Menopause is characterized by various physical,mental and emotional symptoms.ERr 731®is a standardized extract from Rheum rhaponticum root and has been clinically studied for its role in reducing menopausal symptoms.The current systematic review and meta-analysis aimed to evaluate the efficacy of ERr 731®supplementation in alleviating the severity of menopausal symptoms.In this review,we searched across three online databases up to March 2023,evaluated the quality of the included studies by the Physiotherapy Evidence Database scale,and assessed the risk of bias by the Cochrane Risk of Bias tool.We then performed a metaanalysis using RevMan software to estimate the pooled mean difference(MD).The study protocol was registered in the Prospective Register of Systematic Reviews(CRD42023416808).After screening and evaluation,we included four high-quality studies(a total of 390 participants;the ERr 731®group:193 participants;the control group:197 participants)in the meta-analysis.The results showed that ERr 731®supplementation significantly reduced the Menopause Rating Scale score(MD:–15.12;P<0.001),compared with control therapy.Sensitivity analysis revealed no effect of individual studies on the overall pooled estimate or overall observed heterogeneity.The current review provides evidence that ERr 731®supplementation is effective in reducing menopause symptoms.Potential bias and high heterogeneity in the results warrant further clinical studies.

Pile foundations are still the preferred foundation system for high-rise structures in earthquake-prone regions.Pile foundations have experienced failures in past earthquakes due to liquefaction.Research on pile foundations in liquefiable soils has primarily focused on the pile foundation behavior in two or three-layered soil profiles.However,in natural occurrence,it may occur in alternative layers of liquefiable and non-liquefiable soil.However,the experimental and/or numerical studies on the layered effect on pile foundations have not been widely addressed in the literature.Most of the design codes across the world do not explicitly mention the effect of sandwiched non-liquefiable soil layers on the pile response.In the present study,the behavior of an end-bearing pile in layered liquefiable and non-liquefiable soil deposit is studied numerically.This study found that the kinematic bending moment is higher and governs the design when the effect of the sandwiched non-liquefied layer is considered in the analysis as opposed to when its effect is ignored.Therefore,ignoring the effect of the sandwiched non-liquefied layer in a liquefiable soil deposit might be a nonconservative design approach.

Bethanechol Remains the Preferred Drug in Suspected Underactive Bladder: Findings from Nation-Wide Prescription Analysis

Background: Bethanechol chloride, a cholinergic agonist, is often prescribed in suspected underactive bladders to improve detrusor contractility, despite its limited clinical efficacy. We investigated the usage pattern of bethanechol in actual practice with the understanding that it would enable the physicians to make an informed decision on the coherent use of bethanechol. Methods: A nation-wide survey was carried out to obtain the responses of the urologists. Out of the 755 urologists approached, 630 survey responses were considered for analysis. Results: Usage of bethanechol was advocated as very common [318 (50.48%)], common [200 (31.75%)], not so common [107 (16.98%)], and rare [5 (0.79%)] in postoperative urinary retention, where it was preferred either exclusively [255 (40.48%)] or along with alpha blockers [247 (39.21%)]. Predilection to use alpha-blocker [247 (39.21%)], alpha-blocker plus naloxone [4 (0.64%)], naloxone [1 (0.16%)], alpha-blocker plus bethanechol plus naloxone [1 (0.16%)] was also observed. It was also preferred individually in pathologies causing urinary retention such as benign prostatic hyperplasia [125 (19.84%)], diabetic neuropathy [82 (13.02%)], neurological diseases [69 (10.95%)], senile bladder [14 (2.22%)], drugs [13 (2.06%)], and infective/inflammatory conditions [6 (0.95%)]. Other [321 (50.95%)] physicians opted to prescribe bethanechol in two or more of the enumerated indications. Bethanechol was prescribed orally as 25 mg thrice daily [441 (70.00%)], 50 mg thrice daily [86 (13.65%)], 25 mg four times daily [59 (9.37%)], and many “strongly agree” and “agree” that its sustained release formulation may offer better treatment compliance [565 (89.68%)], safety [548 (86.99%)], and efficacy [544 (86.35%)]. Conclusion: Bethanechol was the most prescribed drug for the management of postoperative urinary retention and other pathologies suspected to cause underactive bladder.

