目的比较子宫颈癌筛查中传统子宫颈涂片、单层细胞学,以及人乳头状瘤病毒检测的敏感性、特异性和观察者间的可信性。设计对同时进行了此3项检查并有参考标准(阴道镜和组织学)者进行横断面研究。研究单位法国的公立大学和私人诊所,本研究完全独立于病例提供者。研究对象因先前检出的细胞学异常而进行阴道镜检查的828名妇女和参加常规涂片检查的1757名妇女。主要结果判定指标临床读片和最优化解释(为取得一致所进行的双盲读片)。根据诊断异常的不同阈值计算出敏感性、特异性和加权κ值。结果根据Bethesda系统,传统子宫颈涂片检查得到的结果更为满意(91％比87％),比单层细胞学更可靠(加权κ值0.70比0.57),其敏感性和特异性更高。这些结果适用于临床读片和最优化解释,区分高级别和低级别病变,以及有低和高异常发生率的人群。人乳头状瘤病毒检测与单层细胞学检查同时进行,不论是系统性的还是只对于意义不明的非典型细胞,并不比传统涂片检查优越。结论在子宫颈癌筛选中,单层细胞学检查不如传统子宫颈涂片检查可靠,且更容易出现假阳性和假阴性结果。

炎症性肠病(IBD)包括克罗恩病(CD)、溃疡性结肠炎(uC)和未分型IBD(IBDu)。IBD的确切诊断需综合病史、体检、实验室检查、内镜以及影像学的检查,并严格排除肠道感染、过敏性疾病或原发性免疫缺陷病(PID)。近年对儿童期起病的IBD(PIBD)的研究高度提示3种PIBD的亚型均存在不典型的表型。本标准对2005年波尔图标准进行改良,利用循证方法,整合最新证据,包括诊断方法、以巴黎分型为基础的PIBD亚型的定义以及诊断误区,为新一代的PIBD提供优质的个体化管理以及可靠的诊断、评估和预后。

[背景]人类在生命早期的发育期间可能特别容易受到环境暴露的影响。有关这一主题的人体研究一般集中在单一暴露与健康效应之间的关系。而"暴露组"的概念涵盖了从受孕开始的的全部暴露,完善了基因组。[目的]人类生命早期暴露组(HELIX)项目是一个新的合作研究项目,目的是为了采用新的暴露评估和生物标志分析方法来描述生命早期多种环境因素的暴露,并将这些与生物标志组学和儿童健康结局相关联,从而刻画"生命早期暴露组"。本文描述了该项目的总体设计。[方法]HELIX将利用欧洲现有的6个出生队列研究来估计产前、产后暴露的一系列化学和物理暴露。建立全部队列中总共32 000对母亲和儿童的暴露模型,并在一个包含1 200对母亲和儿童的子队列中测量生物标志。嵌套重复采样的定组研究(n=150)将收集生物标志的变化数据,利用智能手机来评估流动性和体力活动,并监测个体暴露。采用组学技术确定与暴露相关的分子学特征(代谢组、蛋白质组、转录组和表观基因组)。对于多次暴露,采用统计方法估计胎儿和儿童的成长、肥胖、神经发育和呼吸系统结局中的暴露-效应关系。项目还将进行一项健康效应评估测试,以评价组合暴露的风险和收益。[结论]HELIX是描述欧洲人群生命早期暴露组并解开它与组学标志物和儿童健康之间关联的首次尝试之一。作为对暴露组学这一概念的验证,该项目向生命过程中暴露组迈出了重要的第一步。

