Causes and Prognosis of Cases of Acute Obstructive Renal Failure Managed at the Donka National hemodialysis center

Introduction: Acute obstructive renal failure (AORF) is a frequent clinical situation, secondary to obstruction of the urinary excretory tract. Whatever the cause, urinary tract obstruction suddenly opposes glomerular filtration and is responsible for tubulointerstitial lesions. It accounts for 10% of acute renal failure (ARF). The aim of this study was to identify the causes and prognosis of cases of acute obstructive renal failure managed at the Centre National d’hémodialyse Donka. Material and Methods: This was a prospective descriptive study lasting 6 months, from September 1, 2022 to February 29, 2023. All patients undergoing haemodialysis for acute obstructive renal failure who agreed to participate in the study and whose medical records were complete were included. Results: During the course of the study, we registered 97 haemodialysis patients, including 20 cases (20.62%) of acute obstructive renal failure. The mean age of the patients was 57.8 ± 10.54 years, with a male predominance of 11 cases (55%) and a sex ratio of 1.22. The reasons for consultation were dominated by physical asthenia 11 cases (55%), lumbar pain 9 cases (50%), vomiting 6 cases (30%) and acute urine retention 6 cases (30%). Arterial hypertension 16 cases (80%) and urinary tract infection 10 cases (50%) were the most common antecedents. The etiologies of RAOI were dominated by lithiasis 10 cases (50%), neoplasia 6 cases (30%) and benign prostatic hypertrophy 3 cases (15%). mean creatinine was 1267.60 ± 710.76 μmol/l with extremes of 243 μmol/l and 2822 μmol/l, mean urea was 39.56 ± 18.36, hyperkalemia in 14 cases (70%) and hyponatremia in 8 cases (40%). After hemodialysis, 9 cases (45%) recovered renal function, 4 cases (20%) became chronic and 7 cases (35%) died. Conclusion: The frequency of AKI remains non-negligible in our department, and early detection and prompt management would considerably reduce the morbidity and mortality associated with this pathology.

Pleurisy in hemodialysis Patients: Epidemiological and Etiological Aspects at Donka National hemodialysis center

Introduction: Pleural effusion being a frequent complication in hemodialysis patients, its etiologies are diverse and the diagnosis is easy, based on clinical and radiological proofs. The main objective of this study was to describe the epidemiological and etiological profiles of pleurisy in hemodialysis patients at the National hemodialysis center of Donka National Hospital. Patients and methods: it was a retrospective, descriptive and analytical study that extended over a period of three years from January 1, 2017 to December 31, 2019. It concerned, among hemodialysis patients in the center during the study period, all those who had a confirmed pleurisy on chest x-ray. The variables were epidemiological (based on frequency, sex and age), clinical (based on history of the disease, physical examination looking for a reduction or elimination of vesicular murmur) and paraclinical (mainly radiological). Results: Among 286 patients undergoing hemodialysis in our center, pleural effusion was diagnosed in 35 or 12.24%. The average age of our patients was 52.22 years with ranges of 18 and 78 years. The sex ratio M/F was 2.5. Bilateral pleurisy was found in 51.43% of patients;unilateral right in 40% of cases and unilateral left in 8.57%. We observed 68.57% citrus yellow fluid and 31.43% sero haematic fluid. The bacteriology of the pleural fluid was positive in 62.86% against 37.14% negative. Tumor, tuberculosis and non-specific bacterial etiologies have been encountered. Conclusion: Pleurisy is therefore a frequent complication in hemodialysis patients at Donka National hemodialysis center. Etiological research is a major step for better management of these patients. Improving the technical platform of the laboratories should be an important contribution to this stage.

Case Report for Two Hemodialysed Patients Diagnosed for COVID-19 at the Donka National hemodialysis center

In December 2019, coronavirus disease (COVID-19) due to infection with the novel SARS-CoV-2 virus began to break out. hemodialysis patients are at high risk because of the co-morbidities. We report the clinical and biological characteristics of two patients who developed COVID-19 infection in our dialysis center in Donka National Hospital. None of the patients had contact with an infected person. The age was 38 and 54 years. The symptoms common to both patients were: fatigue, diarrhea, fever. Lymphopenia was present in both patients. None of them had a chest X-ray or chest scan because they were positive for the test before admission.