Collecting umbilical cord stem cells is widely practiced due to its numerous benefits. Over the past decade, umbilical cord stem cells (UCSCs) have shown effectiveness in treating various conditions, such as bone pathologies, neuropsychiatry disorders, hereditary diseases, and metabolic disorders. However, factors like immunization affect the quantity and quality of cord harvesting. Studies suggest that antibodies from the mother pass through the umbilical cord to protect the infant against infections. Cleaning the umbilical cord before stem cell extraction is crucial to maintain sterility and cell integrity. Vaccinating a female donor, including for COVID-19, typically does not directly affect the stem cells. Although vaccines aim to trigger an immunological response, they generally do not affect the donor’s stem cells.

Gliomas are the most aggressive brain tumors caused by the abnormal growth of brain tissues.The life expectancy of patients diagnosed with gliomas decreases exponentially.Most gliomas are diagnosed in later stages,resulting in imminent death.On average,patients do not survive 14 months after diagnosis.The only way to minimize the impact of this inevitable disease is through early diagnosis.The Magnetic Resonance Imaging(MRI)scans,because of their better tissue contrast,are most frequently used to assess the brain tissues.The manual classification of MRI scans takes a reasonable amount of time to classify brain tumors.Besides this,dealing with MRI scans manually is also cumbersome,thus affects the classification accuracy.To eradicate this problem,researchers have come up with automatic and semiautomatic methods that help in the automation of brain tumor classification task.Although,many techniques have been devised to address this issue,the existing methods still struggle to characterize the enhancing region.This is because of low variance in enhancing region which give poor contrast in MRI scans.In this study,we propose a novel deep learning based method consisting of a series of steps,namely:data pre-processing,patch extraction,patch pre-processing,and a deep learning model with tuned hyper-parameters to classify all types of gliomas with a focus on enhancing region.Our trained model achieved better results for all glioma classes including the enhancing region.The improved performance of our technique can be attributed to several factors.Firstly,the non-local mean filter in the pre-processing step,improved the image detail while removing irrelevant noise.Secondly,the architecture we employ can capture the non-linearity of all classes including the enhancing region.Overall,the segmentation scores achieved on the Dice Similarity Coefficient(DSC)metric for normal,necrosis,edema,enhancing and non-enhancing tumor classes are 0.95,0.97,0.91,0.93,0.95;respectively.

The cloud service level agreement(SLA)manage the relationship between service providers and consumers in cloud computing.SLA is an integral and critical part of modern era IT vendors and communication contracts.Due to low cost and flexibility more and more consumers delegate their tasks to cloud providers,the SLA emerges as a key aspect between the consumers and providers.Continuous monitoring of Quality of Service(QoS)attributes is required to implement SLAs because of the complex nature of cloud communication.Many other factors,such as user reliability,satisfaction,and penalty on violations are also taken into account.Currently,there is no such policy of cloud SLA monitoring to minimize SLA violations.In this work,we have proposed a cloud SLA monitoring policy by dividing a monitoring session into two parts,for critical and non-critical parameters.The critical and non-critical parameters will be decided on the interest of the consumer during SLA negotiation.This will help to shape a new comprehensive SLA based Proactive Resource Allocation Approach(RPAA)which will monitor SLA at runtime,analyze the SLA parameters and try to find the possibility of SLA violations.We also have implemented an adaptive system for allocating cloud IT resources based on SLA violations and detection.We have defined two main components of SLA-PRAA i.e.,(a)Handler and(b)Accounting and Billing Manager.We have also described the function of both components through algorithms.The experimental results validate the performance of our proposed method in comparison with state-of-the-art cloud SLA policies.

Study on Chrome-free Purging Plugs for Steel Ladles

Purging plug refractories in China typically contain around 3 mass% of super-fine chromium oxide in the matrix in order to improve the performance of the purging plugs, primarily, slag corrosion and wear resistance. Alternatives to chromium oxide containing refractories have gained interest due to health concerns related to the formation of soluble chromium compounds over long storage periods of refractory wastes. Super-ground reactive alumina can replace chromium oxide in purging plug refractories and this paper discussed the new reactive alumina E-SY 88 in comparison to chromium oxide in a typical purging plug castable. The mixing behaviour, wet castable properties, as well as cured, dried, and fired properties at different temperatures up to 1 600 ℃ were compared. In addition, the hot modulus of rupture, creep behaviour, thermal shock resistance and slag corrosion resistance were tested. The microstructure after slag corrosion was investigated by SEM. The results prove that E-SY 88 is an economically viable technical alternative to chromium oxide in purging plug refractories.

