Bullous pemphigoid associated with acquired hemophilia A: A case report

BACKGROUND Acquired hemophilia A (AHA) is a rare and potentially severe bleeding disordercaused by circulating autoantibodies against factor Ⅷ (FⅧ). In approximately50% of the patients, the condition is associated with autoimmune diseases,cancers, medication use, pregnancy, and the post-partum period. Bullous pemphigoid(BP) is a chronic autoimmune subepidermal blistering disease associatedwith tissue-bound and circulating autoantibodies against BP antigens 180 (BP180)and 230 (BP230). AHA-associated BP has a high mortality rate;hence, the understandingof this disease must *** SUMMARY A 69-year-old man presented with erythema, blisters, blood blisters, and crustsaccompanied by severe pruritus for more than 20 days, and ecchymosis andswelling on his left upper arm for 3 days. Pathological examination revealed asubepidermal blister that contained eosinophils. Laboratory tests showed that theBP180 autoantibody levels had increased, isolated activated partial thromboplastintime was notably prolonged (115.6 s), and coagulation FⅧ activity wasextremely low (< 1.0%). Furthermore, the FⅧ inhibitor titer had greatlyincreased (59.2 Bethesda units). Therefore, the patient was diagnosed as having BP associated with AHA, prescribed 0.05% topical halometasone cream, and transferred to a higher-level hospitalfor effective treatment;however, he died after 2 *** AHA associated BP is rare, dangerous, and has a high mortality rate. Therefore, its timely diagnosis and effectivetreatment are necessary.

T cell interactions with microglia in immune-inflammatory processes of ischemic stroke

Anti-breast cancer effect of quercetin nanoparticles based on MOFs for chemo-phototherapy and heat shock proteins inhibition

Phototherapy, including photodynamic therapy(PDT) and photothermal therapy (PTT), is a promisingmethod of tumor treatment, which has the advantages ofstrong specificity and small side effects. However, PDT willaggravate the hypoxic environment in tumor cells, and PTTwill lead to heat-resistant reaction, both of which will make theorganism quickly produce heat shock proteins (HSPs) toprotect itself, thus seriously hindering the therapeuticefficiency of phototherapy. As a natural HSPs inhibitor,quercetin (QR) provides a feasible way to solve this ***, a kind of metal organic framework (MOF), Fe-TCPP(FT), was used as a delivery material and photosensitizer tobuild a chemo-phototherapy platform, and QR was used as achemotherapy drug and HSPs inhibitor. On this basis, the nanoparticles (NPs) were modified with PEG-CPP30 (a cellpenetratingpeptide that specifically recognizes human breast cancer MCF-7 cells), which showed excellent targetingcapacity and biocompatibility. The combination of chemotherapy and phototherapy significantly improved the anti-cancereffect. At the same time, NPs also showed prominent anti-HSPs effect. The effective connection of the two played acomplementary role in the process of tumor treatment, which provided a new strategy for the accurate and efficienttreatment of tumors in the future.

Real-world evidence on the efficacy and safety of vonoprazanamoxicillin dual therapy for Helicobacter pylori treatment in elderly patients

BACKGROUND A dual therapy regimen containing amoxicillin is a common treatment option for the eradication of Helicobacter pylori(***).While substantial research supports the efficacy and safety of vonoprazan and amoxicillin(VA)dual therapy in the general population,there is still a lack of studies specifically focusing on its safety in elderly *** To evaluate efficacy and safety of VA dual therapy as first-line or rescue treatment for *** in elderly *** As a real-world retrospective study,data were collected from elderly patients aged 60 years and above who accepted VA dual therapy(vonoprazan 20 mg twice daily+amoxicillin 1000 mg thrice daily for 14 days)for *** eradication in the Department of Gastroenterology at Peking university First Hospital between June 2020 and January *** status was evaluated by^(13)C-urease breath test 6 weeks after *** adverse events(AEs)during treatment were *** In total,401 cases were ***-one cases were excluded due to loss to follow-up,lack of re-examination,or unwillingness to take *** total of 380 included cases comprised 250 who received VA dual therapy as first-line treatment and 130 who received VA dual therapy as rescue *** was successfully eradicated in 239 cases(95.6%)in the first-line treatment group and 116 cases(89.2%)in the rescue treatment *** overall incidence of AEs was 9.5%for both ***,9.2%of patients experienced an AE in the first-line treatment group and 10.0%in the rescue treatment *** patients discontinued treatment due to AE,with a discontinuation rate of 1.3%.No serious AE *** The VA dual therapy regimen as a first-line treatment and a rescue therapy was effective and safe for elderly patients aged 60 and older.