Background: Patients’ satisfaction is a relevant component in assessing healthcare services. The objective of this study was to study the level of satisfaction of chronic hemodialysis patients and the associated factors in Cotonou CNHU-HKM dialysis center. Patients and Methods: It was a cross-sectional, descriptive and analytical study conducted by administering a questionnaire on days off dialysis. It was carried out from 1st November 2015 to 1st January 2016 in CNHU-HKM Nephrology University Clinic of Cotonou. hemodialysis patients aged 18 years and above having given their informed consent were included in the study. Patients’ recruitment was comprehensive. Likert scale was used in assessing the level of satisfaction with 4 as “very satisfied” and 1 for “very dissatisfied”. Satisfaction was evaluated on the basis of Ware dimensions. The threshold for satisfaction was 50. Outcomes: Overall, 377 patients were included in the study. Respondents’ mean age was 51.5 ± 13.3 years with 1.37 as sex ratio. Arteriovenous fistula was used for 80.1% of hemodialysis patients. 77.7% of the patients underwent dialysis twice a week while 45.9% were administered a four-hour dialysis. The overall average proportion of “satisfied” was 76.5%. The level of satisfaction was 52% for healthcare environment, 61% for service delivery, 73.9% for healthcare accessibility, 76.1% for healthcare structure, 77.5% for healthcare management, 90.3% for quality of healthcare, 88.3% for interpersonal relationship and 93.2% for efficiency and continuum of healthcare. Factors associated with satisfaction included age (p = 0.02), vascular access (p = 0.04) and urea reduction ratio (p = 0.01). In addition, the degree of satisfaction of hemodialysis patients was not statistically associated with sex, marital status, educational level, history of the disease, age of dialysis, number of sessions per week, and duration per session. 目的采用潜在剖面分析的方法探讨临床护理人员心理负荷现状及其影响因素。方法于2023年3月一7月,以便利抽样法采用人口学资料调查表、领悟社会支持量表、简易应对方式问卷和NASA-任务负荷指数量表对我国5所三级医院的526名临床护士进行问卷调查。分别使用Mplus7.3和SPSS24.0软件进行潜在剖面分析和多元logistic回归分析,探讨心理负荷不同潜在剖面的影响因素。结果护理人员心理负荷总分为(69.57±16.93)分,可分为“低心理负荷-高自评”组(n=70,13.3%)、“中心理负荷”组(n=273,51.9%)和“高心理负荷-低自评”组(n=183,34.8%)3个潜在类型。在“高心理负荷-低自评”亚组中,工作年限≥5年、无子女、月收入≥6000元、健康状况差、近1年未接受过心理培训和遭受过工作场所暴力的护士比例高于其他亚组,组间差异具有统计学意义(P<0.05)。此外,logistic回归分析显示,与较高的心理负荷伴随的是消极的应对方式和较低的领悟社会支持。结论护士工作心理负荷可分为3个潜在类型。月收入、健康状况、心理培训、工作场所暴力、消极应对方式和领悟社会支持是心理负荷的影响因素。

Predictive Factors of Renal Failure in COVID-19 Patients at the Anti-COVID center in Lome, Togo

Background: Angiotensin-converting enzyme 2 has been identified as the receptor that allows the entry of SarsCov2 into the human cell. Its expression in the kidney is 100 times higher than in the lung;thus, making the kidney an excellent target for SarsCov2 infection manifesting as renal failure (RF). The objective of this study was to determine the predictive factors of RF during COVID-19 in the Togolese context. Patients and Methods: This was a retrospective descriptive and analytical study conducted at the Lomé Anti-COVID center including the records of patients hospitalized for COVID-19, of age ≥ 18 years and having performed a creatinemia. RF was defined by a GFR 2 calculated according to the MDRD formula. Patients were randomized into 2 groups according to GFRResults: 482 patients were selected for this study with a mean age of 58.02 years. Sixty-five percent of the patients were men, i.e., a sex ratio of 1.88. Fifty-two patients had RF, i.e., a frequency of 10.8%.There were 65% men (315 cases), for a sex ratio (M/F) of 1.88. Risk factors for renal failure in COVID-19 were age ≥ 65 years (ORa 2.42;CIa95% [1.17 - 4.95];p = 0.016), anemia (ORa 2.49;CIa95% [1.21 - 5.26];p = 0.015), moderate (ORa 13;CIa95% [2.30 - 2.44];p = 0.017), severe (ORa 26.2;CIa95% [4.85 - 4.93];p = 0.002) and critical (ORa 108;CIa95% [16.5 - 21.76];p Conclusion: Renal failure would therefore be related to the severity of COVID-19 and is the most formidable factor, conditioning the course of the disease and the patient’s vital prognosis.

Intradialytic Hypertension and Associated Factors in Chronic hemodialysis at the National Hemodidiadiasis center in Donka, Guinea

Introduction: Dialytic high blood pressure (DHBP), although often ignored, is now recognized as a recurring and persistent phenomenon in a subgroup of hemodialysized patients. Its occurrence is associated with an increased risk of hospitalization and death. The objective of the study was to determine the prevalence of intradialytic hypertension and the factors associated with it. Methods: Study was cross-sectional, monocentric, descriptive and analytical over a three-month period from April 22 to July 22, 2019. Included were all patients 18 years of age or older, chronic hemodialysis for at least three months, with intra-dialytic high blood pressure. The blood pressure machine used for the majority of patients was an electronic “OMRON” blood pressure monitor. Epidemiological, clinical, para clinical and dialysis parameters were studied. The data were collected, captured and analyzed using IBM SPSS Statistics Version 20 software. The factors associated with intradialytic high blood pressure were searched using a univariate logistic regression model. The significance threshold for all statistical tests has been set at 5%. Results: Of our 131 patients, 53 had intradialytic hypertension, a frequency of 40.5%. The time of (DHBP) occurrence was more frequent at the 3rd and 2nd hour, 94.34% and 86.79%, respectively. The average age of patients was 45.51 years with extremes ranging from 19 to 70 years. The average Systolic Blood Pressure (SBP) before dialysis was 148 mm Hg ?16.62 and the average Diastolic Blood Pressure (DBP) before dialysis was 88 mm Hg ?12.50. Pre-dialysis Blood Pressure—140/90 mm Hg was noted in 18 cases, or 34.0%. The intradialytic average SBP was 164 mm Hg ?17.25 with extremes of 121 to 202 mm Hg. The intradialytic average DBP was 92 mm Hg ?12.52 with extremes 67 to 124 mm Hg.
