Objective:This study aimed to investigate the effects of TH/CTS/β-CD microspheres by oral administration on ovalbumin allergic asthma of a rat ***:Spray drying method was applied for preparing the TH/CTS/β-CD microspheres,which were used in treatment of asthmatic *** a predetermined time,the levels of eosinophil(Eos),protein,lactate dehydrogenase(LDH),aspartate transaminase(AST),glutamic pyruvic transaminase(GPT),and creatinine(Cr)were ascertained by automatic hematology and biochemical *** tissue histology was performed by hematoxylin-eosin ***:No significant differences were found about the contents of Eos in the blood,and the contents of Eos,protein and LDH in the bronchoalveolar lavage fluid between the microsphere-treated and dexamethasone-treated groups,but the levels were lower in both treated groups than in the model *** the microsphere-treated group,the levels of Cr,AST,and GPT in the serum of rats showed no significant differences compared with the normal *** on lung histopathological findings,the microspheres attenuated inflammatory cell infiltration and mucus hypersecretion compared with the model ***:This study demonstrated that TH/CTS/β-CD microspheres exerted an anti-inflammatory effect and could serve as a novel promising drug delivery system for asthma treatment.

癌症常见于老年人。初诊为恶性肿瘤的患者的中位年龄为66岁,癌症相关死亡的中位年龄为72岁[1]。老年人是癌症患病率增长最快的人群之一[2]。免疫治疗的发展,尤其是免疫检查点抑制剂(immune checkpoint inhibitors,ICIs)的问世,彻底改变了所有年龄层包括老年人在内的肿瘤患者的常规治疗方案。细胞毒性T淋巴细胞相关抗原4、程序性死亡受体1(programmed death receptor-1,PD-1)及其配体(programmed death-ligand 1,PD-L1)的抑制剂可改善多种肿瘤患者的总生存期(overall survival,OS)[3],且部分患者可获得持续缓解[4]。

AIM: To assess the utility and safety of single-operator cholangiopancreatoscopy(SOCPS) using the Spy Glass system in widespread clinical application for biliary and pancreatic ***: This study was a prospective case series conducted in 20 referral centers in Japan. There were 148 patients who underwent SOCPS; 124 for biliary diseases and 24 for pancreatic diseases. The attempted interventions were SOCPS examination, SOCPS-directed tissue sampling, and therapy for stone removal, among others. The main outcomes were related to the procedure success rate in terms of visualizing the target lesions, SOCPS-directed adequate tissue sampling, and complete stone removal. RESULTS: A total of 148 patients were enrolled for the diagnosis of indeterminate biliary and pancreatic lesions or treatment of biliary and pancreatic disease. The overall procedure success rate of visualizing the target lesions was 91.2%(135/148). The overall procedural success rates of visualizing the target lesions of diagnostic SOCPS in the bile duct and pancreatic duct were 95.5%(84/89) and 88.2%(15/17), respectively. Diagnosis: the overall adequate tissue for histologic examination was secured in 81.4% of the 86 patients who underwent biopsy under SOCPS(bile duct, 60/75, 80.0%; pancreatic duct, 10/11, 90.9%). The accuracy of histologic diagnosis using SOCPS-directed biopsies in indeterminate bile duct lesions was 70.7%(53/75). In the pancreatic duct, the accuracy of SOCPS visual impression of intraductal papillary mucinous neoplasm was 87.5%(14/16). Stone therapy: complete biliary and pancreatic stone clearance combined with SOCPS-directed stone therapy using electrohydraulic lithotripsy or laser lithotripsy was achieved in 74.2%(23/31) and 42.9%(3/7) of the patients, respectively. Cost of allogeneic blood transfusion

Blood is a scarce and costly resource to society. Therefore, it is important to understand the costs associated with blood, blood components, and blood transfusions. Previous studies have attempted to account for the cost of blood but, because of different objectives, perspectives, and methodologies, they may have underestimated the true(direct and indirect) costs associated with transfusions. Recognizing these limitations, a panel of experts in blood banking and transfusion medicine gathered at the Cost of Blood Consensus Conference to identify a set of key elements associated with whole blood collection, transfusion processes, follow-up, and to establish a standard methodology in estimating costs. Activity-based costing(ABC), the proposed allinclusive reference methodology, is expected to produce standard and generalizable estimates of the costof blood transfusion, and it should prove useful to payers, buyers, and society(all of whom bear the cost of blood). In this article, we argue that the ABC approach should be adopted in future cost-of-transfusion studies. In particular, we address the supply and demand dilemma associated with blood and blood components; evaluate the economic impact of transfusion-related adverse outcomes on overall blood utilization; discuss hemovigilance as it contributes not to the expense, but also the safety of transfusion; review previous cost-oftransfusion studies; and summarize the ABC approach and its utility as a methodology for estimating transfusion costs.

Red cell distribution width in anemic patients undergoing transcatheter aortic valve implantation

AIM: To determine the impact of red blood cell distribution width on outcome in anemic patients undergoing transcatheter aortic valve implantation(TAVI).METHODS: In a retrospective single center cohort study we determined the impact of baseline red cell distribution width(RDW) and anemia on outcome in 376 patients with aortic stenosis undergoing TAVI. All patients were discussed in the institutional heart team and declined for surgical aortic valve replacement due to high operative risk. Collected data included patient characteristics, imaging findings, periprocedural in hospital data, laboratory results and follow up data. Blood samples for hematology and biochemistry analysis were taken from every patient before and at fixed intervals up to 72 h after TAVI including blood count and creatinine. Descriptive statistics were used for patient's characteristics. KaplanMeier survival curves were used for time to event outcomes. A recursive partitioning regression and classification was used to investigate the association between potential risk factors and outcome ***: Mean age in our study population was 81 ± 6.1 years. Anemia was prevalent in 63.6%(n = 239) of our patients. Age and creatinine were identified as risk factors for anemia. In our study population, anemia per se did influence 30-d mortality but did not predict longterm mortality. In contrast, a RDW > 14% showed to be highly predictable for a reduced short- and longterm survival in patients with aortic valve disease after TAVI ***: Age and kidney function determine the degree of anemia. The anisocytosis of red blood cells in anemic patients supplements prognostic information in addition to that derived from the WHO-based definition of anemia.
