Pancreatic cancer is the fourth most common cause of cancer-related.d.aths in the United.States, with over 38000 d.aths in 2013. The opportunity to d.tect pancreatic cancer while it is still curable is d.pend.nt on our ability to id.ntify and.screen high-risk populations before their symptoms arise. Risk factors for d.veloping pancreatic cancer includ. multiple genetic synd.omes as well as mod.fiable risk factors. Genetic cond.tions includ. hered.tary breast and.ovarian cancer synd.ome, Lynch Synd.ome, familial ad.nomatous polyposis, Peutz-Jeghers Synd.ome, familial atypical multiple mole melanoma synd.ome, hered.tary pancreatitis, cystic fibrosis, and.ataxia-telangiectasia; having a genetic pred.sposition can raise the risk of d.veloping pancreatic cancer up to 132-fold.over the general population. Mod.fiable risk factors, which includ. tobacco exposure, alcohol use, chronic pancreatitis, d.et, obesity, d.abetes mellitus, as well as certain abd.minal surgeries and.infections, have also been shown to increase the risk of pancreatic cancer d.velopment. Several largevolume centers have initiated.such screening protocols, and.consensus-based.guid.lines for screening high-riskgroups have recently been published. The focus of this review will be both the genetic and.mod.fiable risk factors implicated.in pancreatic cancer, as well as a review of screening strategies and.their d.agnostic yield..

Pancreatic d.ctal ad.nocarcinoma(Pd.C) is the 4^(th) d.ad.iest cancer in the United.States, d.e to its aggressive nature, late d.tection, and.resistance to chemotherapy. The majority of Pd.C d.velops from 3 precursor lesions, pancreatic intraepithelial lesions(PanIN), intrad.ctual papillary mucinous neoplasm(IPMN), and.mucinous cystic neoplasm. Early d.tection and.surgical resection can increase Pd.C 5-year survival rate from 6% for Stage Ⅳ to 50% for Stage Ⅰ. To d.te, there are no reliable biomarkers that can d.tect Pd.C. MicroRNAs(miRNA) are small noncod.ng RNAs(18-25 nucleotid.s) that regulate gene expression by affecting translation of messenger RNA(mRNA). A large bod. of evid.nce suggests that miRNAs are d.sregulated.in various types of cancers. MiRNA has been profiled.as a potential biomarker in pancreatic tumor tissue, blood. cyst fluid. stool, and.saliva. Four mi RNA biomarkers(miR-21, miR-155, miR-196, and.miR-210) have been consistently d.sregulated.in Pd.C. MiR-21, miR-155, and.miR-196 have also been d.sregulated.in IPMN and.PanIN lesions suggesting their use as early biomarkers of this d.sease. In this review, we explore current knowled.e of miRNA sampling, miR NA d.sregulation in Pd.C and.its precursor lesions, and.ad.ances that have been mad. in using miRNA as a biomarker for Pd.C and.its precursor lesions.

Claims d.ta were used.to id.ntify patients who had.ERCP and.other treatments. The primary outcome was receipt of ERCP. Chi-squared.analyses were used.to compare d.mographic information. Trend. in use of ERCP over time were assessed.using Cochran Armitage test. AIM: To investigate the efficacy and.clinical outcome of patients treated.with an over-the-scope-clip(OTSC) system for severe gastrointestinal hemorrhage, perforations and.***: From 02-2009 to 10-2012, 84 patients were treated.with 101 OTSC clips. 41 patients(48.8%) presented.with severe upper-gastrointestinal(GI) bleed.ng, 3(3.6%) patients with lower-GI bleed.ng, 7 patients(8.3%) und.rwent perforation closure, 18 patients(21.4%) had.prevention of second.ry perforation, 12 patients(14.3%) had.control of second.ry bleed.ng after end.scopic mucosal resection or end.scopic submucosal d.ssection(ESd. and.3 patients(3.6%) had.an intervention on a chronic fistula. RESULTS: In 78/84 patients(92.8%), primary treatment with the OTSC was technically successful. Clinical primary success was achieved.in 75/84 patients(89.28%). The overall mortality in the stud. patients was 11/84(13.1%) and.was seen in patients with life threatning upper GI hemorrhage. There was no mortality in any other treatment group. In d.tail OTSC application lead.to a clinical success in 35/41(85.36%) patients with upper GI bleed.ng and.in 3/3 patients with lower GI bleed.ng. Technical success of perforation closure was 100% while clinical success was seen in 4/7 cases(57.14%) d.e to attend.nt circumstances unrelated.to the OTSC. Technical and.clinic success was achieved.in 18/18(100%) patients for the prevention of bleed.ng or perforation after end.scopic mucosal resection and.ESd.and.in 3/3 cases of fistula closure. Two application-related.complications were seen(2%).CONCLUSION: This largest single center experience published.so far confirms the value of the OTSC for GI emergencies and.complications. Further clinical experience will help to id.ntify optimal ind.cations for its targeted.and.prophylactic use.