Background:Liver biopsy for the diagnosis of non-alcoholic steatohepatitis(NASH)is limited by its inherent invasiveness and possible sampling *** studies have shown that cytokeratin-18(CK-18)concentrations may be useful in diagnosing NASH,but results across studies have been *** aimed to identify the utility of CK-18 M30 concentrations as an alternative to liver biopsy for non-invasive identification of ***:Individual data were collected from 14 registry centers on patients with biopsy-proven non-alcoholic fatty liver disease(NAFLD),and in all patients,circulating CK-18 M30 levels were *** with a NAFLD activity score(NAS)≥5 with a score of≥1 for each of steatosis,ballooning,and lobular inflammation were diagnosed as having definite NASH;individuals with a NAS≤2 and no fibrosis were diagnosed as having non-alcoholic fatty liver(NAFL).Results:A total of 2571 participants were screened,and 1008(153 with NAFL and 855 with NASH)were finally *** CK-18 M30 levels were higher in patients with NASH than in those with NAFL(mean difference 177 u/L;standardized mean difference[SMD]:0.87[0.69–1.04]).There was an interaction between CK-18 M30 levels and serum alanine aminotransferase,body mass index(BMI),and hypertension(Punder the receiver operating characteristics(AuROC)for NASH was 0.750(95%confidence intervals:0.714–0.787),and CK-18 M30 at Youden’s index maximum was 275.7 u/*** sensitivity(55%[52%–59%])and positive predictive value(59%)were not ***:This large multicenter registry study shows that CK-18 M30 measurement in isolation is of limited value for non-invasively diagnosing NASH.

背景：以前的血管内超声（intravascular ultrasound，IVuS）试验证实，他汀治疗可减缓或阻止动脉粥样硬化的进展，但是迄今尚无应用动脉粥样斑块体积百分比（percent atheroma volume，PAV）证实粥样硬化消退的确切证据。PAV是最严格的评价病变进展和消退的IVuS测量指标。目标：评价极高强度他汀治疗是否能逆转IVuS确定的冠状动脉粥样硬化。设计和地点：于美国、加拿大、欧洲和澳大利亚53个社区和3级保健中心进行前瞻性开标盲法终点试验（A Study to Evaluate the Effect of Rosuvastatin on Intravascular ultrasound-Derived Coronary Atheroma Burden，ASTEROID）。应用马达驱动回撤IVuS评价基线和治疗24个月时冠状动脉粥样斑块负荷。每对基线和随访IVuS测量结果均进行盲法分析。病例：从2002年11月到2003年10月，507例患者有基线IVuS检查结果，并接受至少1个剂量的研究药物。在24个月后，349例患者具有可用于评估的系列IVuS检查结果。干预：所有患者均接受瑞舒伐他汀40ms／d强化治疗。主要观测指标：预先设定了两个一级疗效指标：PAV变化和基线最严重病变10min节段动脉粥样斑块体积变化。二级疗效指标为整个动脉标准化总斑块体积的变化。结果：平均（SD）LDL—C水平由基线时的130．4（34．3）ms／dL降至60．8（20．0）ms／扎，平均下降了53．2％（P〈0．001）。平均（SD）HDL-C水平从基线时的43．1（11．1）ms／dL升至49．0（12．6）ms／dL，平均增加了14．7％（P〈0．001）。整个血管PAV平均（SD）变化为-0．98％（3．15％），中位数为-0．79％（97．5％CI，-1．21％～-0．53％）（与基线比较，P〈0．001）。最严重病变10min节段斑块体积平均（SD）变化为-6．1（10．1）mm^3，中位数为-5．6mm^3（97．5％CI，-6．8～-4．0mm^3）（与基线比较，P〈0．001）。总斑块体积变化中位数降低了6．8％，平均减少了-14．7（25．7）mm^3，中位数为-12．5mm^3（95％CI，-15．1～-10．5mm^3）（与基线比较，P〈0．001）。不良事件少见，与其他他汀试验相似。结论：应用瑞舒伐他汀40ms／d进行极高强度他汀治疗可使LDL-C平均水平达到60．8ms／dL，使HDL—C增加14．7％。这导致所有3个预先设定的IVuS斑块负荷指标均显示动脉粥样硬化消退。因此，将LDL-C降至低于目前指南规定的水平，同时显著提高HDL-C，可以使冠心病患者动脉粥样硬化斑块消退。这些变化对临床预后的影响尚需进一步研究确定。
