Pseudomembranous colitis associated with a triple therapy for Helicobacter pylori eradication

Helicobacter pylori(H.pylori)is one of the most common chronic bacterial infections in humans,affecting half of world’s population.Therapy for H.pylori infection has proven to be both effective and safe.The oneweek triple therapy including proton pump inhibitor,clarithromycin,and amoxicillin or metronidazole is still recommended as a first-line treatment to eradicate H.pylori infection in countries with low clarithromycin resistance.Generally,this therapy is well-tolerated,with only a few and usually minor side effects.However,rare but severe adverse effects such as pseudomembranous colitis have been reported,Clostridium difficile(C.difficile)infection being the main causative factor in all cases.We report the cases of two women who developed pseudomembranous colitis after a 1-wk triple therapy consisting of pantoprazole 20 mg bid,clarithromycin 500 mg bid,and amoxicillin 1 g bid to eradicate H.pylori infection.A limited colonoscopy showed typical appearance of pseudomembranous colitis,and the stool test for C.difficile toxins was positive.Rapid resolution of symptoms and negative C.difficile toxins were obtained in both patients with oral vancomycin.No relapse occurred during a four and eleven-month,respectively,follow up.These cases suggest that physicians should have a high index of suspicion for pseudomembranous colitis when evaluate patients with diarrhea following H.pylori eradication therapy.

Lithium niobate waveguides with high-index contrast and preserved nonlinearity fabricated by a high vacuum vapor-phase proton exchange

Highly confining waveguides(Δne>0.1)without a degraded nonlinear coefficient and low propagation losses have been fabricated in lithium niobate(LN)by a new process that we called high vacuum vapor-phase proton exchange(Hi Vac-VPE).Index contrast,index profile,nonlinearity,and crystallographic phases are carefully investigated.Original analysis of index profiles indicates that the waveguides contain sub-layers whose depths depend on the exchange durations.Propagation behavior,propagation losses,and second-harmonic generation response of Hi Vac-VPE channel waveguides are investigated at telecom wavelength.The results recommend Hi Vac-VPE as a very promising technique for fabricating efficient nonlinear photonic integrated circuits in LN crystals.

A Short Vector Solution of the Foucault Pendulum Problem

The paper studies the motion of the Foucault Pendulum in a rotating non-inertial reference frame and provides a closed form vector solution determined by vector and matrix calculus. The solution is determined through vector and matrix calculus in both cases, for both forms of the law of motion (for the Foucault Pendulum Problem and its “Reduced Form”). A complex vector which transforms the motion equation in a first order differential equation with constant coefficients is used. Also, a novel kinematic interpretation of the Foucault Pendulum motion is given.

We use the theory based on a gravitational gauge group （Wu＇s model） to obtain a spherical symmetric solution of the field equations for the gravitational potential on a Minkowski spacetime. The gauge group, the gauge covariant derivative, the strength tensor of the gauge feld, the gauge invariant Lagrangean with the cosmological constant, the field equations of the gauge potentiaIs with a gravitational energy-momentum tensor as well as with a tensor of the field of a point like source are determined. Finally, a Reissner-Nordstrom-de Sitter-type metric on the gauge group space is obtained.

The effects of two chlorinated chiral stationary phases, namely, Lux Cellulose-2 and Lux i-Cellulose-5, flow-rate, percentage of co-solvent and chemical structures of the compounds on retention and resolution were studied within this article. In this work a backpressure of 150 bar, a temperature of 40 ℃ and 10% of methanol as co-solvent were chosen as operating conditions. The optimum flow-rate was 2 mL/min. The percentage of co-solvent was studied between 7.5% and 15%.We have observed that 15% of methanol gave the best results for most of the compounds. For all the derivatives, the Lux Cellulose-2 provided better resolutions going from 1.50 to 3.59 compared with Lux i-Cellulose-5.