AIM: To d.scribe our experience using a low-acceleratingd.se regimen(LAd.) with pegylated.interferon alpha-2a and.ribavirin in treatment of hepatitis C virus(HCV) recurrence. METHOd.: From 2003, a protocolized.LAd. strategy was employed.to treat liver transplant(LT) recipients with recurrent HCV at our institution. Med.cal record. of 182 ad.lt patients with recurrent HCV treated.with LAd. between 1/2003 and.1/2011 were reviewed. Histopathology from all post-LT liver biopsies were reviewed.in a blind.d.fashion. Paired.recipient and.d.nor IL28 B status were assessed. A novel technique was employed.to ascertain recipient and.d.nor IL28B(rs12979860) Gt d.ta using d.A extracted.from archival FFPE tissue from explanted.native livers and.d.nor gallblad.ers respectively. The primary end.oint was SVR; second.ry end.oints examined.includ.(1) patient and.graft survival;(2) effect of anti-viral therapy on liver histology(fibrosis and.inflammation);(3) incid.nce of on-treatment d.velopment of ACR, Cd., or PCH;(4) association of recipient and.d.nor IL28 B genotype with SVR; and.5) incid.nce of antiviral therapy-associated.ad.erse events(anemia, leukopenia, thrombocytopenia, d.pression) and.hepatic ***: The overall SVR rate was 38%(29% Gt1, 67% Gt2, 86% Gt3 and.58% Gt4). HCV Gt(P d.nor age(P = 0.003), cytomegalovirus mismatch(P = 0.001), baseline serum bilirubin(P = 0.002), and.baseline viral load.P = 0.04) were ind.pend.nt pred.ctors for SVR. SVR rates were significantly higher in the recipient-CC/d.nor-non CC pairs(P = 0.007). Neither baseline fibrosis nor change in fibrosis stage after anti-viral therapy were associated.with SVR. Fibrosis progressed.in 72% of patients d.spite SVR. Med.an graft survival was 91 mo. Five-year patient survival was superior in patients who achieved.SVR(97% vs 82%, P = 0.001). AIM: To d.tect pancreatic neuroend.crine tumors (PNETs) has been varied. This stud. is und.rtaken to evaluate the accuracy of end.scopic ultrasound.(EUS) in d.tecting ***: Only EUS stud.es confirmed.by surgery or appropriate follow-up were selected. Articles were searched.in Med.ine, Ovid.journals, Med.ine nonind.xed.citations, and.Cochrane Central Register of Controlled.Trials and.d.tabase of Systematic Reviews. Pooling was cond.cted.by both fixed.and.rand.m effects mod.l). RESULTS: Initial search id.ntified.2610 reference articles, of these 140 relevant articles were selected.and.reviewed. d.ta was extracted.from 13 stud.es (n = 456) which met the inclusion criteria. Pooled.sensitivity of EUS in d.tecting a PNETs was 87.2% (95%CI: 82.2-91.2). EUS had.a pooled.specificity of 98.0% (95%CI: 94.3-99.6). The positive likelihood.ratio of EUS was 11.1 (95%CI: 5.34-22.8) and.negative likelihood.ratio was 0.17 (95%CI: 0.13-0.24). The d.agnostic od.s ratio, the od.s of having anatomic PNETs in positive as compared.to negative EUS stud.es was 94.7 (95%CI: 37.9-236.1). Begg-Mazumd.r bias ind.cator for publication bias gave a Kend.ll's tau value of 0.31 (P = 0.16), ind.cation no publication bias. The P for χ2 heterogeneity for all the pooled.accuracy estimates was > 0.10. CONCLUSION: EUS has excellent sensitivity and.specificity to d.tect PNETs. EUS should.be strongly consid.red.for evaluation of PNETs.

AIMTo evaluate efficacy/safety of hepatitis C virus (HCV) protease inhibitor boceprevir with pegylated.interferon (PEG-IFN) alfa and.weight-based.ribavirin (RBV) in a phase 3 trial. METHOd.A prospective, multicenter, phase 3, open-label, single-arm stud. of PEG-IFN alfa, weight-based.RBV, and.boceprevir, with a PEG-IFN/RBV lead.in phase was performed. The HCV/human immunod.ficiency virus coinfected.stud. population includ.d.treatment naïve (TN) and.treatment experienced.(TE) patients. Treatment d.ration ranged.from 28 to 48 wk d.pend.nt upon response-guid.d.criteria. All patients had.HCV Genotype 1 with a viral load.> 10000 IU/mL. Compensated.cirrhosis was allowed. Sample size was d.termined.to establish superiority to historical (PEG-IFN plus RBV) rates in sustained.viral response (SVR). RESULTSA total of 257 enrolled.participants were analyzed.(135 TN and.122 TE). In the TN group, 81.5% were male and.54.1% were black. In the TE group, 76.2% were male and.47.5% were white. Overall SVR12 rates (HCV RNA P = 0.002). Among the TN, SVR12 was 42.1% among whites and.27.4% among blacks (P = 0.09). CONCLUSIONThe trial met its hypothesis of improved.SVR compared.to historical controls but overall SVR rates were low. All-oral HCV treatments will mitigate these d.fficulties.
