Objective. Acid secretion produced by a heterotopic gastric mucosal patch (HGMP) in the proximal esophagus, instead of gastric acid, may be responsible for laryngopharyngeal reflux (LPR), passing the upper esophageal sphincter. The aim of this study was to investigate the prevalence of HGMP in the proximal esophagus in patients with posterior laryngitis indicating the presenc e of LPR in comparison with a control group and to elucidate the possible role o f this lesion in the pathogenesis of LPR. Material and methods. A total of 36 co nsecutive patients with posterior laryngitis diagnosed on laryngoscopic examinat ion were enrolled in the study. Esophagoscopy and ambulatory 24 h intra-esophag eal dual-probe pH monitoring were performed in all patients. During endoscopy, special attention was paid to the proximal part of the esophagus, and the proxim al electrode for pH monitoring was placed in this region under endoscopic view. The control group comprised 660 consecutive patients who had undergone upper gas trointestinal endoscopy for the usual indications. When HGMP was found, biopsies were taken for histological confirmation. Results. HGMP was detected in 5 out o f 36 patients. One out of five patients with patches was excluded from the study because the histopathology of this patient‘s patch revealed antral-type mucos a, which is not capable of acid secretion. Thus a total of 35 patients were incl uded in the study, yielding a HGMP prevalence of 11.4%(4/35). Compared with the prevalence of the control group (1.6%), a significant difference was observed (p < 0.005). pH monitoring showed that 45.4%of the patients had abnormal proxim al acid reflux. 炎症性肠病患者的感觉神经性耳聋:一种亚临床肠外表现

Isolated case reports in which symptomatic hearing loss develops suddenly during the course of inflammatory bowel disease (IBD) have been reported, but the presence of subclinical sensorineural hearing loss (SNHL) associated with IBD has been investigated in only two preliminary *** order to research this further, we aimed to investigate the presence of subclinical SNHL in IBD by comparison with a control group and to examine possible relations between the bowel disease parameters and hearing ***, tympanometry, and pure tone audiometry were carried out in 39 patients with IBD (21 Crohn’s disease [CD], 18 ulcerative colitis [UC]) and 25 healthy ageand sex-matched controls. All patients and control subjects had normal otoscopy findings and tympanometry was unremarkable, excluding middle ear disease and conductive hearing loss. Analysis of each frequency examined showed that the average hearing thresholds were increased significantly in the study group compared to those of the control group at higher frequencies (2, 4, and 8 kHz). When these parameters were compared with the control group according to subgroups of IBD, a significant difference was determined for the UC group at frequencies of 2, 4, and 8 kHz and for the CD group only at the frequency of 4 kHz. Although there was a trend of increment in SNHL as the age of the patient and duration and extent of UC increased, no significant correlation was observed between SNHL and these parameters or sex, activity, involvement site, medication history of IBD, and coexistence of other extraintestinal manifestations. In conclusion, it was demonstrated that a subclinical SNHL may be associated with UC and somewhat with CD, affecting mainly the high frequencies. In light of this finding, it may be advisable to investigate labyrinth functions as well as other extraintestinal manifestations in patients with IBD.

Safety of applying midazolam-ketamine-propofol sedation combination under the supervision of endoscopy nurse with patient-controlled analgesia pump in colonoscopy

AIM To compare the results of midazolam-ketaminepropofol sedation performed by an endoscopy nurse and anaesthetist during colonoscopy in terms of patient satisfaction and *** American Statistical Association(ASA) Ⅰ-Ⅱ 60 patients who underwent colonoscopy under sedation were randomly divided into two groups: sedation under the supervision of an anaesthetist(SSA) and sedation under the supervision of an endoscopy nurse(SSEN). Both groups were initially administered 1 mg midazolam, 50 mg ketamine and 30-50 mg propofol. Continuation of sedation was performed by the anaesthetist in the SSAgroup and the nurse with a patient-controlled analgesia(PCA) pump in the SSEN group. The total propofol consumption, procedure duration, recovery times, pain using the visual analogue scale(VAS) and satisfaction score of the patients, and side effects were recorded. In addition, the patients were asked whether they remembered the procedure and whether they would prefer the same method in the case of *** Total propofol consumption in the SSEN group was significantly higher(P 0.05) between the two groups. No long-term required intervention side effects were observed in either *** Colonoscopy sedation in ASA Ⅰ-Ⅱ patients can be safely performed by an endoscopy nurse using PCA pump with the incidence of side effects and patient satisfaction levels similar to sedation under anaesthetist supervision.

Maintaining suitable temperature level around tomato in the greenhouse is essential for the high-quality ***,in summer,the temperature level around the tomato is usually unclear except using a high-precision temperature *** solve this problem,thermal performance of 3D(three-dimensional)tomato model built based on SolidWorks was investigated by the computational fluid dynamics(CFD)*** assess the effect of temperature distribution around the tomato,a simplified 3D tomato numerical model was firstly validated by a set of field measurement *** light intensity and indoor ventilation were regarded as the mainly environment factors in the Venlo greenhouse,thermal stratification around tomatoes at different time of day was further *** numerical results illustrated the different temperature distribution around tomato body under different radiation *** was found that ventilation could obviously adjust the temperature gradient around the tomato,and alleviate high temperature effect particularly in *** ventilation could create a suitable thermal environment for the tomato *** study clearly demonstrated 3D temperature distribution around tomatoes,which is beneficial to provide the reference for accurate detection of 3D tomato temperature and appropriate thermal environment design.
