Beneficiaries’ Willingness to Pay for Resuscitation Provided by Ambulance Attendants: A Survey Using the Contingent Valuation Approach

Background: Japanese emergency medical services (EMS) can be used by anyone for free. Recently, EMS usage has increased;the increased costs and the prolonged time for ambulance transport have become recent social problems. Objective: We surveyed the willingness to pay (WTP) for resuscitation provided by EMS. Methods: In November 2011, men and women (3160) aged 20 - 59 years were asked to assume that they were experiencing a cardiopulmonary arrest (CPA), and their WTP for EMS services was assessed in the following three situations: Case A, ambulance transport alone;Case B, chest compression in addition to ambulance transport;and Case C, artificial ventilation through chest compression and intratracheal intubation, in addition to ambulance transport. We calculated the mean WTP for each case. Results: The WTP for Case A, B, and C were ￥6,696 ($65.0), ￥16,081 ($156.1), and ￥27,505 ($267.0), respectively. The WTP for Case B was significantly higher in respondents aged 40 - 59 years compared to those aged 20 - 39 years. The WTP for case B and C were significantly higher in males compared to females. WTP was significantly lower in students than it was in private employees. Although women’s intention to pay was higher than that of men, their WTP was lower than that of men. Public employees’ and students’ intention to pay was significantly lower than that of private employees. Conclusions: Our study provides information about the optimal fee for EMS, which will be useful for discussions on the feasibility of introducing a fee for EMS in Japan.

Soft tissue swelling incidence using demineralized bone matrix in the outpatient setting

AIM To assess use of demineralized bone matrix(DBM) use in anterior cervical discectomy and fusion(ACDF) in outpatient *** One hundred and forty-five patients with prospectively collected data undergoing single and two level ACDF with DBM packed within and anterior to polyetheretherketone(PEEK) cages. Two groups created, Group 1(75) outpatients and control Group 2(70) hospital patients. Prevertebral soft tissue swelling(PVSTS) was measured anterior to C2 and C6 on plain lateral cervical radiographs preoperatively and one week postoperatively and fusion assessed at two years. RESULTS There was no intergroup significance between preoperative and postoperative visual analogue scales(VAS)and neck disability index(NDI) scores between Group 1 and 2. Mean preoperative PVSTS in Group 1 was 4.7 ± 0.2 mm at C2 level and 11.1 ± 0.5 at C6 level compared to Group 2 mean PVSTS of 4.5 ± 0.5 mm and 12.8 ± 0.5, P = 0.172 and 0.127 respectively. There was no radiographic or clinical evidence of adverse reaction noted. In Group 1 mean postoperative PVSTS was 5.5 ± 0.4 mm at C2 and 14.9 ± 0.6 mm at C6 compared Group 2 mean PVSTS was 4.9 ± 0.3 mm at C2 and 14.8 ± 0.5 mm at C6, P = 0.212 and 0.946 respectively. No significant increase in prevertebral soft tissue space at C2 and C6 level *** ACDF with adjunct DBM packed PEEK cages showed a statistical significant intragroup improvement in VAS neck pain scores and NDI scores(P = 0.001). There were no reported serious patient complications; post-operative radiographs demonstrated no significant difference in prevertebral space. We conclude that ACDF with DBMpacked PEEK cages can be safely done in an ASC with satisfactory outcomes.

Areas at high-altitude,annually attract millions of tourists,skiers,trekkers,and *** not adequately prepared and not considering certain ascent rules,a considerable proportion of those people will suffer from acute mountain sickness(AMS)or even from life-threatening high-altitude cerebral(HACE)or/and pulmonary edema(HAPE).Reduced inspired oxygen partial pressure with gain in altitude and consequently reduced oxygen availability is primarily responsible for getting sick in this *** acclimatization by slowly raising the hypoxic stimulus(e.g.,slow ascent to high altitude)and/or repeated exposures to altitude or artificial,nor-mobaric hypoxia will largely prevent those *** physiological mechanisms of acclimatization and pathophysiological mechanisms of high-altitude diseases,knowledge of symptoms and signs,treatment and prevention strategies will largely contribute to the risk reduction and increased safety,success and enjoyment at high ***,this review is intended to provide a sound basis for both physicians counseling high-altitude visitors and high-altitude visitors themselves.