Hybrid Optimization Based PID Controller Design for Unstable System

PID controllers play an important function in determining tuning para-meters in any process sector to deliver optimal and resilient performance for non-linear,stable and unstable processes.The effectiveness of the presented hybrid metaheuristic algorithms for a class of time-delayed unstable systems is described in this study when applicable to the problems of PID controller and Smith PID controller.The Direct Multi Search(DMS)algorithm is utilised in this research to combine the local search ability of global heuristic algorithms to tune a PID controller for a time-delayed unstable process model.A Metaheuristics Algorithm such as,SA(Simulated Annealing),MBBO(Modiﬁed Biogeography Based Opti-mization),BBO(Biogeography Based Optimization),PBIL(Population Based Incremental Learning),ES(Evolution Strategy),StudGA(Stud Genetic Algo-rithms),PSO(Particle Swarm Optimization),StudGA(Stud Genetic Algorithms),ES(Evolution Strategy),PSO(Particle Swarm Optimization)and ACO(Ant Col-ony Optimization)are used to tune the PID controller and Smith predictor design.The effectiveness of the suggested algorithms DMS-SA,DMS-BBO,DMS-MBBO,DMS-PBIL,DMS-StudGA,DMS-ES,DMS-ACO,and DMS-PSO for a class of dead-time structures employing PID controller and Smith predictor design controllers is illustrated using unit step set point response.When compared to other optimizations,the suggested hybrid metaheuristics approach improves the time response analysis when extended to the problem of smith predictor and PID controller designed tuning.

Comparative efficacy of secukinumab against adalimumab and infliximab in patients with moderate-to-severe plaque psoriasis

Background: Psoriasis is a common, chronic, immune-mediated inflammatory skin disease with increased epidermal proliferation. The objective of this review was to systematically identify the evidence and perform a network meta-analysis (NMA) to estimate the relative efficacy of secukinumab (SEC) against adalimumab (ADA) and infliximab (INF) for the treatment of moderate-to-severe plaque psoriasis.Methods: A systematic literature review (SLR) was conducted according to a pre-specified protocol to identify relevant studies. Initially, the databases were searched from database inception till June 2013, and the SLR was updated in April 2020. The eligibility criteria included adult patients (≥18 years old) with moderate-to-severe plaque psoriasis, and the SLR included randomized controlled trials (RCTs). The comparators of interest were SEC, ADA, INF, and placebo (PLA), while outcomes of interest were Psoriasis Area and Severity Index (PASI) (50, 75, and 90) at weeks 12, 16, and 24. A Bayesian NMA for PASI was utilized with a framework that evaluated the probability of PASI responses in different categories of PASI thresholds within a single model.Results: A total of 23 RCTs that assessed the efficacy of SEC, ADA, and INF in patients with moderate-to-severe plaque psoriasis were identified. At 12 weeks, SEC was associated with a significantly better response compared with PLA and ADA for PASI 75 and 90, while response results were comparable against INF. At 12 weeks, risk ratio (95% confidence interval) derived from NMA for SECvs. ADA and INF for PASI 75 was 1.35 (1.19, 1.57) and 1.01 (0.90, 1.18), respectively. At the 16-week and 24-week time interval, SEC was significantly better than PLA, ADA, and INF for PASI 75 and 90.Conclusion: Efficacy of SEC in the treatment of patient populations with moderate-to-severe plaque psoriasis is well demonstrated through NMA.

Newcastle disease (ND) virus is a leading threat to commercial and domestic poultry in Pakistan. The virus infects and constitutes irreversible impairment to the nervous system, damages the respiratory system, and marks severe gastrointestinal lesions leading to heavy mortality in short-living birds and substantial losses in layers and breeders. The continuous emergence and evolution of the virus made it inclined to evade the humoral response and indirectly the circumvention of artificial active immunization. Newcastle disease is caused by the orthoavula genus of the paramyxoviridae family and has shown high genetic diversity even in their genotypes while information regarding enzootic trends of the virus is scanty in Pakistan. A total of 40 tracheal samples of NDV were collected from different commercial broiler farms and 11 isolates of NDV were identified. In the current study, we determined the genetic diversity of the Newcastle disease virus based on the partial sequencing of the fusion protein gene available in the NCBI database. Genetic analysis showed that seven isolates belonged to class I genotype VII and four belonged to class II genotype II. Interestingly, two isolates had epidemiological connections with vaccine-like class II genotype II. Our findings, concerning the recent outbreaks of class I genotype VII and class II genotype II of NDV in vaccinated commercial flocks, suggest possible potential partial mutations in the fusion protein gene. Genetic diversity and formation of the new cleavage site in an important neutralizing protein of wild strain are linked with the potency of artificial active immunization and a major cause of vaccine failure.
