Digestive and breast cancer patients managed during the first wave of COVID-19 pandemic:Short and middle term outcomes

Background: Emerging evidence has recognized that anemia and iron deficiency are recurrent comorbidities in chronic heart failure (HF) and several trials have established that iron administration improves myocardial asset and clinical scenario in HF. Purpose: Recent acquisitions suggest that iron deficiency represents a concrete bias in the pathogenetic mechanism of chronic HF, so we have investigated the putative role of the hepcidin/ferroportin axis in the cardiovascular setting to advocate novel pharmacological and clinical approaches. Methods: Here, after an excursus on iron metabolism, we first reviewed the ongoing studies on novel iron targeted compounds. Then, we summarize large clinical interventional studies conducted on patient suffering from iron deficiency and HF which have tested the effects of drugging iron regard QoL, hospitalizations and cardiovascular death. Results: Novel compounds such as hepcidin agonist (PTG 300), synthetic human hepcidin (LJPC-401) and anti FPN (Vamifeport) are ongoing in iron overloaded patients, while the hepcidin blocker (PRS-080) is under investigation in anemic patients. Noteworthy, novel insights could arise from the results of a Phase IV interventional study regarding the modification of hepcidin pathway in a large cohort of HF patients (n = 1992) by sodium glucose cotransporter 2 inhibitors. To date, several studies highlight the beneficial effect of iron administration in cardiovascular setting and latest evidences consider hepcidin level as a novel biomarker of cardiac injury and atherosclerosis. Conclusions: We advocate that data from ongoing studies will suggest novel iron targeted therapies for diagnosis, prognosis and therapy transferable in selected heart failed patients.

Dexamethasone and N-acetylcysteine before transarterial chemoembolization in hepatocellular carcinoma:A Western perspective

Post-embolization syndrome(PES)is the most common complication in patients with hepatocellular carcinoma treated with transarterial *** strategies have been evaluated to reduce the incidence of PES,but no standard prevention guidelines currently *** a single-center,placebo-controlled trial,Simasingha et al evaluated the prophylactic administration of a combination of dexamethasone and N-acetylcysteine and documented a significant reduction in the incidence of PES(from 80%to 6%),of post-procedural liver decompensation(from 14%to 0%),and a shorter hospital stay(4 days vs 6 days),alongside an acceptable safety *** results of this study raise several controversial points regarding their applicability in the Western *** the West,there is a greater and increasing prevalence of metabolic and alcoholic etiologies of liver cirrhosis,so a not negligible number of patients with type II diabetes or hypertension would be excluded from high-dosage dexamethasone ***,in the West,there is a preferred use of drug-eluting beads loaded with doxorubicin,which are associated with a lower incidence of PES.A study on prophylaxis with dexamethasone and/or N-acetylcysteine in a Western population is hopefully awaited.

Long-term monitoring reveals the effect of precipitation and silviculture on Nothofagus regeneration in Northern Patagonia mixed forests

Background Tree regeneration is a key component of resilience because it promotes post-disturbance recovery of forests. Northwestern Patagonia from Argentina is occupied by Nothofagus alpina（Na）, N. obliqua（No）, and N. dombeyi（Nd） forests that grow along an intense precipitation gradient, managed throughout shelterwood silvicultural system by technicians of the Lanin Natural Reserve. The objective of this study was to evaluate the influence of seeding cuttings over the dynamics of Nothofagus mixed forests across landscape（precipitation gradient） based mainly on the composition and abundance of tree regeneration, permanent sampling plots and generalized linear mixed models. In particular, we analysed:（i） the structure of sexual and asexual regeneration during 20 years after harvest（the dynamics of managed forests）, and（ii） the structure of sexual regeneration in primary and managed forests after > 20 years of harvest（the effect of silviculture）.Results Nd was the most abundant species in the regeneration of managed forests during all periods in both sites despite its lower contribution to the adult cohort. During the 10–20 years period after harvest, the humid site exhibited higher regeneration density than the mesic site(120,000 and 6000 ind ha-1, respectively), and it decreased afterwards. The number of established regeneration（> 2 m height） was lower for Na in the mesic site and for No in the humid site(0 and 57 ind ha-1, respectively). However, in comparison to No, Na showed a higher number of sprouted stumps and sprouts per stump, and a higher sprout height in the mesic site. No exhibited higher sprout mortality in the humid site. Xiao Tan san Jie Fang Hampers the Growth of Colon Cancer Stem Cells through the Wnt/β-catenin Signaling Pathway

Objective: The purpose of this research was to examine the potential anticancer properties of Xiao Tan san Jie Fang(XTSJF) and its potential mechanism of action against colorectal cancer. Materials and Methods: HCT116 cells were induced into HCT116 spheres in DMEM/F12 medium by treatment with epidermal growth factor + fibroblast growth factor + leukemia inhibitory factor + B27. The proliferation ability and stemness of HCT116 spheres was examined. Various concentrations of XTSJF were used to treat HCT116 spheres to observe the impact on proliferation, apoptosis, and expression of stem cell markers. Next, Wnt/β-catenin pathway-related factor proteins were detected. Results: The findings revealed that XTSJF suppressed cell growth and induced cell death in HCT116 cells in a dosage-dependent manner. Similarly, XTSJF promotes apoptosis, inhibits cell proliferation, prolongs survival, and maintains the expression of stem cells through the Wnt/-catenin/TCF4 axis. XTSJF also inhibits AKT activity and subsequently activates glycogen synthesis kinase-3β expression, inhibiting Wnt/beta-catenin pathway activity and downstream target gene transcript expression. The Wnt/β-catenin signaling pathway is inhibited by the XTSJF, leading to the suppression of colon cancer stem cell proliferation. Conclusion: Xiaotan sanjie Prescription inhibited colon cancer stem cell growth through Wnt/β-catenin signaling pathway.
