Disposable circumcision suture device： clinical effect and patient satisfaction

In our experience patients undergoing circumcision are mostly concerned about pain and penile appearances. We conducted a prospective randomized trial to assess the benefits of a new disposable circumcision suture device （DCSD）. A total of 942 patients were equally divided into three groups （conventional circumcision, Shang ring and disposable suture device group）. Patients in the DCSD group were anesthetized with compound 5% lidocaine cream, the others with a 2% lidocaine penile block. Operation time, intra-operative blood loss, incision healing time, intra-operative and post-operative pain, the penile appearance and overall satisfaction degree were measured. Operation time and intra-operative blood loss were significantly lower in the Shang ring and suture device groups compared to the conventional group （P 〈 0.001）. Intra-operative pain was less in the suture device group compared With the other two groups （P 〈 0.001）; whereas post-operative pain was higher in the conventional group compared to the other two groups （P 〈 0.001）. Patients in the suture device （80.57%） and Shang ring （73,57%） groups were more satisfied with penile appearances compared with the conventional circumcision group （20.06%, P 〈 0.05）. Patients in suture device group also healed markedly faster than the conventional group （P 〈 0.01）. The overall satisfaction rate was better in the suture device group （78.66%） compared with the conventional （47.13%） and Shang ring （50.00%） groups （P 〈 0.05）. The combination of DCSD and lidocaine cream resulted in shorter operation and incision healing times, reduced intra-operative and post-operative pain and improved patient satisfaction with the cosmetic appearances.

Pointed condyloma in the anus is caused by a virus of human papilloma, marked by the presence of rapidly growing cutaneous verrucae with itching, mild pain and offensive secretion. Diagnosis can be made based on biopsy. The disease is of high recurrence and very difficult to be cured radically. The author combined surgery with external washing with chinese herbal medicine and achieved quite good results. The following is report of the treatment.

Objective:To observe the therapeutic efiects of acupuncture on gouty renal ***:72 cases of gouty renal damage were randomiy divided into a treatment group of 42 cases and a control group of 30cases to observe the therapeutic efiects and the changes in 24-hour urinary protein content,blood creatinine,uric acid and urea nitrogen in blood beforetreatment and one month after ***:The total effective rate in the treatment group reached 95.24%,which was remarkably higher than 63.33%in the control *** one month of treatment,the indexes were found reduced in both groups,but the reduction rate in the treatment group was obviously superior to that in the control ***:The patients with repeated attacks of gout may have a higher possibility to sufierfrom renal ***,attention should be paid to its early diagnosis and *** may exert good theraputic efiects on early gout comp1icated with renal damage by adjusting the metabolism and improving the reaal function.

Case HistoryPatient Chen, female, 68 years, case number: 9800297; date of hospitalization: May 11, 1998.Chief complaints: Fever accompanied with cough and lassitude for 3 months and skin rash for 7 days.History of present illness: In early Feb. 1998, the patient got fever accompanied with cough, chest distress, and expectoration of sticky sputum. She had been diagnosed at a hospital and later at a clinic as chronic bronchitis and pneumonia, and treated for more than 50 days with antibiotics and medication of some drugs for cough and sputum, but with no complete remission of the symptoms. One week previously, she developed general skin rash mainly in the face, accompanied with itching, high body temperature of 38~39℃, aggravated cough and chest distress, lassitude, poor appetite, slight thirst, absence of bowel movement for 3 days, and yellow brownish urine.

Clinical curative effect of fuzi-cake-separated moxibustion for preventing dysuria after operation for lower limb fracture

OBJECTIVE: To assess the clinical curative effect of fuzi-cake-separated moxibustion at Zhongji(CV 3)and Guanyuan(CV 4) for preventing dysuria after internal fixation of lower limb ***: Sixty patients conforming to the inclusion standards were randomly divided into a treatment group(n=30) and a control group(n=30).Fuzi-cake-separated moxibustion was performed at Guanyuan(CV 4) and Zhongji(CV 3), 20 min at a time, twice a day, for 3 days before operation in the treatment group. No fuzi-cake-separated moxibustion was performed in the control group. After treatment, the score for symptoms of first urination, urinary time, urinary volume, 24 h remaining urinary volume, incidence of uroschesis, and rate of controlling dysuria were compared to evaluate the curative effect of preventing post-operative ***: The score for symptoms of first urination,24 h remaining urinary volume(maximum 120 m L vs 250 m L, and less than 10 m L in 24 cases vs 15 cases), and the rate of controlling dysuria(83.34% vs30%) were significantly better(P0.05) between the two groups in first post-operative urinary time, urinary volume, or incidence of 24 h ***: Fuzi-cake-separated moxibustion at Zhongji(CV 3) and Guanyuan(CV 4) can better prevent post-operative dysuria, effectively promote the functional restoration of the urinary bladder,and control the incidence of post-operative dysuria.
