Acute necrotizing pancreatitis(ANP)is a critical type of acute pancreatitis(AP)[1,2].ANP has high mortality,long-term hospitalization,high cost,and potential morbidity[3,4].Infected or symptomatic necrosis continually requires debridement as the mainstay of treatment when percutaneous drainage fails[5].With the advent of minimally invasive approaches in recent decades,surgical management of pancreatic necrosis has been *** recommend using a step-up approach instead of direct open pancreatic necrosectomy(OPN)[6].Endoscopic drainage and necrosectomy are appropriate for walled-offnecrosis(WON)near the stomach or duodenum[7].Minimal access retroperitoneal pancreatic necrosectomy(MARPN)is a good approach for laterally located WON that is distant from the stomach or duodenum[8].However,these minimally invasive approaches are still not *** repeatedly debridement itself causes ***,large pieces of necrosis are not easy to clear,and blood vessels are difficult to *** of bleeding,gastrointestinal fistula,and pancreatic fistula are still inevitable.

Exploring electrode materials with larger capacity,higher power density and longer cycle life was critical for developing advanced flexible lithium-ion batteries(LIBs).Herein,we used a controlled two-step method including electrospraying followed with calcination treatment by CVD furnace to design novel electrodes of Si/Si_(x)/C and Sn/C microrods array consisting of nanospheres on flexible carbon cloth substrate(denoted as Si/Si_(x)/C@CC,Sn/C@CC).Microrods composed of cumulated nanospheres(the diameter was approximately 120 nm)had a mean diameter of approximately 1.5μm and a length of around 4.0μm,distributing uniformly along the entire woven carbon *** of Si/Si/Si_(x)/C@CC and Sn/C@CC products were synthesized as binder-free anodes for Li-ion battery with the features of high reversible capacity and excellent *** Si/Six/C electrode exhibited high specific capacity of about 1750 mA∙h∙g^(−1)at 0.5 A∙g^(−1)and excellent cycling ability even after 1050 cycles with a capacity of 1388 mA∙h∙g^(−1).Highly flexible Si/Si_(x)/C@CC//LiCoO_(2)batteries based on liquid and solid electrolytes were also fabricated,exhibiting high flexibility,excellent electrical stability and potential applications in flexible wearable electronics.

Pregnancy in cirrhotic-woman is a rare event [1] [2]. It causes particular risks related to liver disease and its treatments, with maternal and fetal consequences. The largest reported series to date is by Welton’s with 13 cases [3]. In Africa, few cases were reported. The purpose of this work was to report the hepatic and obstetric complications in pregnant women in Burkina Faso, a low-income country. It was a descriptive cross-sectional study with retrospective data collection covering the period from January 1, 2013 to December 31, 2023. All the women diagnosed with cirrhosis through clinical basis, biological and imaging arguments associated with a pregnancy confirmed by ultrasound were included. A total of ten women were included in the study. Among them, two were known cirrhotics and were being followed. In the other eight cases, cirrhosis was discovered during pregnancy. Eight women were HBsAg positive and two of them had only the contact marker of hepatitis B virus (anti- HBc antibody). The evolution was, ascitic decompensation in nine cases, digestive hemorrhage due to rupture of esophageal varices in two cases, nine cases of anemia, two cases of ascitic fluid infection and one case of conjunctival jaundice. Two cases of probable hepatocellular carcinoma were noted with one case of extensive portal thrombosis. The evolution was without maternal complications in one case. Regarding fetal and neonatal complications, there were three cases of hypotrophy, one case of prematurity, one case of acute neonatal distress, two cases of fetal death in utero and one case of fetal tachycardia. Four pregnancies had a favorable evolution without any complications. The association of cirrhosis and pregnancy is an uncommon event, and maternal and fetal complications can occur, making this pregnancy a high-risk pregnancy.

Fontan operation is indicated in a single ventricle congenital heart disease which creates a shunt between superior vena cava and pulmonary arteries leading to increased pressure in central venous system and congestive hepatopathy,namely,Fontan-associated liver disease(FALD)[1].Recently,the long-term prognosis after Fontan operation has been improving and the number of patients diagnosed with hepatocellular carcinoma(HCC)arising from FALD is increasing[1].There are several publications on the safety of laparoscopic,conventional surgery and interventional radiological modalities in FALD-HCC patients[2-5].However,there are no reports regarding the robotic hepatectomy for the FALD-HCC *** was the first report showing the safety of robotic anatomical hepatectomy in FALD-HCC patients.

This article aims to deepen the understanding of the role of Helicobacter pylori(***)infection in the development of cholelithiasis,initiated by the article by Yao et al,who investigated the potential link between *** infection and the development of cholelithiasis through a multicenter retrospective study on an Asian population of over 70000 *** also performed a compre-hensive analysis of previously published studies on *** and cholelithiasis,finding a positive association therein[odds ratio(OR)=1.103,P=0.049].Patients positive for *** also had lower levels of total and direct bilirubin,but higher levels of total cholesterol and low-density lipoprotein cholesterol compared to uninfected patients(P<0.05).Cohort studies have confirmed that *** is a risk factor for cholelithiasis(P<0.0001),and aggregate analyses of case-control and cross-sectional studies have shown a positive association between *** and cholelithiasis in Asia(OR=1.599,P=0.034),but not in Europe(OR=1.277,P=0.246).Moreover,*** appears to be related to a higher ratio of choledocho-lithiasis/cholecystolithiasis(OR=3.321,P=0.033).The authors conclude that *** infection is positively correlated with cholelithiasis,particularly with the choledocholithiasis phenotype,especially in Asia,and it is potentially related to bilirubin and cholesterol metabolism.

