Toxoplasma gondii is a widespread parasite that may infect nearly all warm-blooded vertebrates.1 As an obligatory intracellular apicomplexan parasite,T.gondii has three different types of secretory organelles,including micronemes,rhoptries(ROP),and dense granules(GRAs),during invasion of host cells.2 Initially,the parasite replicates in a variety of host cell types as tachyzoites,especially in macrophages and dendritic cells(DCs).

Metabolic syndrome （MetS） is a serious threat to public health worldwide with an increased risk of developing type 2 diabetes, cardiovascular diseases and all-cause morbidity and mortality. In this study, a urinary metabolomic approach was performed on high performance liquid chromatography quadrupole time-of-flight mass spectrometry to discriminate 36 male MetS patients and 36 sex and age matched healthy controls. Pattern recognition analyses （principal component analysis and orthogonal projections to latent structures discriminate analysis） commonly demonstrated the difference between MetS patients and no-MetS subjects. This study found 8 metabolites that showed significant changes in patients with MetS, including branch-chain and aromatic amino acids （leucine, tyrosine, phenylalanine and tryptophan）, short-chain acylcanitine （tiglylcamitine）, tricarboxylic acid （TCA） cycle intermediate （cis-aconitic acid） and glucuronidated products （cortolone-3-glucuronide and tetrahydroaldoster- one-3-glucuronide）. The candidate biomarkers revealed in this study could be useful in providing clues for further research focusing on the in-depth investigation of the cause of and cure for MetS.

Identification of tick-borne pathogens using metagenomic analyses in H.longicornis feeding on humans in downtown Beijing

On August 14th,2018,a Beijing resident living in Xicheng District found a female H.longicornis tick attached to the skin at the front of his upper shin.On examination,the patient was afebrile and appeared well.The species of the tick was identified through morphological characteristics and phylogenetic analysis based on cytochrome C oxidase subunit I.This H.longicornis tick was screened for tick-borne pathogens such as viruses,bacteria and parasites.RNA pathogens were screened by PCR and sequencing,while DNA pathogens were screened by metagenomic analyses.It was found that the tick was positive for the DNA sequences of zoonotic and animal pathogens such as A phagocytophilum,Ehrlichia minasensis and C.burnetii.Considering the good health condition of the patient,we hypothesized that the pathogens originated from the tick specimen itself rather than host blood meal.For the first time,our study reveals the possible risk of transmission of tick-borne pathogens to human beings through tick bit in downtown Beijing.Further research is needed to screen for tick-borne pathogens among unfed ticks collected from central Beijing.

Objective Solar ultraviolet （UV） radiation is an important environmental factor that affects human health. The understanding of diurnal variations of UV radiation at anatomical sites may be helpful in developing ways to protect humans from the harmful effects of UV radiation. Methods In order to characterize the diurnal variations, the UV exposure values were measured at 30 min intervals by using Solar-UV Sensors and a rotating manikin in Shenyang city of China （41°51＇N, 123°27%）. Measurement data for four representative days （in each of the four seasons respectively） were analyzed. Results The diurnal variations in solar UV radiation at the shoulder, the forehead and the chest were similar to those associated with a horizontal control measurement. However, the diurnal variations at the eye and the cheek exhibited bimodal distributions with two peaks in spring, summer and autumn, and a unimodal distribution in winter. The UV exposure peaks at the eye and the cheek were measured at solar elevation angles （SEA） of about 30° and 40°, respectively. Conclusion The protection of some anatomical sites such as the eye from high UV exposure should not be focused solely on the periods before and after noon, especially in the places and seasons with high SEA.

Hepatitis B is a major health concern in the Asia-Pacific region,and is endemic in China,Southeast Asia,and Africa.Chronic hepatitis B virus(HBV)infection may cause hepatic cirrhosis and liver cancer.It is estimated that there are more than 350 million chronic HBV carriers worldwide,of whom approximately one quarter will die of chronic hepatitis B-related liver diseases.HBV is transmitted horizontally through blood and blood products or by sexual transmission,and vertically from mother to infant.Perinatal infection is the predominant mode of transmission in countries with a high prevalence of hepatitis B surface antigen(HBsAg)carriage,and perinatal transmission leads to high rates of chronic infection.Therefore,it is important to prevent the mother-to-child transmission(MTCT)of HBV.Research has shown that pregnant women with high HBV DNA levels have an increased risk of MTCT.However,most of the obstetrics guidelines do not make a distinction between pregnant women with high HBV DNA levels and those who are HBsAg positive only.This review addresses the management of pregnant women with high levels of HBV viremia,in terms of antiviral therapy,use of hepatitis B immunoglobulin(HBIG),the combined application of hepatitis B vaccine and HBIG,choice of delivery mode and feeding practices.

