate prediction of HP is quite limited. Combining year-to-year increment(DY) method and sliding correlations, we developed a robust seasonal prediction model for late-summer HP days(HPDs) in NC during 1982–2022, utilizing three independent predictors—February sea surface temperature(SST) in the Indian Ocean(SST＿IO), February snow depth over North Asia(SDE＿NA), and May melted snow depth in NC(MSDE＿NC). The SST＿IO anomalies affect NC's precipitation through the Pacific-Japan pattern. The SDE＿NA anomalies are associated with East Asian anomalous anticyclone by southeastern propagation of Rossby wave at Eurasia. The MSDE＿NC anomalies are followed by vertical motion and moisture anomalies in situ and thereby cause precipitation anomalies. This prediction model can well simulate the variations of the HPDs, with a correlation coefficient(CC) of 0.81(0.65) between the observed and predicted HPDs＿DY(HPDs＿anomaly). The percentage with the same sign for 15 extreme HPDs＿anomaly years(PSSE) is 100%.Moreover, in the cross-validation test during 1982–2022, the PSSE for HPDs＿anomaly is as high as 100%, along with a low rootmean-square error of 1.14. For independent hindcasts during 2013–2022, the CC between the observed and predicted HPDs＿DY(HPDs＿anomaly) is 0.93(0.83), together with high Nash-Sutcliffe efficiency(0.82) and agreement index(0.89). Specifically, the predictions are broadly consistent with the observations for 2015, 2016, 2017, 2021, and 2022, reflecting excellent performance of this prediction model of HPDs in NC.

Whole grains of proso and barnyard millets were sequentially extracted using different solvents(hexane,chloroform,ethyl acetate,and methanol).Phytochemical analysis was performed qualitatively,and the total phenolic content in the extracts of proso and barnyard millets was *** and cardiac glycosides were identified in all solvent extracts of both *** and glycosides yielded negative results in all solvent extracts of both *** all the solvent extracts,methanol extracts of proso and barnyard millets showed the presence of major compounds such as flavonoids,terpenoids,amino acids,tannins,and phenolics *** maximum amount of phenols was found in methanolic extracts of proso and barnyard millets(0.669±0.003 and 0.625±0.003),followed by the chloroform extract of proso and barnyard millets(0.284±0.002 and 0.257±0.003).The minimum amount of phenolics was found in the acetone extract of proso and barnyard *** methanol extract of both millets showed the presence of major compounds with high phenolic content.

Membrane Fluidity: About the Origin of Autoimmunity

Fluidity of cellular membranes is essential for life. Two possibilities are known to keep human membranes fluid: unsaturated fatty acids and cholesterol. Whereas liver cells can synthesize cholesterol, unsaturated fatty acids are essential. Life style in Western civilization leads to deprivation of essential fatty acids, to elevated serum-cholesterol-levels and to autoimmunity. Here the hypothesis is presented, and explains the relationship: deprivation of essential fatty acids lead to imminent quasi-crystallization of the membrane. Serum cholesterol-levels are elevated. Incorporation of cholesterol into membranes enhancing fluidity again, is able to repair the effect. At saturation, repair fails. Quasi-crystallization occurs. Proteins tilt into another conformation. This has not been learned during the “self” recognition process of the immune system during the embryonic phase. Immune system attacks the new conformation as “non-self”, autoimmunity emerges.
