AIM:Clinical application of human hepatocytes (HC) is hampered by the progressive loss of growth and differentiation in vitro. The object of the study was to evaluate the effect of a biphasic culture technique on expression and activation of growth factor receptors and differentiation of human adult HC. METHODS: Isolated HC were sequentially cultured in a hormone enriched differentiation medium (DM) containing nicotinamide, insulin, transferrin, selenium, and dexame-thasone or activation medium (AM) containing hepatocyte growth factor (HGF), epidermal growth factor (EGF), and granulocyte-macrophage colony-stimulating factor (GMCSF). Expression, distribution and activation of the HC receptors (MET and EGFR) and the pattern of characteristic cytokeratin (CK) filaments were measured by fluorometry, confocal microscopy and Western blotting. RESULTS: In the biphasic culture system, HC underwent repeated cycles of activation (characterized by expression and activation of growth factor receptors) and re-differentiation (illustrated by distribution of typical filaments CK-18 but low or absent expression of CK-19). In AM increased expression of MET and EGFR was associated with receptor translocation into the cytoplasm and induction of atypical CK-19. In DM low expression of MET and EGFR was localized on the cell membrane and CK-19 was reduced. Receptor phosphorylation required embedding of HC in collagen type I gel. CONCLUSION: Control and reversible modulation of growth factor receptor activation of mature human HC can be accomplished in vitro, when defined signals from the extracellular matrix and sequential growth stimuli are provided. The biphasic technique helps overcome dedifferentiation, which occurs during continuous stimulation by means of growth factors.

Management of occluded self-expanding biliary metal stents in malignant biliary disease

Background: Occlusion of self-expanding metal stents(SEMS) in malignant biliary obstruction occurs in up to 40% of patients. This study aimed to compare the different techniques to resolve stent occlusion in our collective of ***: Patients with malignant biliary obstruction and occlusion of biliary metal stent at a tertiary referral endoscopic center were retrospectively identified between April 1, 1994 and May 31, 2014. The clinical records were further analyzed regarding the characteristics of patients, malignant strictures, SEMS,management strategies, stent patency, subsequent interventions, survival time and case ***: A total of 108 patients with biliary metal stent occlusion were identified. Seventy-nine of these patients were eligible for further analysis. Favored management was plastic stent insertion in 73.4% patients. Second SEMS were inserted in 12.7% patients. Percutaneous transhepatic biliary drainage and mechanical cleansing were conducted in a minority of patients. Further analysis showed no statistically significant difference in median overall secondary stent patency(88 vs. 143 days, P = 0.069), median survival time(95 vs. 192 days, P = 0.116), median subsequent intervention rate(53.4% vs. 40.0%, P = 0.501)and median case charge(€5145 vs. €3473, P = 0.803) for the treatment with a second metal stent insertion compared to plastic stent insertion. In patients with survival time of more than three months,significantly more patients treated with plastic stents needed re-interventions than patients treated with second SEMS(93.3% vs. 57.1%, P = 0.037).Conclusions: In malignant biliary strictures, both plastic and metal stent insertions are feasible strategies for the treatment of occluded SEMS. Our data suggest that in palliative biliary stenting, patients especially those with longer expected survival might benefit from second SEMS insertion. Careful patient selection is important to ensure a proper decision for either management

Non-Uniqueness of Einstein’s Special Relativity, and the Inconclusiveness of High Energy (Relativistic) Physics

In this paper, we present a new form of “special relativity” (BSR), which is isomorphic to Einstein’s “special relativity” (ESR). This in turn proves the non-uniqueness of Einstein’s “special relativity” and implies the inconclusiveness of so-called “relativistic physics”. This work presents new results of principal significance for the foundations of physics and practical results for high energy physics, deep space astrophysics, and cosmology as well. The entire exposition is done within the formalism of the Lorentz SL(2C) group acting via isometries on real 3-dimensional Lobachevskian (hyperbolic) spaces L3 regarded as quotients SL(2C)/SU(2). We show via direct calculations that both ESR and BSR are parametric maps from Lobachevskian into Euclidean space, namely a gnomonic (central) map in the case of ESR, and a stereographic map in the case of BSR. Such an identification allows us to link these maps to relevant models of Lobachevskian geometry. Thus, we identify ESR as the physical realization of the Beltrami-Klein (non-conformal) model, and BSR as the physical realization of the Poincare (conformal) model of Lobachevskian geometry. Although we focus our discussion on ball models of Lobachevskian geometry, our method is quite general, and for instance, may be applied to the half-space model of Lobachevskian geometry with appropriate “Lorentz group” acting via isometries on (positive) half space, resulting yet in another “special relativity” isomorphic with ESR and BSR. By using the notion of a homotopy of maps, the identification of “special relativities” as maps from Lobachevskian into Euclidean space allows us to justify the existence of an uncountable infinity of hybrid “special relativities” and consequently an uncountable infinity of “relativistic physics” built upon them. Comparison of perioperative outcomes between laparoscopic and open partial splenectomy in children and adolescents

Self-expanding metal stents for the palliation of malignant gastric outlet obstruction in patients with peritoneal carcinomatosis