Longitudinal Changes in Liver Aminotransferases Predict Metabolic Syndrome in Chinese Patients with Nonviral Hepatitis

Objective This study explored the correlation of longitudinal changes in serum alanine aminotransferase （ALT） and aspartate aminotransferase （AST） levels with the incidence of metabolic syndrome （Mets） based on a dynamic health examination cohort. Methods A Mets-free dynamic cohort involving 4541 participants who underwent at least three health examinations from 2006 to 2011 was included in the study. Mets was defined according to the Chinese Medical Association Diabetes Branch definition that included hypertension, obesity, hyperlipidemia, and hyperglycemia. Generalized estimating equation （GEE） model was used to analyze multivariate relative risk （RR） of repeated observations of ALT and AST in quartiles for Mets or its components according to gender. Results In all, 826 Mets cases were reported. Adjustment of relevant parameters indicated that time-varying changes in ALT and AST levels were positively associated with the incidence of Mets in a dose-response manner. Positive association between high ALT levels and fatty liver was much stronger than that between high AST levels and fatty liver, particularly in male participants. These associations were consistently observed in the following subgroups： participants with ALT and AST levels of 〈40 U/L, participants with of 〈25 kg/m2, and participants with non-fatty liver. Furthermore, participants with 2 Mets components at baseline showed lower multivariate adjusted RRs of ALT and AST for Mets than participants with 0-1 Mets component. Conclusion These results suggested that elevated serum ALT and AST levels were early biomarkers of Mets or its components.

Background:Following initiation of China’s National Malaria Elimination Action Plan(NMEAP)in 2010,the'1-3-7’approach was developed and rolled out in China to facilitate the malaria elimination programme and accelerate malaria elimination.This study aims to summarize and condense these experiences through a retrospective analysis in Jiangsu Province,which could be adapted and applied in other malaria elimination settings worldwide.Methods:A retrospective analysis of imported malaria cases into China identified through an improved surveillance and response system in Jiangsu Province was carried out for the period of 2001-2020.To improve the malaria surveillance and response system,Centers for Diseases Control and Prevention from the prefectures and counties in Jiangsu province conducted population-level health education to improve healthcare seeking behavior,strengthened capacity of health facilities to improve performance of malaria diagnosis and treatment,and raised the capacity of public health providers to improve implementation of the'1-3-7’approach.Categorical variables were carried out by Chi square tests with Fisher’s exact correction.Results:From 2001 to 2020,a total of 9,879 malaria cases were reported in Jiangsu Province.Since 2012,no indigenous malaria cases have been reported in Jiangsu Province.However,in recent years,there has been a substantial increase of imported falciparum malaria cases.Between 2012 and 2020,an estimated 61.57 million individuals have benefited from population-level health education in Jiangsu Province.For healthcare-seeking services among the 2,423 imported malaria cases,687(28.4%)and 1,104(45.6%)cases visited hospitals on the first day and the second day from symptom onset,respectively.A total of 1,502(61.9%)cases were diagnosed on the first day at medical facilities.Jiangsu Province achieved 100%,99.4%and 98.3%completion rate in terms of case detection and notification(within one day),case investigation(within three days)and foci response and disposition(within seven days),res

AIM: To investigate the association between nutrient intakes and high incidence rate of gastric cancer among residents in Zhoushan Islands.METHODS: A frequency-matched design of case-control study was used during the survey on dietary factors and gastric cancer in Zhoushan Islands, China. A total of 103 cases of gastric cancer diagnosed in 2001 were included in the study and 133 controls were randomly selected from the residents in Zhoushan Islands. A food frequency questionnaire was specifically designed for the Chinese dietary pattern to collect information on dietary intake. A computerized database of the dietary and other relative information of each participant was completed. Total calories and 15 nutrients were calculated according to the food composition table and their adjusted odds ratios (ORs) and 95% confidence intervals (CIs) were estimated by gender using unconditional logistic regression models.RESULTS: High intakes of protein, saturated fat, and cholesterol were observed with the increased risk of gastric cancer particularly among males (ORQ4 vsQ1 were 10.3, 3.24, 2.76 respectively). While carbohydrate was a significant high-risk nutrient (ORQ4 vsQ1 = 14.8; Pfor linear trend = 0.024) among females. Regardless of their gender, the cases reported significantly higher daily intake of sodium mainly from salts. As to the nutrients of vitamins A and C, an inversed association with the risk of GC was found. Baseline characteristics of participants were briefly described.CONCLUSION: The findings from this study confirm the role of diet-related exposure in the etiology of gastric cancer from the point of view of epidemiology. An increased risk of gastric cancer is associated with high intakes of protein, saturated fat, cholesterol and sodium,while consumption of polyunsaturated fat, vitamin A and ascorbic acid may have a protective effect against gastric cancer.

Tomato brown rugose fruit virus(ToBRFV)causes severe fruit loss in tomato(Solanum lycopersicum)and pepper(Capsicum annuum)plants.It is an emerging Tobamovirus that is spreading globally.The major challenge is to develop a reliable method for the detection of the virus,and to better characterize the symptoms it causes.The aims of this study,therefore,were to characterize the symptom development on tomato and pepper plants,and to establish a reliable detection method for the virus.Following infection of the tomato and pepper plants with ToBRFV,the leaves turned chlorotic,mosaic or mottled,while the fruit became rugose,necrotic and marbled,and showed discoloration with yellow or brown spots.Transmission electron microscopy(TEM)revealed single rod-like virus particles characteristic of the Tobamoviruses.Classical reverse transcription PCR(RT-PCR)and quantitative PCR(qPCR)with specific primers and probes confirmed that the virus is ToBRFV.We found that the resistance genes from tomato,Tm-2^(2),and pepper,L1,L2,L3 and L4,did not confer resistance to ToBRFV.Here,we present a PCR-based method as a diagnostic test for detecting ToBRFV in infected seeds.This method will help to prevent further spread of the virus in commercial seeds.
