Prevalence of normoglycemic,prediabetic and diabetic A1c levels

AIM:To investigate normoglycemic,prediabetic and diabetic A1c levels in those with prediabetes;and prediabetic and diabetic A1c levels in those with ***:The National Health and Nutritional Examination Survey(NHANES)2007-2008 and NHANES2009-2010 were utilized to examine and compare trends and differences among five different ethnic groups(Mexican Americans,Other Hispanics,NonHispanic Whites,Non-Hispanic Blacks,Other/Multiracials)with normoglycemic,prediabetic and diabetic A1c levels with self-reported prediabetes and prediabetic and diabetic A1c levels in those with self-reported *** participants of the five ethnic groups were limited to those 20 years of age and older,who had completed the diabetes questionnaire and had A1c *** statistics were computed for all variables.χ2were performed on all five ethnic groups to examine significant differences of normoglycemic,prediabetic and diabetic A1c levels in those with self-reported prediabetes,and prediabetic and diabetic A1c levels in those with self-reported ***:This study demonstrates that of the five different ethnic groups from NHANES 2007-2008 to NHANES 2009-2010,Non-Hispanic Whites(6.5%increase)and Non-Hispanic Blacks(0.2%increase)were the only two groups with an increase in the number of self-reported *** the overall percentage of Mexican Americans who self-reported prediabetes had remained the same(5%)from NHANES2007-2008 to NHANES 2009-2010,χ2analysis showed significant differences when examining the different ranges of A1c levels(normoglycemic,prediabetic and diabetic).Among Mexican Americans who self-reported prediabetes,normoglycemic(P=0.0001)and diabetic(P=0.0001)A1c levels from NHANES 2007-2008 to NHANES *** Non-Hispanic Whites who self-reported prediabetes,prediabetic(P=0.0222);and diabetic(P≤0.0001)A1c levels from NHANES2007-2008 to NHANES *** Non-Hispanic Blacks who self-reported prediabetes,t

Helping Nurses Cope with COVID-19 Pandemic: Evaluating Support Programs

During the height of the Coronavirus Disease (COVID-19) Pandemic, nurses all over the world, including members of the National Association of Nigerian Nurses in North America (NANNNA) experienced a heightened and unprecedented level of stress, anxiety, fear, uncertainty, concern for their safety and that of their loved ones. This prompted NANNNA leadership to initiate and conduct support programs for nurses especially those on the frontline of care. This project evaluates the effectiveness of the support programs conducted by NANNNA during the early surge of COVID-19. The Centers for Disease Control’s (CDC’s) Framework for Evaluation in Public Health guided this evaluation. Focus group discussion and online survey was utilized to gather data. While 19% of respondents reported being diagnosed with COVID-19, the majority (81%) were not. About 44% of respondents had family members who were diagnosed with COVID-19, while 18% reported loss of a close family member due to COVID-19 complications. Most respondents reported that the program was impactful in information sharing as well as significant in helping them feel connected with other nurses. Hence, this subsequently enhanced their support networks, provided emotional and spiritual support. Survey results revealed that NANNNA support programs had positive effects in helping members mitigate the fear, anxiety and uncertainty experienced during the surge of the COVID-19.

Coronavirus disease 2019 (COVID-19) is one of the worst deadly infections that is currently causing devastating effects and damages to humanity across the world. The global pandemic which outbroke in Wuhan in China has equally affected almost every country in the world. Over the past several months of the coronavirus crisis, cities went into lockdown, countries issued stay-at-home orders, and an increasing number of people had been practicing social distancing on a societal scale that has never been witnessed. However, nurses continue to provide care, comfort, and information to patients at all levels while risking their personal safety and well-being. This executive report provides an insight in Nigerian nurses’ perspectives and challenges in combating COVID-19 during the surge. Capturing nursing practice and nurses’ experiences during COVID-19 surge provide real-time guidance to mitigating challenges and improving well-being of nurses.

Education is the key to protecting children against smoking: What parents think and do

The purpose of this study was to examine parents’ communication with their children about the topic of smoking. A qualitative descriptive design was used. Twenty-nine parents who lived in rural communities and who had children in kindergarten to Grade 6 were interviewed. The data were analyzed for themes. A large majority of parents communicated with their children about smoking through verbal interaction, using any one of three approaches: discussing smoking with their children, telling their children about smoking, or acknowledging their children’s understanding of smoking. Those parents also had shown disapproval of smoking, which took different forms and varied from explicit messages in their verbal communication to implicit messages in their behaviours. Three parents had not verbally communicated at all with their children about smoking. Overall, the parents’ communication patterns with their children varied in terms of quality and coherence with recommendations in the literature.

The Impact of Cervical Cancer Treatment on Sexual Function and Intimate Relationships: Is Anyone Listening?