AIM To evaluate the efficacy of self-expanding metal stents(SEMS) for the palliation of malignant gastric outlet obstruction in patients with and without peritoneal carcinomatosis(PC).METHODS We performed a retrospective analysis of 62 patients who underwent SEMS placement for treatment of malignant gastroduodenal obstruction at our hospital over a six-year period. Stents were deployed through the scope under combined fluoroscopic and endoscopic guidance. Technical success was defined as successful stent placement and expansion. Clinical success was defined as an improvement in the obstructive symptoms and discharge from hospital without additional parenteral nutrition. According to carcinomatosis status, patients were assigned into groups with or without evidence of peritoneal *** In most cases, obstruction was caused by pancreatic(47%) or gastric cancer(23%). Technical success was achieved in 96.8%(60/62), clinical success in 79%(49/62) of all patients. Signs of carcinomatosis were identified in 27 patients(43.5%). The diagnosis was confirmed by pathology or previous operation in 7 patients(11.2%) and suspected by CT, MRI or ultrasound in 20 patients(32.2%). Presence of carcinomatosis was associated with a significantly lower clinical success rate compared to patients with no evidence of peritoneal disease(66.7% vs 88.6%, P = 0.036). There was no significant difference in overall survival between patients with or without PC(median 48 d vs 70 d, P = 0.21), but patients showed significantly longer survival after clinical success of SEMS placement compared to those experiencing clinical failure(median 14.5 d vs 75 d, P = 0.0003).CONCLUSION Given the limited therapeutic options and a clinical success rate of at least 66.7%, we believe that SEMS are a reasonable treatment option in patients with malignant gastric outlet obstruction with peritoneal carcinomatosis.

Endoscopic neurosurgery has not yet reached the safety and applicability of microsurgery. Endo-neuro-sonography is a new technique aimed at making endoscopy safer by real-time imaging and navigation capacity (brain-radar). The endo-neurosonographic image is a sonographic scan at the tip of the endoscope (mini-CT) providing additional information to the endoscopic view. Endo-Neuro-Sonography is the first book on this new topic, presenting techniques and equipment, anatomical features, and the first clinical series. Schematic drawings, tables and 237 figures (122 in color) offer a precise overview of this technique. The final chapter is dealing with future concepts on minimally invasive neurosurgery and specific reference to ergonomics in neurosurgery.

CT-Virtuelle Endoskopie: Die Darstellungsfhigkeit der Strukturen und der Laesionen im nasalen Raum

Bilddarstellungsfahigkeit der virtuellen Endoskopie und seiner klinischen Anwendung im Vergleich mit fiberoptischer nasaler Endoskopie auszuwerten. 11 Patienten (22 nasale Rsume) wurden durch virtuelle Endoskopie uberpruft, nachdem axiale Abtastung des Spiral CT durchgefuhrt worden war. Virtuelle Endoskopie wurde durch Exploreranwendungspaket in eine Workstation durchgefuhrt. Bei 9 Patienten (18 nasale Raume) wurde fiberoptische Endoskopie durchgemacht. Virtuelle Endoskopie konnte anatomische Strukturen im nasalen Raum, septale Ab- weichung, die nasale Verengung und Obstruktion, turbinate Hyperplasie und pathologische Massen, die groBer als 3 Millimeter im Durchmesser stud, nachweisen. Jedoch konnen die 'falschen Adhasionen' in der virtuellen Endoskopie erscheinen. Die Hauptbeschrankung der virtuellen Endoskopie war Unfahigkeit, Mucosa auszuwerten und histologische Diagnose in unterstutzen. Virtuelle Endoskopie ist eine neue, non-invasive Methode fur das Demonstrieren der anatomischen Strukturen und der Krankheiten im nasalen Raum. Ihre Bilddarstellungsfahigkeit ist mit fiberoptischer nasaler Endoskopie vergleichbar. Sie ist eine methodische Erganzung der fiberoptischen nasalen Endoskopie.

Cerebrospinal Fluid Magnesium Level in Different Neurological Disorders

Magnesium (Mg) is an essential cofactor for many enzymatic reactions, especially those involved in energy metabolism. The aim of the present study was to determine the CSF concentration of Mg in various neurological disorders (n = 72) and in healthy subjects (n = 75). The control group included 35 males and 40 females, aged 16-89 years (mean age 53 years) who were subjected to a lumbar puncture for diagnostic reasons. The CSF examination was normal mainly as concerns the macroscopically examination, the leukocyte count and the protein level. The determination of Mg was performed with xylidyl-blue photometry. Our normal CSF Mg mean value was 0.97 ± 0.08 mmol/l (range 0.6-1.4 mmol/l). In the group of patients (n = 11) with convulsive seizures a slightly but significantly lower Mg were revealed (0.92 ± 0.03 mmol/l;p = 0.001;paired two-tailed Student’s t-tests). No statistically significant change of CSF Mg levels was noted in patients suffering from alcohol withdrawal syndrome, multiple sclerosis or Bell’s palsy. Our results indi-cate that magnesium deficiency may play a role for seizure manifestation even in patients with a moderate low Mg without neurological signs. Low CSF magnesium is associated with epilepsy, further studies may determine the influ-ence of anti-epileptic drug therapy on CSF magnesium levels.
