AIM:To assess,in a routine practice setting,the sus-tained virologic response(SVR) to telaprevir(TPV) or boceprevir(BOC) in hepatitis C virus(HCV) nullresponders or relapsers with severe liver ***:One hundred twenty-five patients were treated prospectively for 48 wk with TPV or BOC + pegylated-interferon(peg-INF) α2a + ribavirin(PR) according to standard treatment schedules without *** patients were treated in routine practice settings in 10 public or private health care centers,and the data were prospectively *** patients with severe liver fibrosis(Metavir scores of F3 or F4 upon liver biopsy or liver stiffness assessed by elastography),genotype 1 HCV and who were null-responders or relapsers to prior PR combination therapy were included in this ***:The Metavir fibrosis scores were F3 in 35(28%) and F4 in 90(72%) of the *** total,62.9% of the patients were null-responders and 37.1% relapsers to the previous PR *** overall SVR rate at 24 wk post-treatment withdrawal was 59.8%.The SVR was 65.9% in the TPV group and 44.1% in the BOC *** predictive factors of an SVR included a response to previous treatment,relapsers vs null-responders [OR = 3.9;(1.4,10.6),P = 0.0084],a rapid virological response(RVR) [OR 6.9(2.6,18.2),P = 0.001] and liver stiffness lower than 21.3 kPa [OR = 8.2(2.3,29.5),P = 0.001].During treatment,63 patients(50.8%) had at least one severe adverse event(SAE) of grade 3 or 4.A multivariate analysis identified two factors associated with SAEs:female gender [OR = 2.4(1.1,5.6),P = 0.037] and a platelet count below 150 × 103/ mm3 [OR = 5.3(2.3,12.4),P ≤ 0.001].CONCLuSION:More than half of these difficult-to-treat patients achieved an SVR and had SAEs in an actual practice *** SVR rate was influenced by the response to previous PR treatment,the RVR and liver stiffness.

Construction and update of an Online Ensemble Score Involving Linear Discriminant Analysis and Logistic Regression

The present aim is to update, upon arrival of new learning data, the parameters of a score constructed with an ensemble method involving linear discriminant analysis and logistic regression in an online setting, without the need to store all of the previously obtained data. Poisson bootstrap and stochastic approximation processes were used with online standardized data to avoid numerical explosions, the convergence of which has been established theoretically. This empirical convergence of online ensemble scores to a reference “batch” score was studied on five different datasets from which data streams were simulated, comparing six different processes to construct the online scores. For each score, 50 replications using a total of 10N observations (N being the size of the dataset) were performed to assess the convergence and the stability of the method, computing the mean and standard deviation of a convergence criterion. A complementary study using 100N observations was also performed. All tested processes on all datasets converged after N iterations, except for one process on one dataset. The best processes were averaged processes using online standardized data and a piecewise constant step-size.

Vegetables are very important sources of protein and minerals. Some of them even have medicinal properties recognized traditionally. Despite the large number of studies carried out on various vegetables and vegetable crops, very few have scientifically explored the usefulness of S. macrocarpon. This study identified the main groups of chemicals and mineral elements to explain any medicinal or nutritional value. It has also identified some toxic elements contained in this vegetable. Phytochemical screening was carried out on the leaves and fruits of S. macrocarpon. Some mineral elements were determined by Atomic Absorption Spectrophotometry (sodium, potassium, calcium, magnesium) while protein, phosphorous, iron, copper, zinc and toxic metals (lead, cadmium) were determined by Molecular Absorption Spectrophotometry. Fat, ash, moisture and vitamins were sought. The study showed that the leaves of S. macrocarpon were more nutritious than fruits (P macrocarpon require that the consumption of vegetables should be as varied as possible and that the fruit may be consumed with caution. In addition, constraints about vegetable cultivation in Cotonou could lead to research findings that could help provide techniques for producing healthy *** A and K1 were found in both parts of this vegetable very rich in water while vitamin E has not been detected. S. macrocarpon also contains lipids at various levels.

Objective. To assess the frequency of deprivation in pregnant women and estimate the associated medical risks. Material and methods. A retrospective study of the hospital’ s computerised data (6149 women) and two prospective studies with health staff (n=534) and social workers (n=85) in two maternity hospitals. Results. In the retrospective study, 17.5% of mothers were deprived. Deprivation was associated both with more difficulties during pregnancy and the perinatal period and with longer hospital stays. The prospective studies confirmed the prevalence of deprivation and gave a more detailed idea of the associated social and demographic characteristics. Deprivation was related with a combination of risk factors with a particularly important influence of economic risk. A quarter of deprived mothers had unwanted pregnancies and according to the social workers approximately 10% of them were not in a position to take care of their baby. Conclusion. This study emphasizes the importance of deprivation as a perinatal medical risk factor. Screening should take place in early pregnancy in order to find the best way to help such families.

SPINK5(serine protease inhibitor Kazal-type 5),encoding the protease inhibitor LEKTI(lympho-epithelia lKazal-type related inhibitor),is the defective gene in Netherton syndrome(NS)-,a severe inherited keratinizing *** have recently demonstrated epidermal protease hyperactivity in Spink5-/-mice resulting in desmosomal protein ***,we investigated the molecular mechanism underlying the epidermal defect in 15 patients with *** demonstrated that,in a majority of patients,desmoglein 1(Dsg1)and desmocollin 1(Dsc1)were dramatically reduced in the upper most living layers of the *** defects were associated with premature degradation of *** corneum tryptic enzyme(SCTE)-like and stratum corneum chymotryptic enzyme(SCCE)-like activities were increased,suggesting that these proteases participate in the premature degradation of corneodesmosomal *** and SCCE expression was extended to the cell layers where Dsg1 and Dsc1 immunostaining was *** contrast,a subset of six patients with normal epidermal protease activity or residual LEKTI expression displayed apparently normal cadherin expression and less severe disease *** suggests a degree of correlation between cadherin degradation and clinical *** work further supports the implication of premature corneodesmosomal cadherin degradation in the pathogenesis of NS and provides evidence for additional factors playing a role in disease expression.

目的评价氧合过度所致的BOLD信号强度的改变是否与大鼠宫内生长受限(IuGR)有关。方法在妊娠第16天,结扎左侧子宫血管根部,建立妊娠大鼠的IuGR模型。在第19天,应用1.5TMR设备以平衡稳态自由进动(balanced-SSFP)序列对大鼠模型进行BOLDMR成像。比较母体肝脏、对照组及生长受限胎儿胎盘单元(FPu)在氧合过度发生前后的信号强度(SI)。结果 18只大鼠中,母体氧合过度导致所有感兴趣区SI明显增加(PuGR组中,信号强度增加的幅度明显较低:胎盘(n=36)为(6.5±4)%,胎儿(n=34)为(7±5.5)%;PuGR模型中的信号强度改变。这种成像技术可提供一种有用的辅助方法,用以早期诊断、评估和治疗人类IuGR。要点①宫内生长受限是围产期发病和死亡的重要原因。②BOLDMRI可以无创评估胎儿胎盘对母体氧合过度的反应。③IuGR大鼠的胎儿胎盘对母体氧合过度的反应发生改变。④功能MRI有助于评估人IuGR。
