Blood metal concentrations and cardiac structure and function in total joint arthroplasty patients

BACKGROUND There is concern regarding potential long-term cardiotoxicity with systemic distribution of metals in total joint arthroplasty(TJA)*** To determine the association of commonly used implant metals with echocardiographic measures in TJA *** The study comprised 110 TJA patients who had a recent history of high chromium,cobalt or titanium *** underwent two-dimensional,three-dimensional,Doppler and speckle-strain transthoracic echocardiography and a blood draw to measure metal *** and sex-adjusted linear and logistic regression models were used to examine the association of metal concentrations(exposure)with echocardiographic measures(outcome).RESULTS Higher cobalt concentrations were associated with increased left ventricular end-diastolic volume(estimate 5.09;95%CI:0.02-10.17)as well as left atrial and right ventricular dilation,particularly in men but no changes in cardiac *** titanium concentrations were associated with a reduction in left ventricle global longitudinal strain(estimate 0.38;95%CI:0.70 to 0.06)and cardiac index(estimate 0.08;95%CI,-0.15 to-0.01).CONCLUSION Elevated cobalt and titanium concentrations may be associated with structural and functional cardiac changes in some *** studies are warranted to better understand the systemic effects of metals in TJA patients.

Effects of Immediate Dental Loading implant Therapy on Electroencephalography (EEG) and Stress

Purpose: implant therapy restores masticatory function by restoring lost tooth morphology. It has been shown that mastication contributes not only to food intake and digestion, but also to the improvement of overall health. However, there have been no studies on the effects of implant treatment on electroencephalography (EEG). In this study, we investigated the effects of restoration of masticatory function by implant treatment on EEG and stress. Methods: 13 subjects (6 males, 7 females, age 64.1 ± 5.8 years) who had lost masticatory function due to tooth loss and 11 healthy subjects (6 males, 5 females, age 47.6 ± 2.4 years) as a control group. EEG (θ, α, β waves, α/β ratio) and salivary cortisol were measured before immediate dental implant treatment and every month of treatment for 6 months. EEG (θ, α, β waves, α/β ratio) was measured with a simple electroencephalograph miniature DAQ terminal (Intercross-410, Intercross Co., Ltd., Japan) in a resting closed-eye condition, and salivary cortisol was measured using an ELISA kit. Results: Compared to the control group, the appearance of θ and α waves were significantly decreased and β waves were increased, and α/β ratio was significantly decreased. The cortisol level of the subject group was significantly higher compared with the control group. With the course of implant treatment, the appearance of θ and α waves of the subject group increased, while β waves decreased. However, no significant difference was observed. The α/β ratio of the subject group increased from the first month after implant treatment and increased significantly after 5 and 6 months (0 vs. 5 months: p implant treatment and significantly decreased after 3 or 4 months (0 vs. 3 months: p < 0.05, 0 vs. 4 months: p < 0.01). Results of transvenous lead extraction of coronary sinus leads in patients with cardiac resynchronization therapy

Background The need for transvenous lead extraction procedures of coronary sinus （CS） leads is increasing due to rising numbers of implanted cardiac resynchronization therapy devices during the past decade. Methods From January 2009 to June 2013, 27 CS leads were scheduled for extraction in 27 patients （mean age （63.1±14.6） years）. Indications for lead extraction were infection in 13 and lead dysfunction in 14 cases. Isolated extraction of CS leads was performed in eight, extraction of multiple leads in 19 cases. Among leads with an implant time of 〉12 months （n=19） mean implant duration （MID） was （46.4±15.2）（12-76） months. Groups were formed depending on infectious or non-infectious indications （INF vs. Non-INF）, and the use or non-use of extraction tools （ET1 vs. ET0）. Results Among patients with an implant duration of 〉12 months, complete procedural success was 94.7% and clinical success 100%. Operative mortality was zero. In the INF versus NON-INF groups complete procedural success （100% vs. 91.7%, P=0.43）, mean number of required extraction tools （0.7 （0-2） vs. 0.9 （0-3）, P=0.65） and MID （49.1±15.0 vs. 44.7±15.8, P=0.83） did not differ significantly. Comparing the groups ET1 and ET0 showed no significant differences in complications （n=l vs. n=l, P=-0.81） and MID （47.0±17.5 vs. 45.5±12.6, P=0.71）. Conclusions In specialized centers transvenous lead extraction of coronary sinus leads with a mean implant duration of almost four years can be performed safely and effectively. Neither non-infectious indications nor the use of extraction tools negatively affected the outcome of the procedure.

A Neural Excitability Based Coding Strategy for Cochlear implants

A novel cochlear implant coding strategy based on the neural excitability has been developed and implemented using Matlab/Simulink. Unlike present day coding strategies, the Excitability Controlled Coding (ECC) strategy uses a model of the excitability state of the target neural population to determine its stimulus selection, with the aim of more efficient stimulation as well as reduced channel interaction. Central to the ECC algorithm is an excitability state model, which takes into account the supposed refractory behaviour of the stimulated neural populations. The excitability state, used to weight the input signal for selecting the stimuli, is estimated and updated after the presentation of each stimulus, and used iteratively in selecting the next stimulus. Additionally, ECC regulates the frequency of stimulation on a given channel as a function of the corresponding input stimulus intensity. Details of the model, implementation and results of benchtop plus subjective tests are presented and discussed. Compared to the Advanced Combination Encoder (ACE) strategy, ECC produces a better spectral representation of an input signal, and can potentially reduce channel interactions. Pilot test results from 4 CI recipients suggest that ECC may have some advantage over ACE for complex situations such as speech in noise, possibly due to ECC’s ability to present more of the input spectral contents compared to ACE, which is restricted to a fixed number of maxima. The ECC strategy represents a neuro-physiological approach that could potentially improve the perception of more complex sound patterns with cochlear implants.