The purpose of this research was to describe women’s narrative accounts of the impact of cervical cancer treatment on their sexual function and intimate relationships, and to evaluate what changes in care and education are needed to enhance quality of life and intimacy after treatment. The research approach was a narrative design, using semi-structured, in-depth interviews. Narratives were examined within and across interviews, and thematic content analysis completed. The study was done in a gynecologic oncology clinic at a public hospital in the Midwest United States. The sample consisted of twelve women, ranging in age from 27 to 59, who had completed the cervical cancer treatment with chemo-radiation or radiation and surgery, and were now followed by their gynecologic oncologists. Across narratives, five major themes were identified, including unexpected physical complications, not “getting back to normal,” emotional pain and isolation, lack of available information, and inadequate health care provider response to treatment complications and sexual relationship problems. Women’s stories reveal that sex and intimacy issues for cervical cancer survivors remain within a culture of silence. In many situations, health professionals did not provide information that realistically prepared women and partners for probable consequences of treatment, did not assess sexual issues before or after treatment, did not recognize various symptoms as being complications of cancer treatment, did not make referrals, and/or recognized complications, but accepted them as “normal” and without solution. Ethical implications for health professionals and the need for education, communication, and the development of new lines of research are discussed.

The Hole in Holistic Patient Care

Throughout its history, the nursing profession has claimed to provide holistic patient care, which is defined as bio-psycho-socio-spiritual care. Today, however, many nurses do not feel comfortable with the “spiritual” element of care and are uncertain about their professional role in the assessment and delivery of spiritual care. Discomfort and avoidance of attending to the spiritual needs of human beings creates “a hole” in holistic patient care. Contributing factors to the “hole in holistic patient care” include: 1) blurring of boundaries in the language and definitions of “spirituality” and “religion”, 2) insufficient attention to definitions of spirituality and spiritual distress;3) confusion and role conflict with professional identity among disciplines related to responsibility for spiritual care;4) insufficient education and skill development for nurses and other healthcare professionals in the assessment, intervention and appropriate referral of patients experiencing spiritual distress. The purpose of this article is to explore the history of holistic nursing as it pertains to the human dimension of spirituality and conclude with practice models for spiritual assessment and spiritual care that can “fill” the hole in holistic nursing care.

Objective To determine the prevalence and associations of general practice registrars’performing absolute cardio-vascular risk(ACVR)assessment(ACVRa).Design A cross-sectional study employing data(2017–2018)from the Registrar Clinical Encounters in Training project,an ongoing inception cohort study of Australian GP *** outcome measure was whether an ACVRa was *** employed univariable and multivariable *** was conducted for all patient problems/diagnoses,then for an‘at-risk’population(specific problems/diagnoses for which ACVRa is indicated).Setting Three GP regional training organisations(RTOs)across three Australian *** GP registrars training within participating *** 1003 registrars(response rate 96.8%)recorded details of 69105 problems either with Aboriginal and/or Torres Strait patients aged 35 years and older or with non-Indigenous patients aged 45 years and *** these problems/diagnoses,1721(2.5%(95%CI 2.4%to 2.6%))involved an *** ACVRa was‘plausibly indicated’in 10384 problems/*** these,1228(11.8%(95%CI 11.2%to 12.4%))involved ***‘all problems/diagnoses’,on multivariable analysis female gender was associated with reduced odds of ACVRa(OR 0.61(95%CI 0.54 to 0.68)).There was some evidence for Aboriginal and/or Torres Strait Islander people being more likely to receive ACVRa(OR 1.40(95%CI 0.94 to 2.08),p=0.10).There were associations with variables related to continuity of care,with reduced odds of ACVRa:if the patient was new to the registrar(OR 0.65(95%CI 0.57 to 0.75)),new to the practice(OR 0.24(95%CI 0.15 to 0.38))or the problem was new(OR 0.68(95%CI 0.59 to 0.78));and increased odds if personal follow-up was organised(OR 1.43(95%CI 1.24 to 1.66)).For‘ACVRa indicated’problems/diagnoses,findings were similar to those for‘all problems/diagnoses’.Association with Aboriginal and/or Torres Strait Islander status,however,was significant at p<0.05(OR 1.60(95%CI 1.04 to 2.46))a

As hospitals undergo reductions in budget and the demand for endoscopic procedures grows, the need for more efficient endoscopy units has increased. Certified Registered Nurse Anesthetists (CRNAs) play a vital role in Endoscopy Units and because of this, increasing CRNA work efficiency would increase the efficiency of the entire unit. This pilot study aims to explore CRNA workflow and identify workflow barriers present in endoscopy rooms through qualitative interview analysis. Ten semi-structured interviews were conducted at the University of Cincinnati Medical Center. The interview data were analyzed qualitatively to generate workflow diagrams and four themes of workflow barriers and representative quotes. These themes include: 1) physician unavailability, 2) patient delay and variability, 3) suboptimal teamwork and coordination, and 4) issues with supporting tools. The workflow barriers reflect and extend the current literature, and can inform future quality improvement initiatives.