The Spiritual Experience as Scientific Experiment： How Science Can Meet Religion

My paper aims to discuss the approach between the mystic"s experience and the experiment of the scientist. The mystic experience is closer to what is proper to the scientific experiment: both are, finally, ways of tryout. This also means that both are ways to come closer to a deeper understanding of Reality. Raising the problem of the relation between religion and science requires a series of precautions, especially related to the underlying meanings of what we call "religion" or "science". What is meant by religion and religious The terms are too vague, and on the other hand, the differences among religions are so important that it is almost impossible to use the term religion without an abstract meaning. Even the overall invocation of Christianity is improper, as the report of Eastern Christianity to the Western, one reveals different perspectives on some issues that determine the relationship with science. There must be also taken into account the history of the religious doctrines after Enlightenment, as they underwent secular influences that sometimes left tracks in their discourse, the most aggressive factor being the ideological one. And when speaking about the possibility of religion getting open to science it is necessary to specify what type of religious discourse is concerned. Since a generic report of religion to science is slippery, we should rather count on the identification of those types of religious experience and of those religious horizons that can offer a real openness to science. These statements are also true for science: for such a dialogue, there is need of identifying those aspects of science that are not contaminated by ideology or by assumptions alien to its experimental nature.

Modelling Mechanical Properties in Native and Biomimetically Formed Vascular Grafts

The paper presents a detailed analysis of experimental data in order to characterize the elastic properties of arteries.Such analysis would provide a good basis for evaluation of biomimetic vascular grafts.Since the latter needs to exhibit similar properties of native tissue,it is important to accurately characterize the biomimetic sample in a large range of applied stresses. The stress-strain properties vary according to the specific pathology(e.***.arteriosclerosis,aneurism)and the tissue graft must be chosen correctly.Two models are proposed in this paper on the stress-strain characteristics.An extension for frequency-domain analysis is provided for one of the models.The comparison between vascular grafts and native tissue for carotid and thoracic arteries in pigs are in good agreement with results from literature.The proposed experimental method offers suitable parameters for identifying models which characterize both elasticity and stiffness properties of the analyzed tissues(stress-strain).The proposed models show good performance in characterizing the intrinsic material properties.

Rationale and Objectives: Accurate diagnosis and staging of cervical precancers is essential for practical medicine in determining the extent of the lesion extension and determines the most correct and effective therapeutic approach. For accurate diagnosis and staging of cervical precancers, we aim to create a diagnostic method optimized by artificial intelligence (AI) algorithms and validated by achieving accurate and favorable results by conducting a clinical trial, during which we will use the diagnostic method optimized by artificial intelligence (AI) algorithms, to avoid errors, to increase the understanding on interpretation of colposcopy images and improve therapeutic planning. Materials and Methods: The optimization of the method will consist in the development and formation of artificial intelligence models, using complicated convolutional neural networks (CNN) to identify precancers and cancers on colposcopic images. We will use topologies that have performed well in similar image recognition projects, such as Visual Geometry Group Network (VGG16), Inception deep neural network with an architectural design that consists of repeating components referred to as Inception modules (Inception), deeply separable convolutions that significantly reduce the number of parameters (MobileNet) that is a class of Convolutional Neural Network (CNN), Return of investment for machine Learning (ROI), Fully Convolutional Network (U-Net) and Overcomplete Convolutional Network Kite-Net (KiU-Net). Validation of the diagnostic method, optimized by algorithm of artificial intelligence will consist of achieving accurate results on diagnosis and staging of cervical precancers by conducting a randomized, controlled clinical trial, for a period of 17 months. Deep learning (DL) has seen an exponential development in recent years, with major impact in many medical fields, especially in the field of medical image. The purpose of the work converges in determining the importance of each component, describing the specificity and correlations of these elements involved in achieving the precision of interpretation of medical images using DL. The major contribution of this work is primarily to the updated characterisation of the characteristics of the constituent elements of the deep learning process, scientific data, methods of knowledge incorporation, DL models according to the objectives for which they were designed and the presentation of medical applications in accordance with these tasks. Secondly, it describes the specific correlations between the quality, type and volume of data, the deep learning patterns used in the interpretation of diagnostic medical images and their applications in medicine. Finally presents problems and directions of future research. Data quality and volume, annotations and labels, identification and automatic extraction of specific medical terms can help deep learning models perform image analysis tasks. Moreover, the development of models capable of extracting unattended features and easily incorporated into the architecture of DL networks and the development of techniques to search for a certain network architecture according to the objectives set lead to performance in the interpretation of medical images.
