护士心理负荷的影响因素:一项潜在剖面分析

目的采用潜在剖面分析的方法探讨临床护理人员心理负荷现状及其影响因素。方法于2023年3月-7月,以便利抽样法采用人口学资料调查表、领悟社会支持量表、简易应对方式问卷和NASA-任务负荷指数量表对我国5所三级医院的526名临床护士进行问卷调查。分别使用Mplus 7.3和SPSS 24.0软件进行潜在剖面分析和多元logistic回归分析,探讨心理负荷不同潜在剖面的影响因素。结果护理人员心理负荷总分为(69.57±16.93)分,可分为“低心理负荷-高自评”组(n=70,13.3%)、“中心理负荷”组(n=273,51.9%)和“高心理负荷-低自评”组(n=183,34.8%)3个潜在类型。在“高心理负荷-低自评”亚组中,工作年限≥5年、无子女、月收入≥6000元、健康状况差、近1年未接受过心理培训和遭受过工作场所暴力的护士比例高于其他亚组,组间差异具有统计学意义(P<0.05)。此外,logistic回归分析显示,与较高的心理负荷伴随的是消极的应对方式和较低的领悟社会支持。结论护士工作心理负荷可分为3个潜在类型。月收入、健康状况、心理培训、工作场所暴力、消极应对方式和领悟社会支持是心理负荷的影响因素。

BACKGROUNDMost cancer patients are accompanied by anemia, which will be more seriouswhen combined with end-stage renal disease (ESRD). At present, cancer-relatedanemia and renal anemia treatments mainly include erythropoiesis-stimulatingagents (ESAs), iron supplementation, and blood transfusion, but their effects areoften poor with several safety concerns. We have used roxadustat to treat anemiain a cancer patient with ESRD and achieved a successful outcome for the *** SUMMARYA 64-year-old man was diagnosed with right renal cancer (clear cell renal cellcarcinoma). He did not receive surgery or radiotherapy before admission. He wastreated with oral soltan (sunitinib malate) on April 18, 2017. During oral chemotherapy,he had numerous complications, including anemia, hypertension,thyroid hypofunction, skin pigment loss, and renal function deterioration. At last,he progressed to ESRD and began hemodialysis treatment. We initially treated thepatient with high-dose ESAs, iron supplementation, adequate dialysis, and evenblood transfusion, but his anemia did not improve. Roxadustat is a newlydeveloped drug for renal anemia treatment, but not for cancer-related anemia, letalone to treat anemia in cancer patients with ESRD. We prescribed oral roxadustatto the patient. After a period, his hemoglobin gradually increased. He did nothave obvious discomfort symptoms, and his tumor did not progress *** roxadustat could achieve good results in treating anemia in cancer patients with ESRD.

Objective: To explore the detection and clinical significance of immune function, inflammatory factors, Hcy, SF and β2-MG in elderly patients with chronic renal failure. Methods: We selected 95 elderly patients with chronic renal failure treated in our hospital from March 2016 to December 2017 as the observation group, and 100 healthy elderly patients were selected as the control group during the same period. The levels of immune parameters, inflammatory factors, Hcy, SF and β2-MG levels were compared and analyzed. Results: The levels of IgA in the observation group was significantly higher than that in the control group, while IgG, IgM, C3, and C4 were significantly lower than those in the control group. The levels of hs-CRP and IL-6 in the observation group were significantly higher than those in the control group, and the difference was statistically significant. the levels of Hcy, SF andβ2-MG in Observation group were significantly higher than that of in the control group, the difference was statistically significant. Conclusions: Elderly patients with CRF have low immune function, high inflammation level, high expression of Hcy, SF and β2-MG. The diagnosis of these indicators is of great significance for the early diagnosis, prevention and prognosis of elderly CRF patients.

Objective:To study the leptin receptor Gln223Arg locus polymorphism in patients with asthma and its correlation with the disease ***: The patients who were diagnosed with bronchial asthma in our hospital between July 2014 and April 2017 were selected as the asthma group for the study, and the healthy volunteers who received physical examination in our hospital during the same period were selected as the control group. The leptin receptor gene Gln223Arg locus polymorphism was determined, and the contents of inflammatory mediators and airway remodeling indexes in serum as well as the contents of immune cells in peripheral blood were ***: Eotaxin, Galectin-3, YKL-40, SAA, IL-33, ADAM33, NE and MMP9 contents in serum as well as Th17, Th9 and Tfh contents in peripheral blood of asthma group of patients with AA+AG genotype and GG genotype were significantly higher than those of control group whereas TIMP1 and TIMP2 contents in serum as well as Treg contents in peripheral blood were significantly lower than those of control group;Eotaxin, Galectin-3, YKL-40, SAA, IL-33, ADAM33, NE and MMP9 contents in serum as well as Th17, Th9 and Tfh contents in peripheral blood of asthma group of patients with AA+AG genotype were significantly higher than those of patients with GG genotype whereas TIMP1 and TIMP2 contents in serum as well as Treg content in peripheral blood were significantly lower than those of patients with GG ***: The substitution from leptin receptor Gln223Arg locus polymorphism A G replacement in patients with asthma can aggravate the inflammatory response and immune response disorder as well as airway remodeling.

OBJECTIVE: To explore the influence of acupoint exercise on anxiety, depression and sleep quality in hemodialysis patients, aiming to provide scientific basis for clinical diagnosis and treatment. METHODS: A total of 78 hemodialysis patients who were treated in our hospital from May 2015 to May 2018 were selected as subjects. All patients were divided into two groups according to the random number table method. Among them, 40 patients in the observation group were given acupoint operation intervention, and 38 patients in the control group were given routine care. The anxiety level(HAMA), depression level(HAMD), hopeful sleep quality(PSQI) level, and quality of life(SF-36) levels were compared between the two groups. RESULTS: The HAMA and HAMD scores of the two groups were not significantly different before intervention. However, the scores of HAMA and HAMD in the observation group were lower than those in the control group. The scores of the PSQI scale were higher than those in the control group, indicating that the sleeping quality of the observation group was better. And the SF-36 score was higher in the observation group than that of the control group, and the difference was statistically significant. CONCLUSION: Acupoint exercise plays an important role in improving anxiety and depression and quality of sleep in hemodialysis patients. It can effectively improve the quality of life of hemodialysis patients, and can be applied clinically to improve its therapeutic effect.

AIM To evaluate various schemes for paraquat poisoning and different variables that influence the outcome of acute paraquat *** In a cross-sectional study, the information about all cases of acute paraquat poisoning who were admitted to teaching hospitals affiliated to Shiraz University of Medical Sciences, in a five year period(September 2010 to September 2015) were evaluated. The variables included: Demographic data, medical assessment, therapeutic options, laboratory findings, and the outcomes. Data were analyzed using SPSS, version 22. Significant difference between groups was tested using t-test for continues outcomes and χ~2 test for categorical. The significance level was considered to be P hemodialysis during the first 6 h of intoxication, none of the patients had dialysis during this time. Immunosuppressive and corticosteroid medications were not administrated in adequate dosage in 31.1% and 60% of the patients, respectively. Ingestion of more than22.5 cc of paraquat and increase in creatinine level were the most important predictors of *** Treatment should start immediately for these ***, creating a clinical guideline according to the findings can have an impact on the treatment procedure which seems to be necessary.