AIM: To measure a broad profile of pro-and anti-inflammatory cytokines in patients with clinically proven chronic pancreatitis (CP) taking either antioxidant therapy or placebo as part of the larger ANTICIPATE study. METHODS: Patients with chronic pancreatitis were recruited to the ANTICIPATE study following informed consent and were randomised to intervention with either antox version 1.2-based antioxidant therapy or placebo. After a separate ethics committee amendment a subgroup of 7 patients from either arm of the study were selected for additional analysis of cytokines. Cytokines were measured at baseline and after 6 mo of either antox therapy or placebo by biochip array and enzyme-linked immunosorbent assay. RESULTS: Antioxidant therapy and placebo groups were well-matched in terms of age, gender, aetiology of CP, opiate use and disease duration. Baseline antioxidant levels were similar in patients allocated to the antioxidant group as compared to the group al- located to placebo. After 6 mo of antioxidant therapythere was significant elevation in vitamin C levels in the intervention group: 17.6 μg/mL (12.8-29.3 μg/mL) compared to 4.8 μg/mL (1.6-9.1 μg/mL) in placebo (P < 0.001; 95%CI: 9.0-20.2) with similar trends in selenium levels. There was no elevation in a broad array of pro-and anti-inflammatory cytokines in the antioxidant group compared to placebo [interleukin (IL)-1B, IL-4, IL-6, IL-10, tumor necrosis factor-α] either at baseline or after 6 mo of antioxidant therapy. CONCLUSION: Cytokine levels were low at baseline and at 6 mo despite a significant elevation in plasma antioxidants. In patients with CP, with opiate-dependent abdominal pain, circulating cytokine levels are low suggesting that pain in this disease is not simply a manifestation of inflammation.

A I M : To present our extensive experience of hepatectomy for hepatocellular carcinoma using a microwave tissue coagulator to demonstrate the effectiveness of this device. METHODS: A total of 1118 cases(1990-2013) were reviewed,with an emphasis on intraoperative blood loss,postoperative bile leakage and fluid/abscess formation,and adaptability to anatomical resection and hepatectomy with hilar ***: The median intraoperative blood loss was 250 m L; postoperative bile leakage and fluid/abscess formation were seen in 3.0% and 3.3% of cases,respectively. Anatomical resection was performed in 275 cases,including 103 cases of hilar dissection that required application of microwave coagulation near the hepatic hilum. There was no clinically relevant biliary tract stricture or any vascular problems due to heat injury. Regarding the influence of cirrhosis on intraoperative blood loss,no significant difference was seen between cirrhotic and non-cirrhotic patients(P = 0.38),although cirrhotic patients tended to have smaller tumors and underwent less invasive operations. CONCLUSION: This study demonstrated outcomes of an extensive experience of hepatectomy using heat coagulative necrosis by microwave tissue coagulator.

From June, 1986 to June 1989, 24 cases of hilar bile duct carcinoma were explored in the Surgical Department of General Hospital of PLA, 16/24 cases were resected, a resectability rate of 66%. The increase of resectability rate was due to earlier recognition of this condition and the extension of surgery, including major resection of liver as well as radical dissection of the hepato-duodenal ligament and repairative operations on the blood vessels. Among these 16 cases, major hepatic resection was performed in 10 cases, in which, 3 cases of resections of the middle lobe of the liver were done instead of right or extended right lobectomy. No operative mortality in the 30 days' postoperative period, but the postoperative morbidity rate was still high and most of the complications were related to biliary leakage and infection. Three patients died in the follow up period at 6, 14 and 15 months respectively. All of them died from biliary infection. The remaining 13 patients were still alive, the longest being 40 months and the average living time was 16.1 months. Probably, lowering of the operative mortality rate and morbidity rate are still the most important considerations in the surgical treatment of hilar carcinoma at the present time. Extensive liver resection especially on the right side, carried a high mortality rate in the deeply jaundiced patients. We considered that preoperative PTCD was of much less value than that used in lower bile duct obstruction such as tumors of the periampullary region. Preservation of the superior and posterior portion of the right lobe of the liver may be of advantages as to lowering postoperative hepatic failure and infection of the right subphrenic space as observed in this series of cases.

Surgeries for benign diseases of the extrahepatic bile duct(EHBD) are classified as lithotomy(i.e., choledocholithotomy) or diversion(i.e.,choledochojejunostomy). Because of technical challenges, laparoscopic approaches for these surgeries have not gained worldwide popularity. The right upper quadrant of the abdomen is advantageous for laparoscopic procedures,and laparoscopic choledochojejunostomy is safe and feasible. Herein, we summarize tips and pitfalls in the actual procedures of *** choledocholithotomy with primary closure of the transductal incision and transcystic C-tube drainage has excellent clinical outcomes;however, emergent biliary drainage without endoscopic sphincterotomy and preoperative removal of anesthetic risk factors are required. Elastic suture should never be ligated directly on the cystic duct. Interrupted suture placement is the first choice for hemostasis near the EHBD. To prevent progressive laceration of the EHBD, full-layer interrupted sutures are placed at the upper and lower edges of the transductal incision. Cholangioscopy has only two-way operation; using dedicated forceps to atraumatically grasp the cholangioscope is important for smart maneuvering. The duration of intraoperative stone clearance accounts for most of the operative time. Moreover, dedicated forceps are an important instrument for atraumatic grasping of the cholangioscope. Damage to the cholangioscope requires expensive repair. Laparoscopic approach for choledocholithotomy involves technical difficulties. I hope this document with the visual explanation and literature review will be informative for skillful surgeons.