Per-and polyfluoroalkyl substances(PFASs)can disrupt lipid metabolism,and changes in cord blood fatty acid composition have been observed in small *** evidence suggests that exposure to PFASs during pregnancy is linked to decreased newborn size,although the evidence is not *** modifying effect of fatty acids on the associations of gestational PFAS exposure with newborn size is still *** we show that the nutritional status of the fetus,as indicated by the level of fatty acids in the cord blood,mitigates the adverse effects of gestational PFAS exposure on the size of the *** study confirms the adverse developmental effects of PFASs and identifies emerging short-chain PFASs as the primary drivers of reduced newborn size,despite their lower exposure burden compared to legacy ***,we find the protective role of cord blood fatty acids,suggesting potential strategies for mitigating the detrimental effects of emerging environmental exposures on human *** findings provide new evidence of the potential toxicity of emerging PFASs and call for further toxicity evaluations of these pollutants for regulatory *** studies should consider the complex interaction between exposure and nutrition within the human body,particularly during the first thousand days of life,to promote lifelong health.

Background:Cholangiocarcinoma(CCA)is a fatal bile duct cancer associated with infection by the liver fluke,Opisthorchis viverrini,in the lower Mekong *** public health interventions have focused on reducing exposure to ***,but incidence of CCA in the region remains *** this may indicate the inefficacy of public health interventions due to complex social and cultural factors,it may further indicate other risk factors or interactions with the parasite are important in pathogenesis of *** systematic review aims to provide a comprehensive analysis of described risk factors for CCA in addition to *** to guide future integrative *** body:We searched five international and seven Thai research databases to identify studies relevant to risk factors for CCA in the lower Mekong *** studies were assessed for risk of bias and quality in terms of study design,population,CCA diagnostic methods,and statistical *** final 18 included studies reported numerous risk factors which were grouped into behaviors,socioeconomics,diet,genetics,gender,immune response,other infections,and treatment for *** risk factors were reported by two or more studies and were assessed with random effects models during *** meta-analysis indicates that the combination of alcohol and smoking(OR=11.1,95%CI:5.63-21.92,P<0.0001)is most significantly associated with increased risk for CCA and is an even greater risk factor than *** *** analysis also suggests that family history of cancer,consumption of raw cyprinoid fish,consumption of high nitrate foods,and praziquantel treatment are associated with significantly increased *** risk factors may have complex relationships with the host,parasite,or pathogenesis of CCA,and many of these risk factors were found to interact with each other in one or more ***:Our findings suggest that a complex variety of risk factors in ad

Hospitalizations and healthcare costs associated with serious, non-lethal firearm-related violence and injuries in the United States, 1998-2011

Objective:To describe the prevalence,trends,correlates,and short-term outcomes of inpa-tient hospitalizations for firearm-related injuries(FRIs)in the United States between 1998 and ***:We conducted a retrospective,cross-sectional analysis of inpatient hospitalizations using data from the Nationwide Inpatient *** addition to generating national prevalence estimates,we used survey logistic regression to calculate odds ratios(ORs)and 95%confidence intervals(CIs)for the association between FRIs and patient/hospital-level ***-ral trends were estimated and characterized using joinpoint ***:There were 10.5 FRIs(95%CI:9.2-11.8)per 10,000 non-maternal/neonatal inpatient hospitalizations,with assault accounting for 60.1%of FRIs,followed by unintentional/accidental(23.0%)and intentional/self-inflicted FRIs(8.2%).The highest odds of FRIs,particularly FRIs associated with an assault,was observed among patients 18-24 years of age,patients 14-17 years of age,patients with no insurance/self-pay,and non-Hispanic *** mean inpatient length of stay for FRIs was 6.9 days;however,4.7%of patients remained in the hospital over 24 days and 1 in 12 patients(8.2%)died before *** mean cost of an inpatient hospitalization for a FRI was$22,149,which was estimated to be$679 million annually;approximately two-thirds of the annual cost(64.7%)was for assault($439 million).Conclusion:FRIs are a preventable public health issue which disproportionately impacts younger generations,while imposing significant economic and societal burdens,even in the ab-sence of *** of FRIs should be considered a priority in this era of healthcare cost containment.

Polar ships need to meet stringent safety and environmental *** those ships are classified by diferent ice classes based on ice operation ***,the polar ships are also trapped by severe ice condition due to low propulsion ***,it is a realistic question to design the appropriate minimum propulsion power for ice *** paper focuses on the ice resistance and its related propulsion power for the ships with polar code(PC)*** consideration of seven typical polar ice conditions related to the PC rule of International Association of Classification Societies(IACS),a prediction method of ice resistance is developed by Lindqvist's *** results are compared with those of Lindqvist's model and Riska's model by using two real ship *** comparison among propulsion requirements of representative classification societies is made,and a formula of minimum propulsion power is presented on the basis of ice resistance by revised Finnish-Swedish Ice Class Rules(FSICR)*** results are verified by the actual values from seven ice class ships.A relatively good agreement is *** a conclusion,the presented prediction method of ice resistance and minimum propulsion power is recommended for evaluation of ice resistance and its related propulsion power during the process of developing polar ships.
