Preliminary results of a new intrastromal corneal ring segment as a tissue saving procedure in photorefractive keratectomy to correct moderate to high myopia

AIM:To evaluate the clinical results after implantation of a new intrastromal corneal ring segment(ICRS)associated with photorefractive keratectomy(PRK)to correct high myopia(HM)patients with thin ***:We evaluated 42 eyes of 23 HM patients that had ICRS implantation followed by *** mean age of patients was 29.1±7.12 y(range 18 to 40 years old).Uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA),keratometry,spherical equivalent,pachymetry,and aberrometry were compared using ANOVA with repeated measurements evaluated preoperatively and at last follow-up visit after the *** refractive predictability and simulated/real corneal ablation were also ***:The mean follow-up time after PRK was 6.8±1.6 *** mean preoperative UCVA improved from 20/800 preoperative to 20/100 after ICRS and 20/35 after *** mean preoperative BCVA was 20/25(range from 20/30 to 20/20)and remained unchanged after ICRS *** the PRK the mean BCVA was 20/25(range from 20/30 to 20/20).The mean spherical equivalent decreased from-7.25±1.12(range-5.00 to-9.00)preoperatively to-3.32±1.0(range-2.00 to-5.00)postoperatively(Pdecreased from-2.44±1.51 preoperatively to 0.32±0.45(range-0.625 to 0.875)postoperatively(Pderate to high myopia.

Rhinosinusitis affects approximately 20%of the population,and the chronic rhinosinusitis represents over 90%of all cases of *** correct diagnosis is important for proper treatment and to predict its *** study presents two cases of atypical frontal sinus disease,which the follow-up revealed a diagnosis of fungal *** present study aims to describe the cases of two patients with atypical lesions on the left frontal sinus;the treatment options,surgical approach,results,diagnosis and follow-up are further discussed.A significant increase in the reported cases of fungal rhinosinusitis has been seen in the last two decades,justified by the use of broad-spectrum antibiotics and steroids,as well as the increased number of immunocompromised *** study reports the cases of two patients with a type of fungal rhinosinusitis named"fungal ball",characterized by a tangle of hyphae in the sinuses without tissue *** treatment included surgical removal of the fungal infectious process with aeration of the affected sinus,and the procedure was successfully performed in our patients.

In the face of new diseases, medicine needs to reinvent itself in order to contain and control epidemics, such as the one we have recently faced, COVID-19, a disease with a wide spectrum of clinical severity. A new moment has been established, since the application of well-known, effective and safe medications for other diseases, has shown high success rates in the treatment of COVID-19. Thereunto, studies with early intervention are needed, which can change the unfavorable outcome of patients. In this article, we report the successful experience using an oral strategy during the collapse of Belem (Para, Brazil) Health System. Two hundred and ten patients were diagnosed with respiratory failure due to COVID-19, with no option of hospital treatment due to lack of beds and resources. These patients were then started on therapeutic regimen consisting of 40 mg prednisolone, 40 mg enoxaparin and macrolides (500 mg clarithromycin, 500 mg axetylcefuroxime) associated and followed in outpatient facilities. Two hundred and eight patients had an excellent therapeutic response and there were only two fatalities. These results push research boundaries, valuing outpatient treatment with early use of prednisolone in the initial pulmonary phase, preventing severe COVID-19 pneumonitis. Adoption of the proposed treatment intends to reduce the need for hospitalization, as well as lethality, with social robust benefits and incalculable economic savings since involves the use of accessible, safe and not expensive medications.

PURPOSE: To investigate macular optical coherence tomography (OCT) features in patients with chorioretinal anastomosis (CRA) and drusen, as well as their correlation with the Gass occult-CRA hypothesis. deSIGN: Prospective observational case series. METHODS: setting: Tertiary ophthalmic referral center. study population: All patients with biomicroscopic evidence of CRA and drusen consecutively evaluated between February 2003 and March 2004. observation procedures: Third-generation OCT evaluation and stereoscopic angiographic studies. main outcome measures: Macular morphologic features at baseline and at 12 weeks. RESULTS: Twenty eyes with CRA and drusen were identified in 11 patients, seven women (63.6% ) and four men (36.4% ) ranging in age from 69 to 82 years (median, 79 years). Focal elevation of the retinal pigment epithelium was seen in eyes with stage 1 (pre-clinical) CRA. Small hyperreflective signals at the level of the elevated retinal pigment epithelium were seen in stage 2 CRA. In stage 3 CRA, a hyperreflective “ mound” at the level of the elevated retinal pigment epithelium was seen in association with a thickened retina. In stage 4 CRA subretinal pigment epithelium fluid accumulation was present, and complete disorganization of the macular region was observed in stage 5 CRA. Macular changes were observed in eight eyes (40% ) at follow-up, with all but one CRA lesion progressing one stage. CONCLUSION: Morphologic features and changes demonstrated by OCT suggest that fibrovascular detachment of the retinal pigment epithelium followed by development of occult CRA are the initial events occurring in eyes with CRA in age-related macular degeneration. Our findings may support the evolutionary CRA staging system proposed by Gass.
